Incorporation of near-real-time hospital occupancy data to improve hospitalization forecast accuracy during the COVId19 pandemic

Public health decision makers rely on hospitalization forecasts to inform COVId-19 pandemic planning and resource *** forecasts are most relevant when they are accurate,made available quickly,and updated *** rapidly adapted an agent-based model(ABM)to provide weekly 30-day hospitalization forecasts(i.e.,demand for intensive care unit[ICU]beds and non-ICU beds)by state and region in North Carolina for public health decision *** ABM was based on a synthetic population of North Carolina residents and included movement of agents(i.e.,patients)among North Carolina hospitals,nursing homes,and the *** assigned SARSCoV-2 infection to agents using county-level compartmental models and determined agents’COVId-19 severity and probability of hospitalization using synthetic population characteristics(e.g.,age,comorbidities).We generated weekly 30-day hospitalization forecasts during Mayedecember 2020 and evaluated the impact of major model updates on statewide forecast accuracy under a SARS-CoV-2 effective reproduction number range of *** the 21 forecasts included in the assessment,the average mean absolute percentage error(MAPE)was 7.8%for non-ICU beds and 23.6%for ICU *** the major model updates,integration of near-real-time hospital occupancy data into the model had the largest impact on improving forecast accuracy,reducing the average MAPE for non-ICU beds from 6.6%to 3.9%and for ICU beds from 33.4%to 6.5%.Our results suggest that future pandemic hospitalization forecasting efforts should prioritize early inclusion of hospital occupancy data to maximize accuracy.

Background: Large-diameter trees have an outsized influence on aboveground forest dynamics, composition, and structure. Although their influence on aboveground processes is well studied, their role in shaping belowground fungal communities is largely unknown. We sought to test if (i) fungal community spatial structure matched aboveground forest structure;(ii) fungal functional guilds exhibited differential associations to aboveground trees, snags, and deadwood;and (iii) that large-diameter trees and snags have a larger influence on fungal community richness than smaller-diameter trees. We used MiSeq sequencing of fungal communities collected from soils in a spatially intensive survey in a portion of Cedar Breaks National Monument, Utah, USA. We used random forest models to explore the spatial structure of fungal communities as they relate to explicitly mapped trees and deadwood distributed across 1.15 ha of a 15.32-ha mapped subalpine forest. Results: We found 6,177 fungal amplicon sequence variants across 117 sequenced samples. Tree diameter, dead-wood presence, and tree species identity explained more than twice as much variation (38.7% vs. 10.4%) for ectomy-corrhizal composition and diversity than for the total or saprotrophic fungal communities. Species identity and dis-tance to the nearest large-diameter tree (≥ 40.2 cm) were better predictors of fungal richness than were the identity and distance to the nearest tree. Soil nutrients, topography, and tree species differentially influenced the composition and diversity of each fungal guild. Locally rare tree species had an outsized influence on fungal community richness. Conclusions: These results highlight that fungal guilds are differentially associated with the location, size, and species of aboveground trees. Large-diameter trees are implicated as drivers of belowground fungal diversity, particularly for ectomycorrhizal fungi.

Hepatitis B virus(HBV) is the most efficiently transmissible of the bloodborne viruses that are important in healthcare settings. Healthcare workers(HCWs)are at risk for exposure to HBV from infected patients and, if infected, are similarly at risk of transmitting HBV to patients. Published cases of HBV transmission from HCW to patient are relatively rare, having decreased in frequency following the introduction of standard(universal) precautions, adoption of enhanced percutaneous injury precautions such as double-gloving in surgery, and routine HBV vaccination of HCWs. Here we review published cases of HCW-to-patient transmission of HBV, details of which have helped to guide the creation of formal guidelines for the management of HBV-infected HCWs. We also compare the published guidelines for the management of HBV-infected HCWs from various governing bodies, focusing on their differences with regard to vaccination requirements, viral load limits, frequency of monitoring, and restrictions on practice. Importantly, while there are differences among the recommendations from governing bodies, no guidelines uniformly restrict HBV-infected HCWs from performing invasive or exposure-prone procedures.

The mind is embodied;thoughts and feelings interact with states of physiological arousal and physical integrity of the *** this context,there is mounting evidence for an association between psychiatric presentations and the expression variant connective tissue,commonly recognised as joint *** hypermobility is common,frequently under-recognised,significantly impacts quality of life,and can exist in isolation or as the hallmark of hypermobility spectrum disorders(encompassing joint hypermobility syndrome and hypermobile Ehlers-danlos syndrome).In this narrative review,we appraise the current evidence linking psychiatric disorders across the lifespan,beginning with the relatively well-established connection with anxiety,to *** next consider emerging associations with affective illnesses,eating disorders,alongside less well researched links with personality disorders,substance misuse and *** then review related findings relevant to neurodevelopmental disorders and stress-sensitive medical *** growing understanding of mind-body interactions,we discuss potential aetiopathogenetic contributions of dysautonomia,aberrant interoceptive processing,immune dysregulation and proprioceptive impairments in the context of psychosocial stressors and genetic *** examine clinical implications of these evolving findings,calling for increased awareness amongst healthcare professionals of the transdiagnostic nature of hypermobility and related disorders.A role for early screening and detection of hypermobility in those presenting with mental health and somatic symptoms is further highlighted,with a view to facilitate preventative approaches alongside longer-term holistic management ***,suggestions are offered for directions of future scientific exploration which may be key to further delineating fundamental mind-body-brain interactions.
