Mammalian target of rapamycin(m TOR) inhibitor as an attractive drug target with promising antitumor effects has been widely investigated. High quality clinical trial has been conducted in liver transplant(LT) recipients in Western countries. However, the pertinent studies in Eastern world are paucity. Therefore, we designed a clinical trial to test whether sirolimus can improve recurrence-free survival(RFS) in hepatocellular carcinoma(HCC) patients beyond the Milan criteria after LT. This is an open-labeled, single-arm, prospective, multicenter, and real-world study aiming to evaluate the clinical outcomes of early switch to sirolimus-based regimens in HCC patients after LT. Patients with a histologically proven HCC and beyond the Milan criteria will be enrolled. The initial immunosuppressant regimens are center-specifc for the frst 4-6 weeks. The following regimens integrated sirolimus into the regimens as a combination therapy with reduced calcineurin inhibitors based on the condition of patients and centers. The study is planned for 4 years in total with a 2-year enrollment period and a 2-year follow-up. We predict that sirolimus conversion regimen will provide survival benefts for patients particular in the key indicator RFS as well as better quality of life. If the trial is conducted successfully, we will have a continued monitoring over a longer follow-up time to estimate indicator of overall survival. We hope that the outcome will provide better evidence for clinical decision-making and revising treatment guidelines based on Chinese population data.

Background Data on the epidemiology of hypertension in Chinese non-dialysis chronic kidney disease （CKD） patients are *** aim of the present study was to investigate the prevalence,awareness,treatment,and control of hypertension in the non-dialysis CKD patients through a nationwide,multicenter study in *** The survey was performed in 61 tertiary hospitals in 31 provinces,municipalities,and autonomous regions in China （except hong Kong,Macao,and Taiwan）.Trained physicians collected demographic and clinical data and measured blood pressure （BP） using a standardized *** was defned as systolic BP ≥140 mmHg and/or diastolic BP ≥90 mmHg,and/or use of antihypertensive *** 〈140/90 mmHg and 〈130/80 mmHg were used as the 2 thresholds of hypertension *** multivariate logistic regression with adjustment for sex and age,we analyzed the association between CKD stages and uncontrolled hypertension in non-dialysis CKD *** The analysis included 8927 non-dialysis CKD *** prevalence,awareness,and treatment of hypertension in non-dialysis CKD patients were 67.3%,85.8%,and 81.0%,*** hypertensive CKD patients,33.1% and 14.1% had controlled BP to 〈140/90 mmHg and 〈130/80 mmHg,*** successive CKD stages,the prevalence of hypertension in non-dialysis CKD patients increased,but the control of hypertension decreased （P〈0.001）.When the threshold of BP 〈130/80 mmHg was considered,the risk of uncontrolled hypertension in CKD 2,3a,3b,4,and 5 stages increased 1.3,1.4,1.4,2.5,and 4.0 times compared with CKD 1 stage,respectively （P〈0.05）.Using the threshold of 〈140/90 mmHg,the risk of uncontrolled hypertension increased in advanced stages （P〈0.05）.Conclusions The prevalence of hypertension Chinese non-dialysis CKD patients was high,and the hypertension control was *** successive CKD stages,the risk of uncontrolled hypertension increased.

The jiangmen Underground Neutrino Observatory(JUNO)features a 20 kt multi-purpose underground liquid scintillator sphere as its main *** of JUNO's features make it an excellent location for^8B solar neutrino measurements,such as its low-energy threshold,high energy resolution compared with water Cherenkov detectors,and much larger target mass compared with previous liquid scintillator *** this paper,we present a comprehensive assessment of JUNO's potential for detecting^8B solar neutrinos via the neutrino-electron elastic scattering process.A reduced 2 MeV threshold for the recoil electron energy is found to be achievable,assuming that the intrinsic radioactive background^(238)U and^(232)Th in the liquid scintillator can be controlled to 10^(-17)g/*** ten years of data acquisition,approximately 60,000 signal and 30,000 background events are *** large sample will enable an examination of the distortion of the recoil electron spectrum that is dominated by the neutrino flavor transformation in the dense solar matter,which will shed new light on the inconsistency between the measured electron spectra and the predictions of the standard three-flavor neutrino oscillation *** m^(2)_(21)=4.8times10^(-5);(7.5times10^(-5))eV^(2),JUNO can provide evidence of neutrino oscillation in the Earth at approximately the 3sigma(2sigma)level by measuring the non-zero signal rate variation with respect to the solar zenith ***,JUNO can simultaneously measureDelta m^2_(21)using^8B solar neutrinos to a precision of 20% or better,depending on the central value,and to sub-percent precision using reactor antineutrinos.A comparison of these two measurements from the same detector will help understand the current mild inconsistency between the value of Delta m^2_(21)reported by solar neutrino experiments and the KamLAND experiment.

