Comparison of laparoscopic selective colectomy based on barium-strip examination and subtotal colectomy for adult slow-transit constipation

Background:Surgical management of adult slow-transit constipation(ASTC)can be effective for patients with intractable *** study aimed to evaluate whether barium-strip examination and selective colectomy improved post-operative outcomes in ASTC patients in comparison with subtotal ***:A retrospective cohort study of 53 cases with refractory ASTC was conducted between June 2008 and June *** were evaluated by the barium-strip technique,colonoscopy,defecography and anorectal *** in the standard group underwent laparoscopic subtotal colectomy and patients in the laparoscopic selective colectomy(LSC)group underwent LSC at the precise location identified by barium *** bowel movements,the Wexner Constipation Scale and the Gastrointestinal Quality of Life Index(GIQLI)were assessed post-operatively at 3,6,12 and 24 ***:A total of 49 patients were included in the *** median follow-up was 37 months(range,26–60 months).The mean post-operative hospital stay was 12 days and similar between groups(P=0.071).The length of colon resection,operative time and intra-operative blood loss were reduced in the LSC group(all P<0.05).No major complications occurred.A similar number of patients(24 in the standard group and 25 in the LSC group)exhibited hypoganglionosis or aganglionosis in the colon-wall muscle layer(P=0.986).Although there were no significant differences in post-operative spontaneous bowel movements and the Wexner Constipation Scale between the two groups,the mean GIQLI of the LSC group was significantly higher at 3,6 and 24 months post-operatively(all P<0.05).Conclusions:LSC based on barium-strip examination is an appropriate modality for treating ASTC.

In the past 10 years, acellular dermal matrix （ADM） has .been widely used in various fields of surgery, includingcleft palate repair, abdominal wall repair, tendon repair, hernia repair, facial and eyelid soft tissue augmentation surgery, ulcer repair, and vaginal sling repair, among others.1-3 ADM not only provides initial structural strength and bulk, but also allows relatively rapid vascular growth and serves as a scaffold for ＂new＂ tissue formation.4 These properties have led to its application to resolving surgical problems where tissue deficiency remains an issue. Many surgeons have begun to use ADM-assisted breast reconstruction techniques. Compared with ADM, the traditional method of implant-based breast reconstruction is more straightforward but is more prone to postoperative complications. Generally speaking, breast revision surgery procedures are complex, challenging, and unpredictable as the potential for the development of serious complications, such as capsular contracture （CC）, must be addressed. CC has plagued plastic surgeons as the most common complication of reconstructive breast surgery for many yearsY The application of ADM addresses these problems, and as a result, its popularity is growing： in the United States 73% of surgeons have some experience in the use of xenogeneic or allogeneic ADM in breast reconstruction.8 Furthermore, the use of ADM in nipple and areola reconstruction can achieve a more natural appearance and avoids complications of other more traditional techniques.~ In cases of breast reconstruction, techniques that incorporate the use of ADM have been used to treat and reduce the incidence of CC, determine the position of the inframammary fold and final breast contour, enhance the use of available mastectomy skin, and increase the thickness of the soft tissue to reduce implant visibility or palpability. Semiunitary Transforrnation and Isospectral Hamiltonians in Arbitrary Dimensional Space

People’s ever-increasing demand on building environment has promoted the rapid development of intelligent buildings and building automation systems. With the application of BACnet/IP based building controllers, the building control center is connected via high-speed Ethernet with functional controllers on different floors and with different functions. Various functional modules of hardware circuits, which are designed on the ARM9 hardware platform, provide a stable operating environment for software and network by transplanting the embedded Linux system. This paper focuses on realization of data exchange between BACnet protocol stack and other BACnet devices through the protocol stack and the controller.

By means of the complex Clifford algebra, a new realization of multi-dimensional semiunitary transformation is put forward and then applied to studying the isospectrality of nonrelativistic Hamiltonians of multi-dimensional quantum mechanical systems, in which the generalized Pauli coupling interaction and spin-orbit coupling interaction appear naturally. Moreover, it is shown that the semiunitary operators, together with the Hamiltonian of quantum mechanical system, satisfy the polynomially-deformed angular momentum algebra.

Heavy metal pollution in agricultural soils has serious negative influence on human health. Concentrations of Cd, Hg, As, Pb, Cr, Cu, Zn, and Ni in top soils （0-20 cm） of greenhouses and farmlands from four main vegetable production areas Shouguang, Laiyang, Jinxiang, and Zhangqiu in Shandong Province, one of the most rapidly developing regions in China, were measured in this study. Shouguang is mainly occupied by greenhouse vegetables and the other three areas are mainly open field culture. Total of 149 soil samples were collected. The average concentrations of the eight heavy metals of the tested 149 soil samples were all below the threshold values according to ＂Farmland environmental quality evaluation standards for edible agricultural products （HJ332-2006）＂ of China. However, most of the studied heavy metals were present at higher concentrations than those of the natural background levels in local agricultural soils. Among the total 149 soil samples, 22 samples were contaminated by Cd, Ni, Cu, or Hg. Comparisons showed that the main pollution element in greenhouse vegetable soils was Cd, while that of open field vegetable soils was Cu. The results of principal components analysis （PCA） suggested that concentrations of Cr, As, and Ni were mainly controlled by parent rocks; Hg and Pb were affected by anthropogenic activities such as vehicle and industrial fumes and waste water irrigation. Meanwhile, concentrations of Cd, Cu, and Zn were affected mainly by the use of agrochemicals. Most of the heavy metals were positively correlated with each other in concentration. Appropriate measures should be taken to effectively control heavy metal levels in vegetable soils and thus protect human health.

