Clinical features, treatment strategies and outcomes of craniocervical junction arteriovenous fistulas: a cohort study of 193 patients

Background Craniocervical junction(CCJ)arteriovenous fistulas(AVFs)are rare.The current treatment strategies for AVFs with different angioarchitecture need to be clarified.The present study aimed to analyse the correlation between angioarchitecture and clinical characteristics,share our experience in treating this disease and identify risk factors associated with subarachnoid haemorrhage(SAH)and poor outcomes.Methods A total of 198 consecutive patients with CCJ AVFs from our neurosurgical centre were retrospectively reviewed.The patients were grouped according to their clinical manifestations,and their baseline clinical characteristics,angioarchitecture,treatment strategies and outcomes were summarised.Results The patients’median age was 56 years(IQR 47–62 years).The majority of patients were men with 166(83.8%)patients.The most common clinical manifestation was SAH(52.0%),followed by venous hypertensive myelopathy(VHM)(45.5%).The most common CCJ AVFs type was dural AVF,with 132(63.5%)fistulas.The most frequent fistula location was C-1(68.7%)and dural branch of vertebral artery(70.2%)was the most involved arterial feeders for fistulas.The most common direction of venous drainage was descending intradural drainage(40.9%),followed by ascending intradural drainage(36.5%).Microsurgery was the most common treatment strategy applied for 151(76.3%)patients,15(7.6%)patients were treated with interventional embolisation only,and 27(13.6%)received both interventional embolisation and microsurgical treatment.The learning curve for microsurgery only was analysed by cumulative summation method,and the turning point was the 70th case,and blood loss in post-group was lower than that in pre-group(p=0.034).At the last follow-up,there were 155(78.3%)patients with favourable outcomes(modified Rankin Scale(mRS)<3).Age≥56(OR 2.038,95%CI 1.039 to 3.998,p=0.038),VHM as the clinical manifestation(OR 4.102,95%CI 2.108 to 7.982,p<0.001)and pretreatment mRS≥3(OR 3.127,95%CI 1.617 to 6.047,p<0.001)were significantly associated with poor out

Lifestyle is related to many chronic diseases， such as knee osteoarthritis （KOA）. Food intake is important because it can be controlled. However， the causal relationship of food intake and KOA still remain unclear. To identify the relationship between food intake and KOA and to screen relevant targets. First， we performed two-sample Mendelian randomization （MR） analysis. A total of 16 kinds of food intake datasets were included， the inverse variance-weighted analysis showed that the KOA was negatively correlated with cereal intake (odds ratio[OR]0.48; 95% CI 0.35-0.68; P = 1.98 × 10-5). Second， potential drug targets of KOA in plasma were explored， SPOCK2 as the causal protein for plasma with KOA， and increased SPOCK2 (OR 0.84; 95% CI 0.77 – 0.92; P = 5.52 × 10- 5)， decreased the risk of KOA. Integrated transcriptomic analysis （P = 0.0285） and in vivo （P = 0.0395） showed consistent results. Finally， by metabolomics analysis and molecular docking， seven co-expressed small molecules were detected in cereal and were bound to SPOCK2. Our findings indicate the cereal intake was negatively correlated with KOA， and support the SPOCK2 as a novel potential target for cereal intake in KOA. Our study will provide clinical dietary guidance for KOA patients and provide a theoretical basis for the development of novel， side-effect-free drugs for KOA.

Effective surrounding rock control is a prerequisite for realizing safe mining in underground coal mines.In the past three decades, longwall top-coal caving mining(LTCC) and single pass large height longwall mining(SPLL) found expanded usage in extracting thick coal seams in China. The two mining methods lead to large void space left behind the working face, which increases the difficulty in ground control.Longwall face failure is a common problem in both LTCC and SPLL mining. Such failure is conventionally attributed to low strength and high fracture intensity of the coal seam. However, the stiffness of main components included in the surrounding rock system also greatly influences longwall face stability.Correspondingly, surrounding rock system is developed for LTCC and SPLL faces in this paper. The conditions for simultaneous balance of roof structure and longwall face are put forward by taking the stiffness of coal seam, roof strata and hydraulic support into account. The safety factor of the longwall face is defined as the ratio between the ultimate bearing capacity and actual load imposed on the coal wall.The influences provided by coal strength, coal stiffness, roof stiffness, and hydraulic support stiffness,as well as the movement of roof structure are analyzed. Finally, the key elements dominating longwall face stability are identified for improving surrounding rock control effectiveness in LTCC and SPLL faces.

Hydrogen peroxide(H_(2)O_(2))in situ electrosynthesis by O_(2)reduction reaction is a promising alternative to the conventional Fenton treatment of refractory wastewater.However,O_(2)mass transfer limitation,cathodic catalyst selectivity,and electron transfer in O_(2)reduction remain major engineering obstacles.Here,we have proposed a systematic solution for efficient H_(2)O_(2)generation and its electro-Fenton(EF)application for refractory organic degradation based on the fabrication of a novel ZrO_(2)/CMK-3/PTFE cathode,in which polytetrafluoroethylene(PTFE)acted as a hydrophobic modifier to strengthen the O_(2)mass transfer,ZrO_(2)was adopted as a hydrophilic modifier to enhance the electron transfer of O_(2)reduction,and mesoporous carbon CMK-3 was utilized as a catalyst substrate to provide catalytic active sites.Moreover,feasible mass transfer of O_(2)from the hydrophobic to the hydrophilic layer was designed to increase the contact between O_(2)and the reaction interface.The H_(2)O_(2)yield of the ZrO_(2)/CMK-3/PTFE cathode was significantly improved by approximately 7.56 times compared to that of the co nventional gas diffusion cathode under the same conditions.The H_(2)O_(2)generation rate and Faraday efficiency reached125.98 mg·cm^(-2)·h^(-1)(normalized to 5674.04 mmol·g^(-1)·h^(-1)by catalyst loading)and 78.24%at-1.3 V versus standard hydrogen electrode(current density of-252 mA·cm^(-2)),respectively.The high H_(2)O_(2)yield ensured that sufficient OH was produced for excellent EF performance,resulting in a degradation efficiency of over 96%for refractory organics.This study offers a novel engineering solution for the efficient treatment of refractory wastewater using EF technology based on in situ high-yield H_(2)O_(2)electrosynthesis.

