Background:Professional health organizations are not currently recommending Tai Ji Quan alongside aerobic exercise to treat *** aimed to examine the efficacy of Tai Ji Quan as antihypertensive lifestyle ***:Tai Ji Quan interventions published in english and Chinese were included when they involved healthy adults,reported pre-and postintervention blood pressure(BP),and had a non-exercise/non-diet control *** systematically searched 11 electronic databases for studies published through July 31,2018,yielding 31 qualifying controlled ***(1)evaluated the risk of bias and methodological study quality,(2)performed meta-regression analyses following random-effects assumptions,and(3)generated additive models representing the largest possible clinically relevant BP ***:Participants(n=3223)were middle-aged(56.6±15.1 years of age,mean±Sd)adults with prehypertension(systolic BP(SBP)=136.9±15.2 mmHg,diastolic BP(dBP)=83.4±8.7 mmHg).Tai Ji Quan was practiced 4.0±1.4 sessions/week for 54.0§10.6 min/session for 22.3±20.2 ***,Tai Ji Quan elicited significant reductions in SBP(-11.3 mmHg,95%CI:-14.6 to-8.0;d+=-0.75)and dBP(-4.8 mmHg,95%CI:-6.4 to-3.1;d+=-0.53)***(pes with hypertension,Tai Ji Quan trials published in english elicited SBP reductions of 10.4 mmHg and dBP reductions of 4.0 mmHg,which was half the magnitude of trials published in Chinese(SBP reductions of 18.6 mmHg and dBP reductions of 8.8 mmHg).Conclusion:Our results indicate that Tai Ji Quan is a viable antihypertensive lifestyle therapy that produces clinically meaningful BP reductions(i.e.,10.4 mmHg and 4.0 mmHg of SBP and dBP reductions,respectively)among individuals with *** magnitude of BP reductions can lower the incidence of cardiovascular disease by up to 40%.

随着科学与技术的发展，临床检验近年出现了许多新的检验项目，尤其是利用免疫学原理对某些浓度很低的特殊蛋白的检测，但这些检测的结果可以是方法特异的，一种方法结果可能无法与其他方法结果相比较。为实现标准化或一致化，不同免疫分析的方法设计需具有可比性，校准物需溯源至同一参考物质。新的欧盟法规要求体外诊断产品的校准物和控制物的定值需具有溯源性，此要求适用所有体外诊断产品，对全球制造商和临床实验室产生影响。溯源原理基于eN ISO 17511，方法间结果的可比性有赖于合理设计的参考系统。此参考系统包括下列基本要素：足够明确的分析物定义，参考测量程序，适当的一级和次级参考物质，经过认可的参考实验室网络，临床溯源性相关信息，以及测量不确定度表述等。溯源性通过不间断的校准链（溯源链）实现，后者由交替出现的测量程序和校准物质组成。SI溯源链始于分析物的SI单位定义，随后通过一级参考测量程序和一级参考物质传递SI单位。量值的准确性或正确性源于一级参考测量程序和一级参考物质，但由于一级参考物质价格昂贵，一级参考测量程序无法在常规检测中应用，尚需中间的校准物和方法将准确性传递至产品校准物，最终至患者样品。每一次传递都会增加误差或不确定度，因此溯源链的传递步骤越少越好。正确的传递链要求每一测量程序的检测特异性和每一校准品的互通性，达不到这些要求会使溯源链中断。制造商还需确定校准品定值的不确定度并将这一信息提供给用户，在计量学上不确定度与定值同样重要。临床实验室位于溯源链的终端，结果的溯源性有赖于制造商所使用和描述的参考测量程序和参考物质。建立校准物的溯源性和改进检验结果的可比性需要标准化计划，这意味着使检测结果尽可能接近上一级参考测量程序得到的真值。采用较低级别的公认方法为特定的参考物质定值时，上述过程表述为“一致化”，而不是“标准化”，因为一致的结果不一定是真值。为整理和公布检验医学现有较高级别参考物质、参考测量程序和参考实验室，国际计量局（BIPM）、国际临床化学联合会（IFCC）和国际实验室认可协会（ILAC）成立检验医学溯源联合委员会（简称JCTLM）。JCLTM的主要任务是通过实施参考系统，建立检验结果溯源性，改进其可比性，从而提高医疗卫生质量并降低费用。JCTLM设立工作框架，按照国际标准（ISO 17511，15193，15194）建立参考物质和参考测量程序认定步骤。工作任务被很快确立并付诸实施，包括建立参考物质和参考测量程序列表，建立参考实验室网络，建立新的参考物质和参考测量程序的审批程序，以及合作研发新的参考物质和参考测量程序。

