Khettaras in the Taflalet oasis(Morocco):contribution to the promotion of tourism and sustainable development

The khettaras constitute a hydraulic system for mobilising water by gravity from the water table to the surface to irrigate felds in *** system,which has been fairly widespread in north Africa,in particular in Algeria(foggara)and Morocco(khettaras),was introduced several centuries ago in the oases of southeastern Morocco and has contin‑ued to operate despite various natural and anthropic *** on these ingenious and millennial hydraulic systems,successive civilisations living in these environments have been able to establish laws and regulations for the management and mobilisation of natural resources,especially ***,to get the most out of these systems,users have had to abide by rules(Al Orf/Azref)designed to protect,promote,and care for the systems to enhance their *** hydraulic systems have declined in recent years,notably due to extensive groundwater pumping,climate variability(severe droughts),and the local population’s disinterest for this type of ***,we have unfortunately witnessed the loss of a thousand-year-old local ancestral know-how that had been developed by diferent succeeding civilisations in the *** initiatives(2008–2011)have been undertaken by the Moroccan government for the restoration and development of some abandoned khettaras in the Taflalet Oasis so that they could be utilised in the development of an oasis-based cultural tourism and integrated in a tourist circuit known as *** initiative,still in its infancy,has constituted an alternative and a fairly encouraging development for this national and world heritage.

Rheumatic manifestations of inflammatory bowel disease

This article reviews the literature concerning rheu-matic manifestations of inflammatory bowel disease (IBD),including common immune-mediated pathways,frequency,clinical course and therapy. Musculoskel-etal complications are frequent and well-recognized manifestations in IBD,and affect up to 33% of pa-tients with IBD. The strong link between the bowel and the osteo-articular system is suggested by many clinical and experimental observations,notably in HLA-B27 transgenic rats. The autoimmune pathogenic mechanisms shared by IBD and spondyloarthropathies include genetic susceptibility to abnormal antigen pre-sentation,aberrant recognition of self,the presence of autoantibodies against specific antigens shared by the colon and other extra-colonic tissues,and increased intestinal permeability. The response against microor-ganisms may have an important role through molecular mimicry and other mechanisms. Rheumatic mani-festations of IBD have been divided into peripheral arthritis,and axial involvement,including sacroiliitis,with or without spondylitis,similar to idiopathic anky-losing spondylitis. Other periarticular features can oc-cur,including enthesopathy,tendonitis,clubbing,peri-ostitis,and granulomatous lesions of joints and *** and osteomalacia secondary to IBD and iatrogenic complications can also occur. The manage-ment of the rheumatic manifestations of IBD consists of physical therapy in combination with local injection of corticosteroids and nonsteroidal anti-inflammatory drugs; caution is in order however,because of their possible harmful effects on intestinal integrity,perme-ability,and even on gut inflammation. sulfasalazine,methotrexate,azathioprine,cyclosporine and lefluno-mide should be used for selected indications. In some cases,tumor necrosis factor-α blocking agents should be considered as first-line therapy.

To explore the effect of strontium on the structure of as-cast A356 alloy, the strontium was incorporated to the alloy by metallothermic reduction of srO where the mineral was added to the melt through the submerged powders injection technique. The evaluation of the modification of the eutectie silicon and the chemical analysis of samples were done using optical microscopy （OP） and inductively coupled plasma （ICP）, respectively, while microstructural analyses by scanning electron microscopy （sEM）, and the injection time was variable. Magnesium was added to the melt to increase the reactivity and reduce the surface tension of the molten aluminum. It was possible to increase the strontium content from 0 to 0.027% after 20 minutes treatment. This concentration was sufficient to bring about full modification structure of eutectic silicon of as cast alloy A 356 and the acceptable quality metallurgical of alloy.

