First Trimester Preeclampsia Screening and Prevention:Perspective in Chinese Mainland

Preeclampsia(PE),a multisystem disorder in pregnancy,is one of the leading causes of perinatal morbidity and mortality that poses financial and physical burdens worldwide.Preterm PE with delivery at<37 weeks of gestation is associated with a higher risk of adverse maternal and perinatal outcomes than term PE with delivery at≥37 weeks of gestation.A myriad of first trimester screening models have been developed to identifying women at risk of preterm PE.In fact,the Fetal Medicine Foundation(FMF)first trimester prediction model has undergone successful internal and external validation.The FMF triple test enables the estimation of patient-specific risks,using Bayes theorem to combine maternal characteristics and medical history together with measurements of mean arterial pressure,uterine artery pulsatility index,and serum placental growth factor.Establishing a quality control process for regular monitoring and to ensure data standardization,reliability,and accuracy is key to maintaining optimal screening performance.The rate of preterm PE can be reduced by 62%by using the FMF prediction model,followed by the administration of low-dose aspirin.Recent evidence has also demonstrated that metformin has the potential for preventing PE in patients at high-risk of the disorder.In this article,we will summarize the existing literature on the different screening methods,different components of risk assessment,therapeutic interventions,and clinical implementation of the first trimester screening and prevention program for PE with specific considerations for Chinese mainland.

Emergency management capacity assessment for urban rail transit-an example of Beijing Metro Line 13

In order to improve the emergency management capability of urban rail transit system and reduce accidents during metro operation,an emergency management capability evaluation method combining analytic hierarchy process(AHP)and technique for order preference by similarity to ideal solution(TOPSIS)is proposed.Based on the Prevention Preparation Response Recovery(PPRR)model,factors influencing the emergency management capability of the urban rail transit system are summarized from the perspective of‘human,machine,environment and management’.Then,an emergency management capability evaluation index system containing of 20 secondary indicators is constructed in four stages:emergency prevention,emergency preparation,emergency response and emergency recovery.The weights of indicators are calculated using the AHP method,and the closeness of each indicator to the optimal solution is analysed with the TOPSIS method.Finally,take the Beijing Metro Line 13 as an example to investigate the level of emergency management capability of urban rail transit.The results show that the emergency management capability of Beijing’s urban rail transit system is‘well’,among which hazard prevention measures(0.31)and emergency response team(0.34)have a greater weight on the emergency management capability of rail transit.The model can more accurately assess the emergency management capability of urban rail transit and provide a basis for emergency management.

Computed tomography radiogenomics:A potential tool for prediction of molecular subtypes in gastric stromal tumor

BACKGROUND Preoperative knowledge of mutational status of gastrointestinal stromal tumors(GISTs)is essential to guide the individualized precision therapy.AIM To develop a combined model that integrates clinical and contrast-enhanced computed tomography(CE-CT)features to predict gastric GISTs with specific genetic mutations,namely KIT exon 11 mutations or KIT exon 11 codons 557-558 deletions.METHODS A total of 231 GIST patients with definitive genetic phenotypes were divided into a training dataset and a validation dataset in a 7:3 ratio.The models were constructed using selected clinical features,conventional CT features,and radiomics features extracted from abdominal CE-CT images.Three models were developed:ModelCT sign,modelCT sign+rad,and model CTsign+rad+clinic.The diagnostic performance of these models was evaluated using receiver operating characteristic(ROC)curve analysis and the Delong test.RESULTS The ROC analyses revealed that in the training cohort,the area under the curve(AUC)values for model_(CT sign),model_(CT sign+rad),and modelCT_(sign+rad+clinic)for predicting KIT exon 11 mutation were 0.743,0.818,and 0.915,respectively.In the validation cohort,the AUC values for the same models were 0.670,0.781,and 0.811,respectively.For predicting KIT exon 11 codons 557-558 deletions,the AUC values in the training cohort were 0.667,0.842,and 0.720 for model_(CT sign),model_(CT sign+rad),and modelCT_(sign+rad+clinic),respectively.In the validation cohort,the AUC values for the same models were 0.610,0.782,and 0.795,respectively.Based on the decision curve analysis,it was determined that the model_(CT sign+rad+clinic)had clinical significance and utility.CONCLUSION Our findings demonstrate that the combined modelCT_(sign+rad+clinic)effectively distinguishes GISTs with KIT exon 11 mutation and KIT exon 11 codons 557-558 deletions.This combined model has the potential to be valuable in assessing the genotype of GISTs.

