Comparison of drug eluting stents with bare metal stents in daily practice for bifurcation lesions in Chinese patients

Background Recently, numerous randomized and controlled trials have demonstrated great advantages of drug eluting stents （DES） with respect to significant reduction of restenosis and recurrence of symptoms and improvement of clinical outcomes after percutaneous coronary intervention （PCI）. Little is known about the comparative effects between DES and bare metal stents （BMS） for bifurcation angioplasty in the Chinese population. We compared the inpatient and 7-month follow-up outcomes between DES and BMS for the treatment of bifurcation lesions. Methods From April 2004 to October 2005, 291 Chinese patients [85.9% male, mean age （57.8± 10.4） years] underwent DES （387 lesions） and/or BMS （297 lesions） implantation for bifurcation lesions. Clinical and angiographic follow-up was performed at 7 months. Results Compared with BMS group, patients in DES group had significantly lower rates of restenosis at main branch （9.5% vs 28.7%, P 〈 0.001） or side branch （14.5% vs 37.0%, P 〈 0.001） and major adverse cardiac events （MACE）（14.0% vs 26.3%, P = 0.000）. The occurrence rate of late in-stent thrombosis did not differ between the two groups in both main （0.8% vs 0, P = 0.224） and side branches （1.4% vs 0, P =0.198）. Target lesion revascularization （TLR） was less frequent in DES group for main branch （8.3% vs 21.3%, P 〈 0.001） and for side branch （7.6% vs 23.5%, P 〈 0.001）. Multivariate regression analysis revealed that total stent length （OR = 1.029, P = 0.01）, postprocedural in-stent minimum lumen diameter （OR = 0.476, P = 0.03） and stent type （OR = 3.988, P = 0.0001） were independent predictors of TLR for main branch. Prior history of coronary intervention （OR = 2.424, P =0.041）, angulated lesion （OR = 2.337, P = 0.033）, postdilation （OR = 0.267, P = 0.035） and stent type （DES vs BMS, OR = 5.459, P = 0.000） were independent predictors of TLR for side branch. The microstructure evolution and the pitting corrosion resistance of a supermartensitic stainless steel after deep cryogenic treatment process were clarified through X-ray diffraction,field emission scanning electron microscopy,transmission electron microscopy(TEM)and electrochemical *** results showed that the microstructure of supermartensitic stainless steel mainly consisted of reversed austenite,tempered martensite,and M_(23)C_(6)carbides after *** deep cryogenic treatment promoted the refinement of the martensite laths and the precipitation of the carbides in comparison with the traditional *** analysis indicated that the segregation of Si atoms at the boundary was found at the interface between carbide and *** pitting corrosion potential of the specimens subjected to deep cryogenic treatment decreased with the elevated tempering temperature,and the lowest pitting corrosion potential was found at the tempering temperature of 650℃.The sensitivity of the pitting corrosion potential was attributed to the precipitation of M_(23)C_(6)carbides and Si atoms *** atoms segregation engendered the formation of Cr-depleted zone near M_(23)C_(6)and impeded the recovery of Cr-depleted zone.

BACKGROUND Icotinib could have potential effect and tolerability when used sequentially with chemotherapy for advanced epidermal growth factor receptor(EGFR)-mutated non-small cell lung cancer(NSCLC).AIM To evaluate the efficacy and safety of chemotherapy followed by icotinib maintenance therapy as first-line treatment for advanced EGFR-mutated *** This multicenter,open-label,pilot randomized controlled trial enrolled 68 EGFRmutated stage IIIB/IV NSCLC patients randomized 2:3 to the icotinib alone and chemotherapy+icotinib *** The median progression-free survival in the icotinib alone and chemotherapy+icotinib groups was 8.0 mo(95%CI:3.84-11.63)and 13.4 mo(95%CI:10.18-16.33),respectively(P=0.0249).No significant differences were found in the curative effect when considering different cycles of chemotherapy or chemotherapy regimen(all P>0.05).CONCLUSION A sequential combination of chemotherapy and EGFR-tyrosine kinase inhibitor is feasible for stage IV EGFR-mutated NSCLC patients.

