Cryptogenic multifocal ulcerous stenosing enteritis: Radiologic features and clinical behavior

To investigate the characteristic radiologic findings of cryptogenic multifocal ulcerous stenosing enteritis (CMUSE) which can be differentiated from other similar bowel disease and to assess their clinical behavior. METHODSTwenty pathologically and clinically confirmed CMUSE patients (males:females = 8:12; mean age: 40.4 years) between March 2002 and August 2015 from seven academic centers in South Korea were retrospectively reviewed. We evaluated small bowel series (SBS; n = 25), computed tomography (CT) enterography (n = 21), magnetic resonance (MR) enterography (n = 2), and abdominopelvic CT (n = 18) images, focusing on enteric and perienteric manifestations. Any change in radiologic features during follow-up period was recorded. We evaluated clinical data including presenting symptoms, laboratory finding and presence of relapse from electronic medical records. Histopathologic findings were also evaluated. RESULTSThe main symptoms were abdominal pain (n = 12) and anemia (n = 10). All patients showed small bowel strictures (n = 52, mean: 2.6 per patient) on initial CT/MR, located in the ileum (n = 47) or jejunum (n = 5). Strictures showed short-length (mean: 10.44 mm) and circumferential bowel wall thickening (mean: 5.56 mm) with layered enhancement (n = 48) that were also noted on initial SBS (n = 36) with shallow ulcers (n = 10). Some ulcerative lesions or wall thickening progressed into strictures on follow-up SBS/CT, and some strictures revealed recurrent ulceration on follow-up SBS. There were no penetrating disease features like fistula or abscess and no gastrointestinal tract involvement except the small bowel. Nine patients experienced disease recurrence (median relapse-free period: 32 mo) even post-operatively. How has the disease course of pediatric ulcerative colitis changed throughout the biologics era?A comparison with the IBSEN study

BACKGROUND In Korea,infliximab was approved for use in children with ulcerative colitis(UC)in October *** To compare the clinical course of UC before and after the introduction of biological agents,and to compare with the IBSEN *** Patients under 18 years of age,who were diagnosed with UC and followed from January 2003 to October 2020,were included in the *** A(n=48)was followed for at least 2 years between January 2003 and October 2012,and Group B(n=62)was followed for at least 2 years between November 2012 and October *** compared endoscopic remission,drug composition,relapse rate,steroidfree period,and the quality of life of each *** plotted the clinical course of the included patients using the pediatric UC activity index score,and compared our patients with those in the IBSEN *** After 2 years of treatment,colonoscopy evaluation revealed different outcomes in the two treatment *** was confirmed in 14 patients(29.2%)of Group A,and in 31 patients(50.0%)of Group B(P<0.012).The median cumulative corticosteroid-free period was 3.0 years in Group A and 4.4 years in Group ***-free period of Group B was significantly longer than that of Group A(P<0.001).There was a statistically significant difference between the two groups in evaluation of the relapse rate during the observation period(P<0.001).The plotted clinical course graphs of Group A showed similar proportions to the graphs in the IBSEN ***,in Group B,the proportion of patients corresponding to curve 1(remission or mild severity after initial high activity)was high at 76%(47/62).CONCLUSION The incidence of relapse has decreased and the steroid-free period has increased after the introduction of the biological *** clinical course also showed a different pattern from that of IBSEN *** active use of biological agents may change the long-term disease course in moderate to severe pediatric UC.

Hair loss due to medical conditions,such as alopecia,male pattern baldness,and cancer chemotherapy treatment,has been a comm onproblem for many in *** n A(CsA),a fungal metabolite,has been reported to be a hair growth modulatory agent and is apotential drug for hair ***,the effect of topical application of CsA is limited by its poor water *** deliverysystems developed to enhance its solubility still showed poor skin *** overcome these limitations,in this study,we have developed a novel chitosan nanocapsule platform using Pluro nic F127 and chitosan without any chemical crosslinking or complicated preparati on steps for the enhanced water solubility and high tran sdermal penetration of *** chitosan nanocapsules(ChiNCs)optimized in terms of structural stability by using chitosan with various molecular weights ranging from 3 to 100 kDa enhanced the skin permeation of CsA through humancadaver skin in *** administration of the CsA loaded ChiNCs in creased the hair follicles by c.a.7 times higher than that of thecontrol group,and effectively induced hair growth in C57BL/6 mice in *** results suggest that ChiNCs could be used as a platform foreffective transdermal delivery of various hydrophobic drugs.