Handily etching nickel foams into catalyst-substrate fusion self‐stabilized electrodes toward industrial‐level water electrolysis

The key challenge of industrial water electrolysis is to design catalytic electrodes that can stabilize high current density with low power consumption(i.e.,overpotential),while industrial harsh conditions make the balance between electrode activity and stability more difficult.Here,we develop an efficient and durable electrode for water oxidation reaction(WOR),which yields a high current density of 1000 mA cm−2 at an overpotential of only 284 mV in 1M KOH at 25°C and shows robust stability even in 6M KOH strong alkali with an elevated temperature up to 80°C.This electrode is fabricated from a cheap nickel foam(NF)substrate through a simple one-step solution etching method,resulting in the growth of ultrafine phosphorus doped nickel-iron(oxy)hydroxide[P-(Ni,Fe)O_(x)H_(y)]nanoparticles embedded into abundant micropores on the surface,featured as a self-stabilized catalyst–substrate fusion electrode.Such self-stabilizing effect fastens highly active P-(Ni,Fe)O_(x)H_(y)species on conductive NF substrates with significant contribution to catalyst fixation and charge transfer,realizing a win–win tactics for WOR activity and durability at high current densities in harsh environments.This work affords a cost-effective WOR electrode that can well work at large current densities,suggestive of the rational design of catalyst electrodes toward industrial-scale water electrolysis.

A mechanistic study on the regioselective Ni-catalyzed methylation–alkenylation of alkyne with AlMe3 and allylic alcohol

The recently reported Ni-catalyzed methylation–allylation of alkynes with allylic alcohols and AlMe3 reagents delivers valuable tetrasubstituted alkene units in a highly regioselective fashion.Motivated by the experimental significance and the mechanistic ambiguity(e.***.the details of C–O bond activation,the function of the Lewis acid AlMe3,etc.),we conducted a detailed mechanistic study using density functional theory(DFT)calculations.The reaction was found to occur via the C–O bond activation of allylic alcohol,allylation of alkyne,transmetalation,reductive elimination and catalyst regeneration steps.AlMe3 could easily react with the allylic alcohol to form a cyclodialuminoxane species bearing two groups of Lewis acid–base interactions.In this regard,the C(allyl)–O bond is remarkably activated,resulting in a relatively facile C–O activation(compared to the typical Lewis-acid mediated reactions).In addition,the Al–Me interaction is a double-edged sword to the methylation step:the cyclodialuminoxane group functions as the methyl resource,while the full dissociation of the Al–Me interaction(via a cis-to-trans isomerization)is a requisite to fully release the methyl group.In this context,the“Me-relay”pathway involving the[Ni]–Me intermediate is found to be more plausible than all other mechanistic possibilities(such as the typical concerted Me-transfer mechanism in previous studies).

Success rate of current human-derived gastric cancer organoids establishment and influencing factors:A systematic review and meta-analysis