Treatment of chronic hepatitis C with direct-acting antivirals: The role of resistance

The use of direct-acting antivirals(DAAs) to treat chronic hepatitis C has resulted in a significant increase in rates of sustained viral response(around 90%-95%) as compared with the standard treatment of peginterferon/ribavirin. Despite this, however, the rates of therapeutic failure in daily clinical practice range from 10%-15%. Most of these cases are due to the presence of resistant viral variants, resulting from mutations produced by substitutions of amino acids in the viral target protein that reduce viral sensitivity to DAAs, thus limiting the efficacy of these drugs. The high genetic diversity of hepatitis C virus has resulted in the existence of resistance-associated variants(RAVs), sometimes even before starting treatment with DAAs, though generally at low levels. These preexisting RAVs do not appear to impact on the sustained viral response, whereas those that appear after DAA therapy could well be determinant in virological failure with future treatments. As well as the presence of RAVs, virological failure to treatment with DAAs is generally associated with other factors related with a poor response, such as the degree of fibrosis, the response to previous therapy, the viral load or the viral genotype. nonetheless, viral breakthrough and relapse can still occur in the absence of detectable RAVs and after the use of highly effective DAAs, so that the true clinical impact of the presence of RAVs in therapeutic failure remains to be determined.

Therapy with stem cells in inflammatory bowel disease

Inflammatory bowel disease (IBD) affects a part of the young population and has a strong impact upon quality of life. The underlying etiology is not known, and the existing treatments are not curative. Furthermore, a significant percentage of patients are refractory to therapy. In recent years there have been great advances in our knowledge of stem cells and their therapeutic applications. In this context, autologous hematopoietic stem cell transplantation (HsCT) has been used in application to severe refractory Crohn’s disease (CD), with encouraging results. Allogenic HsCT would correct the genetic defects of the immune system, but is currently not accepted for the treatment of IBD because of its considerable risks. Mesenchymal stem cells (MsCs) have immune regulatory and regenerative properties, and low immunogenicity (both autologous and allogenic MsCs). Based on these properties, MsCs have been used via the systemic route in IBD with promising results, though it is still too soon to draw firm conclusions. Their local administration in perianal CD is the field where most progress has been made in recent years, with encouraging results. The next few years will be decisive for defining the role of such therapy in the management of IBD.

Clostridium difficile (C. difficile) is a spore-forming, toxin-producing, gram-positive anaerobic bacterium that is the principal etiologic agent of antibiotic-associated diarrhea. Infection with C. difficile (CDI) is characterized by diarrhea in clinical syndromes that vary from selflimited to mild or severe. since its initial recognition as the causative agent of pseudomembranous colitis, C. difficile has spread around the world. CDI is one of the most common healthcare-associated infections and a significant cause of morbidity and mortality among older adult hospitalized patients. Due to extensive antibiotic usage, the number of CDIs has increased. Diagnosis of CDI is often difficult and has a substantial impact on the management of patients with the disease, mainly with regards to antibiotic management. The diagnosis of CDI is primarily based on the clinical signs and symptoms and is only confirmed by laboratory testing. Despite the high burden of CDI and the increasing interest in the disease, episodes of CDI are often misdiagnosed. The reasons for misdiagnosis are the lack of clinical suspicion or the use of inappropriate tests. The proper diagnosis of CDI reduces transmission, prevents inadequate or unnecessary treatments, and assures best antibiotic treatment. We review the options for the laboratory diagnosis of CDI within the settings of the most accepted guidelines for CDI diagnosis, treatment, and prevention of CDI.

This study investigates the development of novel montmorillonite/castor oil blends to formulate sustainable lubricating greases to promote the replacement of petrochemicals industry-derived materials by substances obtained from renewable ***,the effect of the thickener concentration on the rheological,chemical,thermal,tribological properties,and atomic force microscopy(AFM)microstructure of these systems were *** results showed that the C20A nanoclay content could be used to modulate the viscosity values,the linear viscoelastic functions,and tribological properties of these montmorillonite *** general,these gel-like dispersions exhibited remarkable lubricant properties;the samples showed values of the friction coefficient and wear scars similar or lower than those obtained with model bentonite grease.

Characterization by EBsD of Antimony-calcium Rich Phases Formed during Purification of Aluminum scrap

We studied the influence of antimony on the modification rate of the silicon eutectic with sodium and/or strontium for castiig of A1-si alloys, and introduced controlled Ca and sb to the system and studied the resulting compounds, which were determined to be Cazsb and A1sb. scanning electron microscopy together with the complementary technique of electron backscatter diffraction （EBsD） were used to characterize the materials. The experimental results provided the basis for us to establish that antimony removal from AI occurs mainly through the reaction （sb）A1 ＋ 2Ca = Ca^sb ＋ A1.