为建立猪肺炎支原体(Mhp)血清学调查及免疫评估方法,本研究利用DNAStar生物学软件对Mhp的P46和P65进行抗原表位分析,利用重叠延伸PCR(SOE-PCR)获得P46-P65融合基因,构建重组表达质粒pETP46-P65,将其转化大肠杆菌BL21(DE3)感受态细胞,经诱导后获得了可溶性表达的重组P46(aa33~aa419)-P65(aa307~aa627)蛋白(rP46-P65)。以纯化的r P46-P65为包被抗原,经优化各反应条件后建立了检测Mhp抗体的间接ELISA方法。特异性试验结果显示所建立的方法与CSFV、FMDV、PEDV、PCV2、PRV、PRRSV和APP等阳性血清均无交叉反应。该方法可检测到最高稀释至6 400倍的Mhp阳性血清,批内和批间变异系数均小于5%。利用IDEXX试剂盒和本研究建立的方法同时检测298份临床血清样品,前者的检测阳性率为62.4%(186/298),后者的检测阳性率为73.8%(220/298),两者检测结果的总符合率为88.6%。IDEXX检测为阳性的血清,采用本研究建立的ELISA方法检测也均为阳性;而部分Mhp阳性猪经IDEXX试剂盒检测为阴性的血清,该ELISA方法检测结果却为阳性,其中79.4%(27/34)检测结果有差异的血清经Mhp颜色变化试验证明均为阳性,表明本研究建立的ELISA方法的敏感性明显高于IDEXX方法。本研究建立的检测猪Mhp抗体的间接ELISA方法与目前广泛使用的方法相比优势明显,为临床血清流行病学调查及血清抗体水平评估提供了可行方法。

利用熔铸和形变热处理工艺制备得到了一种Cu-Sn-Fe-Ni合金,并通过静置腐蚀和电化学测试研究了合金在3.5%NaCl溶液中的静态腐蚀行为,用扫描电子显微镜和X射线光电子能谱对合金腐蚀后微观组织和腐蚀产物进行了表征,最后讨论分析了该合金腐蚀机理。结果表明:合金在质量分数3.5%NaCl溶液中静态腐蚀速率为0.0473 mm/a,耐腐蚀性能较好;合金随着浸泡时间的增加,其耐腐蚀性先增加后减弱;合金在浸泡过程有明显的优先腐蚀倾向,首先发生脱Fe腐蚀,接着Cu氧化形成较致密的钝化膜,进而降低合金腐蚀速率,随后Sn和Ni开始溶解腐蚀,形成氧化膜,使钝化膜更为致密,但此时钝化膜下层依然发生脱Fe腐蚀,促使致密的钝化膜发生局部破坏,导致合金的耐腐蚀性能下降。

Protective effect of indomethacin in renal ischemia-reperfusion injury in mice

Objective： To evaluate the renoprotection effects of non-steroidal anti-inflammatory drugs （NSAIDs） in renal ischemia-reperfusion injury （IRI） and the cyclooxygenase （COX）-1/2 blockade association by indomethacin （IMT） in the mice model. Methods： After the left renal pedicle of mice was clamped, IMT was administrated by intraperitoneal injection with four doses： 1, 3, 5, and 7 mg/kg. Blood and kidney samples were collected 24 h after IRI. The renal functions were assayed by the cytokines and serum creatinine （SCr） using enzyme-linked immunosorbent assay （ELISA） kits. Kidney samples were analyzed by hematoxylin and eosin （H＆E） and immunohistochemistry stainings. Results： The mice administered with 5 mg/kg IMT had a marked reduction in SCr and significantly less tubular damage The tumor necrosis factor a （TNF-α） activity in renal homogenates and interleukin 6 （IL-6） activity in serum had a marked reduction at doses of 5 and 7 mg/kg IMT. The administration of 3 and 5 mg/kg IMT had a marked reduction in the ratio of thromboxane B2 to 6-keto-prostaglandin F1α. COX-1 and COX-2 stainings were weaker in 5 mg/kg IMT groups than that in the other groups. Conclusions： There was a dose response in the IMT function of renal IRI in mice, and IMT had a protective effect in a certain dose range. The effect of IMT on mice IRI was related to COX-1/2 blockades.