JUNO is a multi-purpose neutrino observatory under construction in the south of *** publication presents new sensitivity estimates for the measurement of the △m_(31)^(2),△m_(21)^(2),sin^(2)θ_(12),and sin^(2)θ_(13) oscillation parameters using reactor antineutrinos,which is one of the primary physics goals of the *** sensitivities are obtained using the best knowledge available to date on the location and overburden of the experimental site,the nuclear reactors in the surrounding area and beyond,the detector response uncertainties,and the reactor antineutrino spectral shape constraints expected from the TAO satellite *** is found that the △m_(21)^(2) and sin^(2)θ_(12) oscillation parameters will be determined to 0.5%precision or better in six years of data *** the same period,the △m_(31)^(2) parameter will be determined to about 0.2%precision for each mass ordering *** new precision represents approximately an order of magnitude improvement over existing constraints for these three parameters.

Introduction One of main scientiﬁc goals of the Large High Altitude air Shower Observatory(LHAASO)is to accurately measure the energy spectra of different cosmic ray compositions around the‘knee’*** Wide Field-of-View(FoV)Cherenkov Telescope Array(WFCTA),which is one of the main detectors of LHAASO and has 18 telescopes,is built to achieve this *** telescopes are put together and point to connected directions for a larger *** Telescopes are deployed spatially as close as possible,but due to their own size,the distance between two adjacent telescopes is about 10 ***,the Cherenkov lateral distribution and the parallax between the two telescopes should be considered in the event building process for images crossing over the boundaries of FoVs of the *** event building method for Cherenkov images measured by multiple telescopes of WFCTA is *** performance of the shower measurements using the combined images is evaluated by comparing with showers that are fully contained by a virtual telescope in *** and conclusion It is proved that the developed event building process can help to increase the FoV of WFCTA by 30%while maintaining the same reconstruction quality,compared to the separate telescope reconstruction method.

KM2A is one of the main sub-arrays of LHAASO,working on gamma ray astronomy and cosmic ray physics at energies above 10 *** simulation is the important foundation for estimating detector performance and data *** is a big challenge to simulate the KM2A detector in the framework of Geant4 due to the need to track numerous photons from a large number of detector units(>6000)with large altitude difference(30)and huge coverage(1.3).In this paper,the design of the KM2A simulation code G4KM2A based on Geant4 is *** process of G4KM2A is optimized mainly in memory consumption to avoid memory *** simplifications are used to significantly speed up the execution of *** running time is reduced by at least 30 times compared to full detector *** particle distributions and the core/angle resolution comparison between simulation and experimental data of the full KM2A array are also presented,which show good agreement.

Theγ-ray emission from the W51 complex is widely acknowledged to be attributed to the interaction between the cosmic rays(CRs)accelerated by the shock of supernova remnant(SNR)W51C and the dense molecular clouds in the adjacent star-forming region,***,the maximum acceleration capability of W51C for CRs remains *** on observations conducted with the Large High Altitude air Shower Observatory(LHAASO),we report a significant detection ofγrays emanating from the W51 complex,with energies from 2 to 200 *** LHAASO measurements,for the first time,extend theγ-ray emission from the W51 complex beyond 100 TeV and reveal a significant spectrum bending at tens of *** combining the"π^(0)-decay bump"featured data from Fermi-LAT,the broadbandγ-ray spectrum of the W51 region can be well-characterized by a simple pp-collision *** observed spectral bending feature suggests an exponential cutoff at~400 TeV or a power-law break at~200 TeV in the CR proton spectrum,most likely providing the first evidence of SNRs serving as CR accelerators approaching the PeV ***,two young star clusters within W51B could also be theoretically viable to produce the most energeticγrays observed by *** findings strongly support the presence of extreme CR accelerators within the W51 complex and provide new insights into the origin of Galactic CRs.
