How has the disease course of pediatric ulcerative colitis changed throughout the biologics era?A comparison with the IBSEN study

BACKGROUND In Korea,infliximab was approved for use in children with ulcerative colitis(UC)in October *** To compare the clinical course of UC before and after the introduction of biological agents,and to compare with the IBSEN *** Patients under 18 years of age,who were diagnosed with UC and followed from January 2003 to October 2020,were included in the *** A(n=48)was followed for at least 2 years between January 2003 and October 2012,and Group B(n=62)was followed for at least 2 years between November 2012 and October *** compared endoscopic remission,drug composition,relapse rate,steroidfree period,and the quality of life of each *** plotted the clinical course of the included patients using the pediatric UC activity index score,and compared our patients with those in the IBSEN *** After 2 years of treatment,colonoscopy evaluation revealed different outcomes in the two treatment *** was confirmed in 14 patients(29.2%)of Group A,and in 31 patients(50.0%)of Group B(P<0.012).The median cumulative corticosteroid-free period was 3.0 years in Group A and 4.4 years in Group ***-free period of Group B was significantly longer than that of Group A(P<0.001).There was a statistically significant difference between the two groups in evaluation of the relapse rate during the observation period(P<0.001).The plotted clinical course graphs of Group A showed similar proportions to the graphs in the IBSEN ***,in Group B,the proportion of patients corresponding to curve 1(remission or mild severity after initial high activity)was high at 76%(47/62).CONCLUSION The incidence of relapse has decreased and the steroid-free period has increased after the introduction of the biological *** clinical course also showed a different pattern from that of IBSEN *** active use of biological agents may change the long-term disease course in moderate to severe pediatric UC.

Hair loss due to medical conditions,such as alopecia,male pattern baldness,and cancer chemotherapy treatment,has been a comm onproblem for many in *** n A(CsA),a fungal metabolite,has been reported to be a hair growth modulatory agent and is apotential drug for hair ***,the effect of topical application of CsA is limited by its poor water *** deliverysystems developed to enhance its solubility still showed poor skin *** overcome these limitations,in this study,we have developed a novel chitosan nanocapsule platform using Pluro nic F127 and chitosan without any chemical crosslinking or complicated preparati on steps for the enhanced water solubility and high tran sdermal penetration of *** chitosan nanocapsules(ChiNCs)optimized in terms of structural stability by using chitosan with various molecular weights ranging from 3 to 100 kDa enhanced the skin permeation of CsA through humancadaver skin in *** administration of the CsA loaded ChiNCs in creased the hair follicles by c.a.7 times higher than that of thecontrol group,and effectively induced hair growth in C57BL/6 mice in *** results suggest that ChiNCs could be used as a platform foreffective transdermal delivery of various hydrophobic drugs.

Strigolactones （SLs） constitute a new class of plant hormones which are active as germination stimulants for seeds of parasitic weeds of Striga, Orobanche, and Pelipanchi spp, in hyphal branching of arbuscular mycorrhizal （AM） fungi and as inhibitors of shoot branching. In this review, the focus is on molecular features of these SLs. The occurrence of SLs in root exudates of host plants is described. The naming protocol for SL according to the International Union of Pure and Applied Chemistry （IUPAC） rules and the ＇at a glance＇ method is explained. The total synthesis of some natural SLs is described with details for all eight stereoisomers of strigol. The problems encountered with assign- ing the correct structure of natural SLs are analyzed for orobanchol, alectrol, and solanacol. The structure-activity relationship of SLs as germination stimulants leads to the identification of the bioactiphore of SLs. Together with a tentative mechanism for the mode of action, a model has been derived that can be used to design and prepare active SL analogs. This working model has been used for the preparation of a series of new SL analogs such as Nijmegen-1, and analogs derived from simple ketones, keto enols, and saccharine. The serendipitous finding of SL mimics which are derived from the D-ring in SLs （appropriately substituted butenolides） is reported. For SL mimics, a mode of action is proposed as well. Recent new results support this proposal. The stability of SLs and SL analogs towards hydrolysis is described and some details of the mechanism of hydrolysis are discussed as well. The attempted isolation of the protein receptor for germination and the current status concerning the biosynthesis of natural SLs are briefly discussed. Some non-SLs as germinating agents are mentioned. The structure-activity relationship for SLs in hyphal branching of AM fungi and in repression of shoot branching is also analyzed. Biologic agents with various mechanisms against Crohn’s disease(CD)have been released and are widely used in clinical ***,two anti-tumor necrosis factor(TNF)agents,infliximab(IFX)and adalimumab(ADL),are the only biologic agents approved by the Food and Drug Administration for pediatric CD ***,in pediatric CD,the choice of biologic agents should be made more carefully to achieve the therapeutic *** are currently no headto-head trials of biologic agents in pediatric or adult *** is a lack of accumulated data for pediatric CD,which requires the extrapolation of adult data for the positioning of biologics in pediatric *** a pharmacokinetic point of view,IFX is more advantageous than ADL when the inflammatory burden is high,and ADL is expected to be advantageous over IFX in sustaining remission in the maintenance ***,we reviewed the safety profile,immunogenicity,preference,and compliance between IFX and ADL and provide practical insights into the choice of anti-TNF therapy in pediatric *** evaluation of clinical indications and disease behavior is essential when prescribing anti-TNF *** addition,factors such as the efficacy of induction and maintenance of remission,safety profile,immunogenicity,patient preference,and compliance play an important role in evaluating and selecting treatment options.

