Use of the Medifast Meal Replacement Program for Weight loss in Overweight and Obese Clients: A Retrospective Chart Review of Three Medifast Weight Control Centers (MWCC)

Objective: A chart review was performed to evaluate the effectiveness of the Medifast (MD) meal replacement (MR) plan in a Medifast Weight Control Center (MWCC) on body weight, body composition, and other health measures at 4, 12, 24 weeks, and final weight loss visit. Methods: Charts included adults aged 18 - 70 (n = 446) with a BMI ≥ 25 kg/m2 who attended one of three MWCCs and were following the MD MR program. Data were collected electronically and included weight, systolic and diastolic blood pressure, pulse, lean muscle mass (lMM), body fat mass, % body fat, and abdominal circumference. Compliance measures included attendance at weekly visits, intake of MRs and supplements, food journals, and ketone testing. Results: Significant weight loss and % weight loss were achieved at all time points with clinically significant weight loss (>5%) occurring in just 4 weeks. Additionally, significant improvements in body composition were seen at all time points coupled with increases in % total body weight as lMM (% lMM improved by 3.5%, 9.8%, 16.0%, and 13.9%, respectively). Blood pressure and pulse were significantly improved, demonstrating the clinical benefit for clients. Multivariate regression revealed a strong inverse relationship between weight change, % compliance with attendance, and the number of weeks that MRs were taken as recommended as well as a positive association with number of ketone tests. Conclusion: The MD MR plan, combined with the support and accountability available in the MWCC, is an efficacious program that promotes significant weight loss and improvements in body composition. These results reveal significant associations between components of compliance and weight loss, but particularly highlight the importance of attendance, a focus of the MWCC model compared to nonclinic models.

Invasive black spiny-tailed iguanas(Ctenosaura similis)on Gasparilla Island,Florida,USA

The native range of Ctenosaura similis extends from southern Mexico through Panama. From an initial introduction of 3 animals in 1979, the species now numbers in the thousands on Gasparilla Island in southwest Florida. In response to complaints of property damage from residents and threats to native species, local officials and the US Department of Agriculture Wildlife Services began a removal program in 2008. Through 2011, trappers removed 9467 ctenosaurs. The number removed declined from 32 iguanas/day in 2008 to 1.9 iguanas/day in 2011 despite no easing of the control effort. We necropsied 2757 ctenosaurs to document aspects of their natural history. Females outnumbered males overall, although the largest size class (>300 mm snout-vent length) included 32 males and just 2 females. Reproduction was seasonal. We found oviducal eggs in females from early Apr to early Jun, approximately 2 months later than C. similis in its native range. We trapped hatchlings from late Jul to early Oct coincident with the summer rainy season. Clutch size increased with female body size, with 62 being the largest clutch size recorded. In general, the biology of the invasive population on Gasparilla Island resembles native C. similis populations in Central America, except for the lack of large individuals. We suggest that shorter day length and colder temperatures create environmental conditions that are suboptimal for individual growth compared to those in the native range.

Deep learning-based analysis of macaque corneal sub-basal nerve fibers in confocal microscopy images

Immuno-oncology combinations： raising the tail of the survival curve

There have been exponential gains in immuno-oncology in recent times through the development of immune checkpoint inhibitors. Already approved by the U.S. Food and Drug Administration for advanced melanoma and non-small cell lung cancer,immune checkpoint inhibitors also appears to have significant antitumor activity in multiple other tumor types. An exciting component of immunotherapy is the durability of antitumor responses observed, with some patients achieving disease control for many years. Nevertheless, not all patients benefit, and efforts should thus now focus on improving the efficacy of immunotherapy through the use of combination approaches and predictive biomarkers of response and resistance. There are multiple potential rational combinations using an immunotherapy backbone, including existing treatments such as radiotherapy, chemotherapy or molecularly targeted agents, as well as other immunotherapeutics. The aim of such antitumor strategies will be to raise the tail on the survival curve by increasing the number of long term survivors, while managing any additive or synergistic toxicities that may arise with immunotherapy combinations. Rational trial designs based on a clear understanding of tumor biology and drug pharmacology remain paramount. This article reviews the biology underpinning immuno-oncology, discusses existing and novel immunotherapeutic combinations currently in development, the challenges of predictive biomarkers of response and resistance and the impact of immuno-oncology on early phase clinical trial design.

Genetic differentiation between and within Northern Native American language groups:an argument for the expansion of the Native American CODIS database

Exposure to dietary mercury alters cognition and behavior of zebra finches

Environmental stressors can negatively affect avian cognitive abilities, potentially reducing fitness, for example by altering response to predators, display to mates, or memory of locations of food. We expand on current knowledge by investigating the effects of dietary mercury, a ubiquitous en- vironmental pollutant and known neurotoxin, on avian cognition. Zebra finches Taeniopygia gut- tata were dosed for their entire lives with sub-lethal levels of mercury, at the environmentally rele- vant dose of 1.2 parts per million. In our first study, we compared the dosed birds with controls of the same age using tests of three cognitive abilities： spatial memory, inhibitory control, and color association. In the spatial memory assay, birds were tested on their ability to learn and remember the location of hidden food in their cage. The inhibitory control assay measured their ability to ig- nore visible but inaccessible food in favor of a learned behavior that provided the same reward. Finally, the color association task tested each bird＇s ability to associate a specific color with the presence of hidden food. Dietary mercury negatively affected spatial memory ability but not inhibi- tory control or color association. Our second study focused on three behavioral assays not tied to a specific skill or problem-solving： activity level, neophobia, and social dominance. Zebra finches exposed to dietary mercury throughout their lives were subordinate to, and more active than, con- trol birds. We found no evidence that mercury exposure influenced our metric of neophobia. Together, these results suggest that sub-lethal exposure to environmental mercury selectively harms neurological pathways that control different cognitive abilities, with complex effects on be- havior and fitness.
