Tai Ji Quan as antihypertensive lifestyle therapy:A systematic review and meta-analysis

Background:Professional health organizations are not currently recommending Tai Ji Quan alongside aerobic exercise to treat *** aimed to examine the efficacy of Tai Ji Quan as antihypertensive lifestyle ***:Tai Ji Quan interventions published in English and Chinese were included when they involved healthy adults,reported pre-and postintervention blood pressure(BP),and had a non-exercise/non-diet control *** systematically searched 11 electronic databases for studies published through July 31,2018,yielding 31 qualifying controlled ***(1)evaluated the risk of bias and methodological study quality,(2)performed meta-regression analyses following random-effects assumptions,and(3)generated additive models representing the largest possible clinically relevant BP ***:Participants(n=3223)were middle-aged(56.6±15.1 years of age,mean±SD)adults with prehypertension(systolic BP(SBP)=136.9±15.2 mmHg,diastolic BP(DBP)=83.4±8.7 mmHg).Tai Ji Quan was practiced 4.0±1.4 sessions/week for 54.0§10.6 min/session for 22.3±20.2 ***,Tai Ji Quan elicited significant reductions in SBP(-11.3 mmHg,95%CI:-14.6 to-8.0;d+=-0.75)and DBP(-4.8 mmHg,95%CI:-6.4 to-3.1;d+=-0.53)***(p<0.001).Controlling for publication bias among samples with hypertension,Tai Ji Quan trials published in English elicited SBP reductions of 10.4 mmHg and DBP reductions of 4.0 mmHg,which was half the magnitude of trials published in Chinese(SBP reductions of 18.6 mmHg and DBP reductions of 8.8 mmHg).Conclusion:Our results indicate that Tai Ji Quan is a viable antihypertensive lifestyle therapy that produces clinically meaningful BP reductions(i.e.,10.4 mmHg and 4.0 mmHg of SBP and DBP reductions,respectively)among individuals with *** magnitude of BP reductions can lower the incidence of cardiovascular disease by up to 40%.

Desiccation tolerance （DT）, the ability to survive loss of more than 90% of cellular water （≤0.1 g H2O g^-1 dry mass） for prolonged periods, is common in seeds （90% of angiosperm species） but rare in vegetative tissues （0.04% of land plants; reviewed in Gaff and Oliver, 2013）. Vegetative DT is more widespread in non-tracheophytes, being present in 25 species of algae, 158 bryophytes species, and all lichens, but is rare in tracheophytes, being displayed in 64 pteridophyte and 135 angiosperm species （Gaff and Oliver, 2013）.

Biliary complications following liver transplantation: Singlecenter experience over three decades and recent risk factors

AIM To identify independent risk factors for biliary complications in a center with three decades of experience in liver *** A total of 1607 consecutive liver transplantations were analyzed in a retrospective study. Detailed subset analysis was performed in 417 patients, which have been transplanted since the introduction of Model of End-Stage Liver Disease(MELD)-based liver allocation. Risk factors for the onset of anastomotic biliary complications were identified with multivariable binary logisticregression analyses. The identified risk factors in regression analyses were compiled into a prognostic model. The applicability was evaluated with receiver operating characteristic curve analyses. Furthermore, Kaplan-Meier analyses with the log rank test were applied where appropriate. RESULTS Biliary complications were observed in 227 cases(14.1%). Four hundred and seventeen(26%) transplantations were performed after the introduction of MELD-based donor organ allocation. Since then, 21%(n = 89) of the patients suffered from biliary complications, which are further categorized into anastomotic bile leaks [46%(n = 41)], anastomotic strictures [25%(n = 22)], cholangitis [8%(n = 7)] and non-anastomotic strictures [3%(n = 3)]. The remaining 18%(n = 16) were not further classified. After adjustment for all univariably significant variables, the recipient MELD-score at transplantation(P = 0.006; OR = 1.035; 95%CI: 1.010-1.060), the development of hepatic artery thrombosis post-operatively(P = 0.019; OR = 3.543; 95%CI: 1.233-10.178), as well as the donor creatinine prior to explantation(P = 0.010; OR = 1.003; 95%CI: 1.001-1.006) were revealed as independent risk factors for biliary complications. In seropositive rheumatoid arthritis(RA),the onset of clinically apparent inflammatory arthritis(IA)is typically preceded by a prolonged period of autoimmunity manifest by the presence of circulating autoantibodies that can include antibodies to citrullinated protein antigens(ACPA)and rheumatoid *** period before clinical IA can be designated preclinical RA in those individuals who have progressed to a clinical diagnosis of RA,and an“at‐risk”status in those who have not developed IA but exhibit predictive biomarkers of future clinical *** the goal of developing RA prevention strategies,studies have characterized immune phenotypes of preclinical RA/at-risk *** these studies,a model has emerged wherein mucosal inflammation and dysbiosis may lead first to local autoantibody production,which should normally be transient,but instead is followed by a systemic spread of the autoimmunity as manifested by serum autoantibody elevations,ultimately driving the development of clinically identified joint *** model can be envisioned as the progression of disease development through serial“checkpoints”that in principle should constrain or resolve autoimmunity;however,instead,the checkpoints“fail”and clinical RA *** we review the immune processes that are likely to be present at each step and the potential therapeutic strategies that could be envisioned to delay,diminish,halt,or even reverse the progression to clinical ***,these prevention strategies could utilize existing therapies approved for clinical RA,therapies approved for other diseases that target relevant pathways in the preclinical/at-risk state,or approaches that target novel pathways.

Background:Parkinson’s disease(PD)is often accompanied by clinically identified *** effective pharmacotherapies that concomitantly treat both motor and psychological symptoms can pose a challenge to *** this reason,alternatives to standard anti-depressant treatments,such as repetitive transcranial magnetic stimulation(rTMS),have been evaluated within the Parkinson’s ***:A literature search was conducted on the PubMed database for all studies that evaluated rTMS as a treatment in patients with both depression and PD.A meta-analysis was performed on all studies that reported mean pre-and post-rTMS depression inventory *** used depression inventories included both self-report and clinician-administered *** size for individual study groups and across all studies was ***:Six of 7 studies meeting inclusion criteria reported significantly improved depression scores,large effect sizes,and significant *** weighted average effect size was calculated at 1.32 across all study groups that applied ***:Across all but one study,rTMS appears to effectively reduce depression scores among self-reported and clinician administered *** total weight average effect size showed that,when considering study sample sizes and degree of findings,this form of neurostimulation can relieve PD patients of their depressive ***,rTMS is a promising alternative to traditional anti-depressant therapies when treating refractory depression in patients with PD.