Infliximab vs adalimumab:Points to consider when selecting antitumor necrosis factor agents in pediatric patients with Crohn’s disease

Biologic agents with various mechanisms against Crohn’s disease(CD)have been released and are widely used in clinical ***,two anti-tumor necrosis factor(TNF)agents,infliximab(IFX)and adalimumab(ADL),are the only biologic agents approved by the Food and Drug Administration for pediatric CD ***,in pediatric CD,the choice of biologic agents should be made more carefully to achieve the therapeutic *** are currently no headto-head trials of biologic agents in pediatric or adult *** is a lack of accumulated data for pediatric CD,which requires the extrapolation of adult data for the positioning of biologics in pediatric *** a pharmacokinetic point of view,IFX is more advantageous than ADL when the inflammatory burden is high,and ADL is expected to be advantageous over IFX in sustaining remission in the maintenance ***,we reviewed the safety profile,immunogenicity,preference,and compliance between IFX and ADL and provide practical insights into the choice of anti-TNF therapy in pediatric *** evaluation of clinical indications and disease behavior is essential when prescribing anti-TNF *** addition,factors such as the efficacy of induction and maintenance of remission,safety profile,immunogenicity,patient preference,and compliance play an important role in evaluating and selecting treatment options.

Knowledge and Attitudes of Oncology Nurses Toward Clinical Trials-A Secondary Publication

Purpose:The purpose of this study was to identify the knowledge and attitudes of oncology nurses toward clinical trials and to provide evidence for the development of clinical trial education programs for oncology ***:The study was conducted on 142 nurses who had more than six months of nursing experience working with cancer patients at a tertiary hospital in Seoul,Korea.A structured questionnaire was used to measure the knowledge and attitudes of oncology nurses toward clinical ***:The participants scored an average of 15.03±3.52 out of 19 in terms of knowledge about clinical *** terms of attitudes toward clinical trials,the participants scored an average of 5.91±1.37 out of *** was a significant positive correlation between the knowledge and attitudes of the participants toward clinical trials(r=0.23,P=0.007).Conclusion:This study found that there was a relationship between the knowledge and attitudes of oncology nurses toward clinical *** improve the competency of oncology nurses and provide high-quality care to patients participating in clinical trials,more systematic and sustainable education is required.

Objective:To investigate the anti-obesity activity and the action mechanism of the roots of Adenophora triphylla *** extract(ATE) in high-fat diet(HFD)-induced obese mice and 3T3-LI ***:The roots of Ademption triphylla were extracted with 70%*** demonstrate the compounds,linoleic acid was analyzed by using gas chromatography:and the anti-obesity effects and possible mechanisms of ATE were examined in 3T3-L1 adipocytes and HFD-induced obese ***:Treatment with ATE inhibited the lipid accumulation without cytotoxicity in 3T3-L1 ***,200 and 400 mg/kg ATE treatment significantly decreased the body weight gain,white adipose tissues(WATs) weight and plasma triglyceride level,while 100 and 200 mg/kg ATE treatment increased the plasma high-density lipoprotein cholesterol level in the HFD-induced obese mice,as compared with the HFD *** with 200 and 400 mg/kg ATE also lowered the size of adipocytes in adipose tissue and reduced the lipid accumulation in *** treatment showed significantly lower expression level of adipogenesis-relatcd proteins,such as peroxisome proliferator-activated receptor y,fatty acid binding protein(aP2),fatty acid synthase in 3T3-L1 adipocytes;and furthermore,decreased peroxisome proliferatoractivated receptor γ,aP2,fatty acid synthase,sterol regulatory element binding protein-1c,and lipoprotein lipase mRNA expression levels in WAT of the HFD-induced obese ***:These results suggested that the ATE has an anti-obesity effect,which may be elicited by regulating the expression of adipogenesis and lipogenesis-related genes and proteins in adipocytes and WAT of the HFD-induced obese mice.