Understanding the mechanism of soil organic carbon(SOC)sequestration is of paramount importance in sustaining crop productivity and mitigating climate ***-term trials were employed to investigate the responses of total SOC and its pools,i.e.,mineral-associated OC(MOC),particulate OC(POC,containing Light-POC and Heavy-POC),to fertilization regimes at Yangling(25-year),Tianshui(35-year)and Pingliang(37-year)under a rain-fed cropping system in the Loess *** fertilization regimes in each trial included three treatments,i.e.,control(no nutrient input,CK),chemical fertilizers(CF),and organic manure plus chemical fertilizers(MCF).Relative to the CK,long-term fertilization appreciably increased SOC storage by 134,89 and 129 kg ha^(–1)yr^(–1)under CF,and 418,153 and 384 kg ha^(–1)yr^(–1)under MCF in plough layer soils(0–20 cm),respectively,at the Yangling,Tianshui and Pingliang *** MOC pools accounted for 72,67 and 64%of the total SOC at the above three sites with sequestration rates of 76,57 and 83 kg ha^(–1)yr^(–1)under CF and 238,118 and 156 kg ha^(–1)yr^(–1)under MCF,***,the MOC pool displayed a saturation behavior under MCF *** POC accordingly constituted 27,33 and 36%of SOC,of which Light-POC accounted for 11,17 and 22%and Heavy-POC for 17,16 and 15%of SOC,*** sequestration rates of POC were 58,32 and 46 kg ha^(–1)yr^(–1)under CF,and 181,90 and 228 kg ha^(–1)yr^(–1)under MCF at the three respective sites,in which Light-POC explained 59,81 and 72%of POC under CF,and 60,40 and 69%of POC under MCF,with Heavy-POC accounting for the *** with CK,the application of CF alone did not affect the proportions of MOC or total POC to SOC,whereas MCF application markedly reduced the proportion of MOC and increased the POC ratio,mainly in the Light-POC *** distribution of SOC among different pools was closely related to the distribution and stability of *** present study confirmed that organic

Von Meyenburg complexes(VMCs) are a rare type of ductal plate malformation. We herein report two Chinese families with VMCs, and the suspicious gene mutation of this disease. Proband A was a 62-year-old woman with abnormal echographic presentation of the liver. She received magnetic resonance imaging(MRI) examination and liver biopsy, and the results showed she had VMCs. Histologically proved hepatocellular carcinoma was found 1 year after the diagnosis of VMCs. Proband B was a 57-year-old woman with intrahepatic diffuselesions displayed by abdominal ultrasonography. Her final diagnoses were VMCs, congenital hepatic fibrosis, and hepatitis B surface e antigen-negative chronic hepatitis B after a series of examinations. Then, all the family members of both proband A and proband B were screened for VMCs by MRI or ultrasonography. The results showed that four of the 11 family members from two families, including two males and two females, were diagnosed with VMCs. DNA samples were extracted from the peripheral blood of those 11 individuals of two VMCs pedigrees and subjected to polymerase chain reaction amplification of the polycystic kidney and hepatic disease 1(PKHD1) gene. Two different mutation loci were identified. Heterozygous mutations located in exon 32(c.4280 delG, ***1427 ValfsX 6) in family A and exon 28(c.3118 C>T, ***1040 Ter) in family B were detected. We speculate that PKHD1 gene mutations may be responsible for the development of VMCs.