BACKGROUND Human-derived gastric cancer organoids(GCOs)are widely used in gastric cancer research;however,the culture success rate is generally low.AIM To explore the potential influencing factors,and the literature on successful culture rates of GCOs was reviewed using meta-analysis.METHODS PubMed,Web of Science,and EMBASE were searched for studies.Two trained researchers selected the studies and extracted data.STATA 17.0 software was used for meta-analysis of the incidence of each outcome event.The adjusted Methodological Index for Non-Randomized Studies scale was used to assess the quality of the included studies.Funnel plots and Egger’s test were used to detect publication bias.Subgroup analyses were conducted for sex,tissue source,histo-logical classification,and the pathological tumor-node-metastasis(pTNM)cancer staging system.RESULTS Eight studies with a pooled success rate of 66.6%were included.GCOs derived from women and men had success rates of 67%and 46.7%,respectively.GCOs from surgery or biopsy/endoscopic submucosal dissection showed success rates of 70.9%and 53.7%,respectively.GCOs of poorly-differentiated,moderately-differentiated and signet-ring cell cancer showed success rates of 64.6%,31%,and 32.7%,respectively.GCOs with pTNM stages I-II and III-IV showed success rates of 38.3%and 65.2%,respectively.Y-27632 and non-Y-27632 use showed success rates of 58.2%and 70%,respectively.GCOs generated with collagenase were more successful than those constructed with Liberase TH and TrypLE(72.1%vs 71%,respectively).EDTA digestion showed a 50%lower success rate than other methods(P=0.04).CONCLUSION GCO establishment rate is low and varies by sex,tissue source,histological type,and pTNM stage.Omitting Y-27632,and using Liberase TH,TrypLE,or collagenase yields greater success than EDTA.

Fatty acid feedstocks enable a highly efficient glyoxylate-TCA cycle for high-yield production of β-alanine

Metabolic engineering to produce tricarboxylic acid(TCA)cycle-derived chemicals is usually associated with problems of low production yield and impaired cellular metabolism.In this work,we found that fatty acid(FA)feedstocks could enable high-yield production of TCA cycle-derived chemicals,while maintaining an efficient and balanced metabolic flux of the glyoxylate-TCA cycle,which is favorable for both product synthesis and cell growth.Here,we designed a novel synthetic pathway for production of β-alanine,an important TCA cycle-derived product,from FAs with a high theortecial yield of 1.391 g/g.By introducing panD,improving aspA,and knocking out iclR,glyoxylate shunt was highly activated in FAs and the yield of β-alanine reached 0.71 g/g from FAs,much higher than from glucose.Blocking the TCA cycle at icd/sucA/fumAC nodes could increase β-alanine yield in a flask cultivation,but severely reduced cell growth and FA utilization during fed-batch processes.Replenishing oxaloacetate by knocking out aspC and recovering fumAC could restore the growth and lead to a titer of 35.57 g/l.After relieving the oxidative stress caused by FA metabolism,β-alanine production could reach 72.05 g/l with a maximum yield of 1.24 g/g,about 86% of the theoretical yield.Our study thus provides a promising strategy for the production of TCA cycle-derived chemicals.

Honokiol attenuates acetaminophen-induced acute liver injury by inhibiting hepatic CYP1A2 activity and improving liver mitochondrial dysfunction

Objective:Acetaminophen(APAP)overdose is a common cause of liver injury.This study aimed to investigate the protective effect of honokiol(Hon)against APAP-induced hepatotoxicity and its potential mechanism.Methods:C57BL/6 mice were administrated with Hon(10 and 30 mg/kg)after APAP(300 mg/kg)treatment.On 1.5 h and 5 h after Hon treatment,mice were sacrificed.Serum and liver were collected.And then,liver injury-related indexes,APAP metabolism-related indexes,mitochondrial respiratory chain function-related indexes,and mitochondrial membrane function-related protein expression were evaluated.Results:It was found that Hon significantly decreased serum alanine aminotransferase(ALT)/aspartate aminotransferase(AST)activity and glutathione(GSH)depletion,increased hepatic catalase(CAT)and GSH peroxidase(GSH-Px)activities,reduced hepatic MDA and 3-nitrotyrosine contents,inhibited hepatic CYP1A2 activity and APAP protein adducts(APAP-CYS)formation.Meanwhile,oxidative phosphorylation capacity of complex I and electron transfer capacity of complex IV in mitochondrial respiratory chain was increased,whereas the release of H2O2 in the mitochondria was decreased following Hon treatment.Furthermore,Hon markedly down-regulated p-JNK in both cytosol and mitochondria,and obviously inhibited the release of apoptosis inducing factor(AIF)and endonuclease G(EndoG)from mitochondria to cytosol.Conclusion:Hon alleviated APAP-induced liver injury through the following pathways:Reducing the production of APAP-CYS by inhibiting CYP1A2 activity;Ameliorating hepatic oxidative stress by increasing the levels of hepatic CAT,GSH-Px and GSH;Improving mitochondrial respiratory chain function by promoting oxidative phosphorylation capacity of complex I and electron transfer capacity of complex IV;Improving the function of mitochondrial membrane by inhibiting p-JNK and its translocation to mitochondria,thereby reducing the release of AIF and EndoG.