Effects of the Syzygium aromaticum L. extract on antioxidation and inhibition of matrix metalloproteinase in human dermal fibroblast

Objective: To investigate cosmetic potential of Syzygium aromaticum L.(S. aromaticum L.) and to determine its antioxidant and anti-wrinkling effects. Methods: Using highperformance liquid chromatography, eugenol component was quantitated. The antioxidant activity of S. aromaticum L. was analyzed by 2,2-diphenyl-1-picrylhydrazyl radical scavenging and superoxide dismutase like activities. To determine cell viability, elastase and matrix metalloproteinase-1(MMP-1) activity, human dermal fibroblasts(HS68) were treated with S. aromaticum L. The inhibitory effect of S. aromaticum L. on tumor necrosis factor alpha induced MMPs expression in HS68 was analyzed by realtime-PCR. Results: The eugenol content was confirmed in S. aromaticum L. S. aromaticum L. was observed to have high 2,2-diphenyl-1-picrylhydrazyl radical scavenging activity and superoxide dismutase like activity. S. aromaticum L. had no cytotoxicity against the HS68 and dose-dependently increased elastase inhibition. Moreover, S. aromaticum L. significantly decreased MMP-1 content and inhibited gene levels of MMP-1, MMP-2, MMP-3 and MMP-9. Conclusions: The findings suggest that S. aromaticum L. has great potential as a cosmeceutical ingredient with antioxidant and antiwrinkling effects.

Brain metabolism in patients with vegetative state after post-resuscitated hypoxic-ischemic brain injury： statistical parametric mapping analysis of F-18 fluorodeoxyglucose positron emission tomography

Background Hypoxic-ischemic brain injury （HIBI） after cardiopulmonary resuscitation is one of the most devastating neurological conditions that causing the impaired consciousness. However, there were few studies investigated the changes of brain metabolism in patients with vegetative state （VS） after post-resuscitated HIBI. This study aimed to analyze the change of overall brain metabolism and elucidated the brain area correlated with the level of consciousness （LOC） in patients with VS after post-resuscitated HIBI. Methods We consecutively enrolled 17 patients with VS after HIBI, who experienced cardiopulmonary resuscitation. Overall brain metabolism was measured by F-18 fluorodeoxyglucose positron emission tomography （F-18 FDG PET） and we compared regional brain metabolic patterns from 17 patients with those from 15 normal controls using voxel-by-voxel based statistical parametric mapping analysis. Additionally, we correlated the LOC measured by the JFK-coma recovery scale-revised of each patient with brain metabolism by covariance analysis. Results Compared with normal controls, the patients with VS after post-resuscitated HIBI revealed significantly decreased brain metabolism in bilateral precuneus, bilateral posterior cingulate gyrus, bilateral middle frontal gyri, bilateral superior parietal gyri, bilateral middle occipital gyri, bilateral precentral gyri （PFEw correctecd 〈0.0001 ）, and increased brain metabolism in bilateral insula, bilateral cerebella, and the brainstem （PFEWcorrectecd 〈0.0001 ）. In covariance analysis, the LOC was significantly correlated with brain metabolism in bilateral fusiform and superior temporal gyri （P uncorrected 〈0.005）. Conclusions Our study demonstrated that the precuneus, the posterior cingulate area and the frontoparietal cortex, which is a component of neural correlate for consciousness, may be relevant structure for impaired consciousness in patient with VS after post-resuscitated HIBI. Background Permanent vegetative state is defined as the impaired level of consciousness longer than 12 months after traumatic causes and 3 months after non-traumatic causes of brain injury. Although many studies assessed the cerebral metabolism in patients with acute and persistent vegetative state after brain injury, few studies investigated the cerebral metabolism in patients with permanent vegetative state. In this study, we performed the voxel-based analysis of cerebral glucose metabolism and investigated the relationship between regional cerebral glucose metabolism and the severity of impaired consciousness in patients with permanent vegetative state after acquired brain *** We compared the regional cerebral glucose metabolism as demonstrated by F-18 fluorodeoxyglucose positron emission tomography from 12 patients with permanent vegetative state after acquired brain injury with those from 12 control subjects. Additionally, covariance analysis was performed to identify regions where decreased changes in regional cerebral glucose metabolism significantly correlated with a decrease of level of consciousness measured by JFK-coma recovery scare. Statistical analysis was performed using statistical parametric *** Compared with controls, patients with permanent vegetative state demonstrated decreased cerebral glucose metabolism in the left precuneus, both posterior cingulate cortices, the left superior parietal lobule （Pcorrected 〈0.001）, and increased cerebral glucose metabolism in the both cerebellum and the right supramarginal cortices （Pcorrected 〈0.001）. In the covariance analysis, a decrease in the level of consciousness was significantly correlated with decreased cerebral glucose metabolism in the both posterior cingulate cortices （Puncorrected 〈0.005）.Conclusion Our findings suggest that the posteromedial parietal cortex, which are part of neural network for consciousness, may be relevant structure for pathophysiological mechanism in pat