Forage supply has been stressed due to the rapid increase in China's livestock ***,the long-term dynamics of the relationships between forage demand and multi-sourced supply are not ***,we examine the annual forage demand,or practical carrying capacity(PCC),and supply,or theoretical carrying capacity(TCC)from 2000 to 2019 in *** construct a forage supply-demand index(FSDI)to represent the forage supply pressure using MODIS-derived net primary productivity products and provincial statistical datasets,and we consider two ***,natural grasslands are the sole source of ***,natural grassland forage supply is supplemented with straw *** find an increase in PCC in northwestern China's major pastoral and agropastoral provincial regions,including Inner Mongolia,Gansu,Ningxia and Qinghai,at rate of 0.24-3.59 million sheep units(SU)a *** the primary source of forage,the theoretical carrying capacity of natural grasslands(TCCgrass)expanded at a rate of 3 million SU/yr *** straws fed 126.58 million SU nationally in 2019,which accounted for 11.3%of the total practical carrying capacity and alleviated the forage supply pressure by reducing FSDI by 26.56%.During 2000–2019,the theoretical carrying capacity of straw crops(TCCcrop)increased rapidly from 76.5 million SU to 126.6 million SU,which accounted for 10%-15%of the total forage supply at the national *** also discovered large carrying capacity gaps(TCCgap)in the northwestern pastoral provincial regions of Inner Mongolia,Xinjiang,Gansu,and some agricultural provinces such as Shandong and Henan,when we considered forage supply from both natural grasslands and straw *** findings showed a large forage gap in the traditional pastoral regions,and we also discussed green fodder as a potential solution for balancing the supply of and demand for forage,which may shed light on crop and forage planning.

研究、对比了均匀和非均匀风速下蒸发器的性能,实验测量了某空调蒸发器表面的风速非均匀分布,数值模拟了该非均匀风速下蒸发器的性能,实验验证了非均匀风速下模拟结果的准确性.研究结果表明:非均匀分布时风速比均匀风速大的管路换热量较之均匀风速时的有所增加,风速比均匀风速小的管路换热量较之均匀风速时的有所减小,且换热量减小的幅度比增加的更加显著;传热系数随风速非线性增加;各支路出口状态差别越大,蒸发器换热量的衰减幅度就越大.

Background Cancer-associated fibroblasts (CAFs) play a vital role in facilitating tumor progression through extensive reciprocal interplay with cancer cells. Tumor-derived extracellular vesicles (EVs) are the critical mediators involved in the crosstalk between cancer cells and stromal cells, contributing to the metastasis of cancers. Yet, the biological mechanisms of tumor-derived EVs in triggering CAFs phenotype to stimulate the lymph node (LN) metastasis of bladder cancer (BCa) are largely unknown. Here, we aimed to explore the effects and molecular mechanisms of tumor-derived EV-mediated CAFs phenotype in regulating BCa LN metastasis. Methods The high-throughput sequencing was utilized to identify the crucial long non-coding RNA (lncRNA) associated with CAF enrichment in BCa. The functional role of the transition of fibroblasts to CAFs induced by LINC00665-mediated EVs was investigated through the in vitro and in vivo assays. Chromatin isolation by RNA purification assays, fluorescence resonance energy transfer assays, cytokine profiling and patient-derived xenograft (PDX) model were performed to explore the underlying mechanism of LINC00665 in the LN metastasis of BCa. Results We found that CAFs are widely enriched in the tumor microenvironment of BCa, which correlated with BCa lymphangiogenesis and LN metastasis. We then identified a CAF-associated long non-coding RNA, LINC00665, which acted as a crucial mediator of CAF infiltration in BCa. Clinically, LINC00665 was associated with LN metastasis and poor prognosis in patients with BCa. Mechanistically, LINC00665 transcriptionally upregulated RAB27B expression and induced H3K4me3 modification on the promoter of RAB27B through the recruitment of hnRNPL. Moreover, RAB27B-induced EVs secretion endowed fibroblasts with the CAF phenotype, which reciprocally induced LINC00665 overexpression to form a RAB27B-HGF-c-Myc positive feedback loop, enhancing the lymphangiogenesis and LN metastasis of BCa.