High-power tunable femtosecond mid-infrared(MIR)pulses are of great interest for many scientific and industrial applications.Here we demonstrate a compact fluoride-fiber-based system that generates single solitons tunable from 3 to 3.8μm.The system is composed of an Er:ZBLAN fiber oscillator and amplifier followed by a fusion-spliced Dy:ZBLAN fiber amplifier.The Er:ZBLAN fiber amplifier acts as a power booster as well as a frequency shifter to generate Raman solitons up to 3μm.The Dy:ZBLAN fiber amplifier transfers the energy from the residual 2.8μm radiation into the Raman solitons using an in-band pumping scheme,and further extends the wavelength up to 3.8μm.Common residual pump radiation and secondary solitons accompanying the soliton self-frequency shift(SSFS)are recycled to amplify Raman solitons,consequently displaying a higher output power and pulse energy,a wider shifting range,and an excellent spectral purity.Stable 252 fs pulses at3.8μm with a record average power of 1.6 W and a pulse energy of 23 n J are generated.This work provides an effective way to develop high-power widely tunable ultrafast single-soliton MIR laser sources,and this method can facilitate the design of other SSFS-based laser systems for single-soliton generation.

The pre-blasting zones have a significant influence on the coal broken and the recovery rate of the top coal caving mining.This paper explores the coal movement mechanism in the top coal with pre-blasting zones based on the geological parameters of Jiangcang No.1 mine in the western of China.The steeply inclined ultra-thick coal seam was mined by using the sublevel top-coal caving mining method.The geo-image process system(GIPS)software was used to track the mark points on the coal and roof.The support was used to record the load on the top of the support during the caving mining process.The results show that the horizontal coal is moved to the shield window and the vertical coal is moved downward in the first level,while that in the second level shows consistency for the integrity of the coal.The vertical displacements of the corresponding points in the first level are lairger than that in the second level.However,the load on the top of the support in the first level is smaller than that in the second level.It indicated that the pre-blasting zones will give a more loosen top-coal and a small degree of difficulty in the top coal caving.

Lung cancer burden and trends from 2000 to 2018 in China:Comparison between China and the United States

Objective: This study aims to provide an analysis of the current status and trends of lung cancer incidence and mortality rates in China, comparing trends with those in the United States(U.S.).Methods: Data on lung cancer incidence and mortality rates spanning 2000 to 2018 were extracted from the China Cancer Registry Annual Report and the Surveillance, Epidemiology, and End Results database for China and the U.S., respectively. Crude incidence and mortality rates were calculated by sex and age, with age-standardized incidence rates(ASIR) and mortality rates(ASMR) calculated using the Segi-Doll world standard population.Trend analyses employed Joinpoint regression models to determine average annual percentage change(AAPC).The study also assessed the proportion of new cases and deaths by sex and age.Results: In 2018, the ASIR of lung cancer for males in China was 50.72 per 100,000 and the ASMR was 39.69 per 100,000, the ASIR for females was 26.25 per 100,000 and the ASMR was 15.24 per 100,000. Both ASIR and ASMR were higher in males and the highest in the population aged 65 years and older, with the lowest among those aged 20-49 years. In China, female ASIR demonstrated an increasing trend(AAPC: 1.16%), while ASMR decreased in both sexes(AAPCs:-0.48% for males,-1.00% for females). The U.S. exhibited decreasing trends in both ASIR and ASMR across sexes and age groups.Conclusions: The study identified an increasing trend in lung cancer incidence among females and a decreasing mortality trend in both sexes in China. These trends are likely linked to factors such as smoking prevalence,advancements in cancer screening, and improved medical care. The findings underscore the need for tailored lung cancer prevention measures in China, particularly the reinforcement of anti-smoking policies.

Understanding the characteristics of drawing body shape is essential for optimization of drawing parameters in longwall top coal caving mining.In this study,both physical experiments and theoretical analysis are employed to investigate these characteristics and derive a theoretical equation for the drawing body shape along the working face in an inclined seam.By analyzing the initial positions of drawn marked particles,the characteristics of the drawing body shape for different seam dip angles are obtained.It is shown that the drawing body of the top coal exhibits a shape-difference and volume-symmetry characteristic,on taking a vertical line through the center of support opening as the axis of symmetry,the shapes of the drawing body on the two sides of this axis are clearly different,but their volumes are equal.By establishing theoretical models of the drawing body in the initial drawing stage and the normal drawing stage,a theoretical equation for the drawing body in an inclined seam is proposed,which can accurately describe the characteristics of the drawing body shape.The shape characteristics and volume symmetry of the drawing body are further analyzed by comparing the results of theoretical calculations and numerical simulations.It is shown that one side of the drawing body is divided into two parts by an inflection point,with the lower part being a variation development area.This variation development area increases gradually with increasing seam dip angle,resulting in an asymmetry of the drawing body shape.However,the volume symmetry coefficient fluctuates around 1 for all values of the seam dip angle variation,and the volumes of the drawing body on the two sides are more or less equal as the variation development volume is more or less equal to the cut volume.Both theoretical calculations and numerical simulations confirm that the drawing body of the top coal exhibits the shape-difference and volume-symmetry characteristic.