Applications of sediment transport and water flow characteristics based sediment transport simulation models for a river system are presented in this *** existing water-sediment model and a new sediment-water model are used to formulate the simulation models representing water and sediment movement in a river *** sediment-water model parameters account for water flow characteristics embodying sediment transport properties of a *** models are revised formulations of the multiple water inflows model describing water movement through a river system as given by the Muskingum *** models are applied to a river system in Mississippi River basin to estimate downstream sediment concentration,sediment discharge,and water *** system and the river section parameters are estimated using a revised and the original multiple water inflows models by applying the genetic *** models estimate downstream sediment transport rates on the basis of upstream sediment/water flow rates to a *** performance is evaluated by using standard statistical criteria;downstream water discharge resulting from the original multiple water inflows model using the estimated river system parameters indicate that the revised models satisfactorily describe water movement through a river *** obtained in the study demonstrate the applicability of the sediment transport and water flow characteristics-based simulation models in predicting downstream sediment transport and water flow rates in a river system.

We propose a new framework for entity and event extraction based on generative adversarial imitation learning-an inverse reinforcement learning method using a generative adversarial network(GAN).We assume that instances and labels yield to various extents of difficulty and the gains and penalties(rewards)are expected to be *** utilize discriminators to estimate proper rewards according to the difference between the labels committed by the ground-truth(expert)and the extractor(agent).Our experiments demonstrate that the proposed framework outperforms state-of-the-art methods.

Objective:To investigate the anti-obesity activity and the action mechanism of the roots of Adenophora triphylla *** extract(ATE) in high-fat diet(HFD)-induced obese mice and 3T3-LI ***:The roots of Ademption triphylla were extracted with 70%*** demonstrate the compounds,linoleic acid was analyzed by using gas chromatography:and the anti-obesity effects and possible mechanisms of ATE were examined in 3T3-L1 adipocytes and HFD-induced obese ***:Treatment with ATE inhibited the lipid accumulation without cytotoxicity in 3T3-L1 ***,200 and 400 mg/kg ATE treatment significantly decreased the body weight gain,white adipose tissues(WATs) weight and plasma triglyceride level,while 100 and 200 mg/kg ATE treatment increased the plasma high-density lipoprotein cholesterol level in the HFD-induced obese mice,as compared with the HFD *** with 200 and 400 mg/kg ATE also lowered the size of adipocytes in adipose tissue and reduced the lipid accumulation in *** treatment showed significantly lower expression level of adipogenesis-relatcd proteins,such as peroxisome proliferator-activated receptor y,fatty acid binding protein(aP2),fatty acid synthase in 3T3-L1 adipocytes;and furthermore,decreased peroxisome proliferatoractivated receptor γ,aP2,fatty acid synthase,sterol regulatory element binding protein-1c,and lipoprotein lipase mRNA expression levels in WAT of the HFD-induced obese ***:These results suggested that the ATE has an anti-obesity effect,which may be elicited by regulating the expression of adipogenesis and lipogenesis-related genes and proteins in adipocytes and WAT of the HFD-induced obese mice.

Purpose:The purpose of this study was to identify the knowledge and attitudes of oncology nurses toward clinical trials and to provide evidence for the development of clinical trial education programs for oncology ***:The study was conducted on 142 nurses who had more than six months of nursing experience working with cancer patients at a tertiary hospital in Seoul,Korea.A structured questionnaire was used to measure the knowledge and attitudes of oncology nurses toward clinical ***:The participants scored an average of 15.03±3.52 out of 19 in terms of knowledge about clinical *** terms of attitudes toward clinical trials,the participants scored an average of 5.91±1.37 out of *** was a significant positive correlation between the knowledge and attitudes of the participants toward clinical trials(r=0.23,P=0.007).Conclusion:This study found that there was a relationship between the knowledge and attitudes of oncology nurses toward clinical *** improve the competency of oncology nurses and provide high-quality care to patients participating in clinical trials,more systematic and sustainable education is required.