Target serum concentration of vancomycin may be reached earlier with a loading dose

Background:Vancomycin treatment failure against vancomycin-susceptible gram-positive cocci is not rare in the intensive care unit(ICU).One of the reasons for this is the substandard drug trough concentration.We aimed to examine the hypothesis that the target serum concentration could be reached earlier with a loading dose of vancomycin.Methods:This retrospective cohort study was conducted at our ICU between June 2018 and June 2020 and involved patients who were suspected of having,or confirmed to have,gram-positive cocci infection and treated with vancomycin.One group of the patients was administered a loading dose of vancomycin(loading group)and compared with the group that did not receive a loading dose(control group).The baseline characteristics,vancomycin serum concentrations,and clinical outcomes were collected and analyzed.Results:Fifty-five patients were finally included,of which 29 received a loading dose of vancomycin.The serum concentration of vancomycin before the second dose was significantly higher for the loading group than for the control group(10.3±6.1 mg/Lvs.5.7±4.4 mg/L,P=0.002).The results for both groups were similar before the fifth dose(12.4±7.3 mg/Lvs.10.3±6.3 mg/L in the loading and the control groups,respectively;P=0.251).The 28-day mortality was lower for the loading group than for the control group(6.7%vs.34.6%in the loading and control groups,respectively;P=0.026).No significant differences were observed in serum creatinine(Cr)concentrations of the two groups.Conclusion:With the loading dose of vancomycin,the target serum concentration of vancomycin may be reached earlier without increasing the risk of acute kidney injury.

Effect of Paclobutrazol Application on Plant Growth and Flower Quality in Herbaceous Peony

Herbaceous peony(Paeonia lactiflora Pall.)is an important ornamental plant worldwide.In its natural state,P.lactiflora often manifests traits like rapidly elongating internodal growth,loose plant types,and soft inflorescence stems.However,very little has been known about the measures for controlling these traits.This study investigated the effect of applying paclobutrazol(PBZ)on the plant growth and flower quality in P.lactiflora.The results indicated that PBZ application reduced the plant height(8.05%),plant crown width(14.72%),and leaf area(10.90%),but increased the leaf thickness(18.18%)and stem diameter(over 11%)in P.lactiflora.Meanwhile,PBZ application was also found to increase the chlorophyll(Chl)a(29.63%),Chl b(33.33%),Chl a+b(30.56%),SPAD(27.32%),relative water content(0.47%),soluble sugar(5.09%)and activities of three antioxidant enzymes(superoxide dismutase 169.66%,peroxidase 3.59%,catalase 319.30%),but decreased the relative electrical conductivity(18.52%).Additionally,the application of PBZ was found to affect the flowering quality of P.lactiflora,increasing the flower diameter and fresh weight only in the flower-bud stage.This initiates the bloom stage,where there was a decrease in the total content of the aromatic compounds except for the flower-bud stage,and faded the flower color by reducing the content of anthocyanin.These results demonstrated that the application of PBZ can regulate the P.lactiflora plant types with no significant decrease in its ornamental values.This might provide a theoretical basis for further applying PBZ in P.lactiflora for use in urban landscape spaces.
