BACKGROUND Gastrointestinal cancer(GICA)is associated with a higher incidence of venous thromboembolism(VTE)compared to other solid tumors,moreover,recurrent VTE and major bleeding(MB)complications during anticoagulation treatment have an associated increase ***-VTE remains a challenging clinical scenario with MB concerns for utilization of direct oral anticoagulants(DOAC),especially with active cancer *** To evaluate patient risk factors,effectiveness(VTE)and safety(MB)of DOACs and low molecular weight heparin(LMWH)in patients with active *** A retrospective chart review of patients receiving DOACs and LMWH with GICA and symptomatic or incidental VTE treated at comprehensive cancer center from November 2013 to February 2017 was *** criteria included active GI cancer diagnosed at any stage or treatment+/-6 mo of VTE diagnosis,whom were prescribed 6 mo or more of DOACs or *** Chi-squared test was used for overall and the Fisher exact test for pairwise comparisons of the proportions of patients experiencing recurrent VTE and MB *** ratios were used to compare the relative odds of the occurrence of the outcome given exposure to the risk *** A total of 144 patients were prescribed anticoagulation,in which 106 fulfilled inclusion criteria apixaban(27.3%),rivaroxaban(34.9%)and enoxaparin(37.7%),and 38 were *** median age was 66.5 years at GICA diagnosis and 67 years at CAVTE event,with 62%males,80%Caucasian,70%stage IV,pancreatic cancer(40.5%),30%Khorana Score(≥3 points),and 43.5%on active ***-four percent of patients completed anticoagulation therapy(range 1 to 43 mo).Recurrent VTE at 6 mo was noted in 7.5%(n=3),6.8%(n=2)and 2.7%(n=1)of patients on enoxaparin,apixaban and rivaroxaban,respectively(all P=NS).MB at 6 mo were 5%(n=2)for enoxaparin,6.8%(n=2)for apixaban and 21.6%(n=8)for rivaroxaban(overall P=0.048;vs LMWH P=0.0423;all other P=NS).Significant predictors of a prim

Vonoprazan-amoxicillin dual regimen with Saccharomyces boulardii as a rescue therapy for Helicobacter pylori:Current perspectives and implications

Yu et al’s study in the World Journal of Gastroenterology(2023)introduced a novel regimen of Vonoprazan-amoxicillin dual therapy combined with Saccharomyces boulardii(***)for the rescue therapy against Helicobacter pylori(***),a pathogen responsible for peptic ulcers and gastric *** is a potassium-competitive acid blocker renowned for its rapid and long-lasting acid suppression,which is minimally affected by *** to proton pump inhibitors,which bind irreversibly to cysteine residues in the H+/K+-ATPase pump,Vonoprazan competes with the K+ions,prevents the ions from binding to the pump and blocks acid *** with increasing antibiotic resistance,effects on the gut microbiota,patient compliance,and side effects have led to the advent of a dual regimen for *** studies suggested that *** plays a role in stabilizing the gut barrier which improves *** eradication *** an acceptable safety profile,the dual-adjunct regimen was effective regardless of prior treatment failure and antibiotic resistance profile,thereby strengthening the applicability in clinical ***,*** comes in various formulations and dosages,warranting further exploration into the optimal dosage for supplementation in rescue ***,larger,randomized,double-blinded controlled trials are warranted to confirm these promising results.

The relationship between oxidative degradation and ammonia emission of carbon capture amines based on their chemical structures

This work investigates the effect of chemical structural positioning of different functional groups in 29 amines covering primary, secondary and tertiary alkanolamines as well as multi-alkylamines and cyclic amines on both amine degradation and ammonia (NH3) emissions during post-combustion amine-based carbon dioxide capture. The results helped to elucidate possible relationships between degradation and emissions as related to the chemical structure of the amine. The results showed that longer alkyl chain lengths in multi-alkylamines caused a more drastic decrease in both degradation and NH3 emissions followed by secondary alkanolamines. The decrease in those activities for primary and tertiary alkanolamines as well as cyclic amines was low and more so for NH3 emissions. In contrast, the increase in hydroxyl groups in secondary alkanolamines caused a drastic increase in degradation and NH3 emissions. On the other hand, having more hydroxyl groups in sterically hindered primary and tertiary alkanolamines caused a more drastic decrease in degradation and a smaller decrease in NH3 emissions due to the steric hindrance within their structure. An increase in the number of amino groups in an amine caused an increase in both degradation and NH3 emission rates because these provided the reactive sites for the formation of free radicals. This effect was not as large in alkyl-cyclic amines as in multi-alkylamines due to the ability of the former to resist oxidative degradation. Furthermore, branched alkyl groups between amino and hydroxyl groups more drastically increased both the degradation and NH3 emission activities than branched alkyl groups located at the nitrogen atom. Failure to Rescue as a Quality Metric in Congenital Heart Surgeries in a High-Complexity Service Provider Institution Located in a Middle-Income Country

Background:Failure to rescue has been an effective quality metric in congenital heart ***,mor-bidity and mortality depend greatly on non-modifiable individual factors and have a weak correlation with better-quality *** aim to measure the complications,mortality,and risk factors in pediatric patients undergoing congenital heart surgery in a high-complexity institution located in a middle-income country and compare it with other institutions that have conducted a similar ***:A retrospective observational study was conducted in a high-complexity service provider institution,in Cali,*** pediatric patients undergoing any congenital heart surgery between 2019 and 2022 were *** main outcomes evaluated in the study were complication,mortality,and failure to rescue *** and multivariate logistic regression analysis was performed with mortality as the outcome ***:We evaluated 308 congenital heart *** the outcomes,201(65%)complications occurred,23(7.5%)patients died,and the FTR of the entire cohort was 11.4%.The presence of a postoperative complication(OR 14.88,CI 3.06–268.37,p=0.009),age(OR 0.79,CI 0.57–0.96,p=0.068),and urgent/emergent surgery(OR 8.14,CI 2.97–28.66,p<0.001)were the most significant variables in predicting ***:Failure to rescue is an effective and comparable quality measure in healthcare institutions and is the major contributor to postoperative mortality in congenital heart *** our higher mortality and complication rate,we obtained a comparable failure to rescue rate to high-income countries’health institutions.