海洋硅循环是海洋生物地球化学循环的关键过程之一,对调控全球二氧化碳浓度、海洋酸碱度和多种元素(氮、磷、铁、铝等)的循环具有重要作用。在当今气候变化和人类活动影响日益增强的背景下,硅循环与“生物泵”及碳循环的紧密联系,是其成为地球科学领域研究热点的主要原因。海洋中硅的外部来源主要为河流、地下水、大气沉降、海底玄武岩风化作用和海底热液输送5个途径,在全球气温变暖趋势的影响下,极地冰川融化成为高纬度海域不可忽视的硅源。生物硅在沉积物中的埋藏、硅质海绵和生物硅的反风化作用是重要的海洋硅移除过程。海洋硅循环过程复杂,受生物(生物吸收、降解)、物理(吸附、溶解)和化学(矿化分解和反风化作用)多重因素的影响,针对海洋硅循环关键过程的研究有助于综合评估海洋硅的“源-汇”和收支。本文总结了海洋硅循环的主要过程及海洋硅的收支,根据国际和国内研究现状讨论了当前海洋硅循环研究中面临的主要问题和挑战。现有研究成果显示,海洋硅的外源输入和输出通量比以往的评估分别增加了2.4和2.2倍。在短时间尺度内(<8 ka),全球海洋中硅的收支大致平衡,海洋硅循环基本处于稳定状态。气候变化和人类活动导致河流输送至陆架边缘海的硅通量发生变化,可能影响硅藻等海洋浮游植物种群结构,是未来海洋硅循环研究需要关注的问题之一。陆架边缘海较高沉积速率和强烈的反风化作用提高了该区域生物硅的埋藏效率,准确评估该区域生物硅的埋藏通量仍是亟须解决的难题。目前的研究评估了全球海洋浮游硅藻、硅质海绵以及放射虫生产力,而海洋底栖硅藻生产力的贡献受到忽视,未来需要关注底栖硅藻对生物硅的贡献及其在海洋硅的生物地球化学过程中的作用。

Topical formulations, commonly applied for treatment of anterior eye diseases, require frequent administration due to rapid clearance from the ocular surface, typically through the lacrimal drainage system or through over-spillage onto the lids. we report on a mucoadhesive nanoparticle drug delivery system that may be used to prolong the precorneal residence time of encapsulated drugs. The nanoparticles were formed from self-assembly of block copolymers composed of poly（d, L-lactide） and dextran. The enhanced mucoadhesion properties were achieved by surface functionalizing the nanoparticles with phenylboronic acid. The nanoparticles encapsulated up to 12 wt.% of Cyclosporine A （CycA） and sustained the release for up to five days at a clinically relevant dose, which led us to explore the therapeutic efficacy of the formulation with reduced administration frequency. By administering CycA-loaded nanoparticles to dry eye-induced mice once a week, inflammatory infiltrates were eliminated and the ocular surface completely recovered. The same once a week dosage of the nanoparticles also showed no signs of physical irritation or inflammatory responses in acute （1 week） and chronic （12 weeks） studies in healthy rabbit eyes. These findings indicate that the nanoparticles may significantly reduce the frequency of administration for effective treatment of anterior eye diseases without causing ocular irritation.

Improved Interpretive Accuracy of Sugar Content on Beverage Labels Following an educational Intervention for Children

To address childhood obesity, it is important to help children make better choices with regards to intake of sugary drinks and snacks. The purpose of this study was to evaluate the validity of a novel educational intervention designed to convert grams of sugar into teaspoons on labeled beverages. A convenience sample of children （n = 21） at a charter elementary school in Washington, d. C was studied. A pre-test required an estimation of teaspoons of sugar on labels, followed by a practice demonstration and song. A post-test was given requiring estimation of teaspoons of sugar in on the same labels. Two weeks later, a retention test as well as qualitative questions assessed participant learning and information usage. Test scores revealed increased accuracy from pre-test （26.2%） to post-test （79.8%）. estimations were more accurate after the demonstration, as evidenced by the decreased range in average variation of responses （＋25.6 tsp to ＋1.8 tsp）. Two-week recall maintained accuracy （66.7%）, with an average variance of ＋3.8 teaspoons. Qualitative responses revealed participants had learned and used the conversion concepts. These results suggest that the educational intervention was effective. Qualitative data suggest behavior change, increased awareness, and the potential for change extending to caregivers.

Background: Pediatric patients with type 1 diabetes (T1d) have increased risk for low bone mineral density, which may be due in part to low 25-hydroxyvitamin d levels. Vitamin d levels are influenced by sunlight exposure and thus display geographical variation. we hypothesize that the prevalence of 25-hydroxyvitamin d deficiency (d insufficiency (20 - 29 ng/mL) in children with T1d living in the United States is higher than in healthy children and that diabetes duration, HbA1c, and insulin dose/kg are inversely associated with 25-hydroxyvitamin d levels. Methods: Medical records of patients with T1d being followed in Tennessee were reviewed for demographics, medical information, and 25-hydroxyvitamin d levels during the previous 2 years. Control subjects were obtained from a de-identified database of healthy pediatric subjects living in a similar geographical area. Chi squared tests and multivariable linear regression were performed. Results: Children and adolescents with T1d (n = 276;median age 14 years) have a significantly higher percentage of vitamin d deficiency and insufficiency compared with healthy pediatric controls (n = 100;median age 11.2 years) (68% versus 44%;p he median 25-hydroxyvitamin d level is 24 ng/mL versus 31 ng/mL, respectively. After adjusting for age, race, gender, UV light exposure, BMI, and multivitamin supplementation, children and adolescents with T1d have a serum 25-hydroxyvitamin d level 6.7 ng/mL lower than the control population [CI(4.11, 9.21), p he T1d population, there is no clear association between diabetes?duration, HbA1c, or insulin dose/kg and 25-hydroxyvitamin d levels. Conclusions: There is an increased prevalence of 25-hydroxyvitamin d deficiency and insufficiency in US. children with T1d compared with geographically similar children without diabetes.
