AIM: To evaluate any differences between the percentages of involved breast volume, pathologic attributes,and tumor marker expression of T3 and T4a-c tumors in locally advanced breast cancers(BC).METHODs: All patients with T3 N > 0 and T4a-c BC without evidence of distant metastasis(M0), presenting to the Breast Clinic from 1980 to 2010, were examinedto determine whether their BC's involved ≥ 50% of their breast volumes, defined by gross replacement of at least one hemisphere. Core needle biopsy or postmastectomy specimens from tumors involving a known percent of breast volume were evaluated for:(1)pathological grades and lympho-vascular invasion(LVI);(2) hormone receptor(ER/PR) expression > 0; and(3)epidermoid growth factor 2(her2) over-expression(3+)by immune-histochemical staining or fluorescent in situ ***: The data base included 98 patients with T3N> 0 M0 and 120 with T4a-c, any N disease, M0 disease. T3 tumor masses involved 50% or more of the breast in 23/98(24%), and T4a-c tumors 65/120(54%)(P s and 23% of T4a-c tumors presented with total breast replacement. There were no significant differences between the pathological attributes and marker expression of the T3 and T4a-c ***: These data suggest that erosion of the overlying skin or underlying chest wall by some BC may be due to neglect and delay, rather than inherent biological aggressiveness.

We report on the design and first results from experiments looking at the formation of radiative shocks on the shenguang-Ⅱ(sG-Ⅱ) laser at the shanghai Institute of Optics and Fine Mechanics in ***-heating of a two-layer CH/CH-Br foil drives a～40 km/s shock inside a gas cell filled with argon at an initial pressure of 1 *** use of gas-cell targets with large(several millimetres) lateral and axial extent allows the shock to propagate freely without any wall interactions,and permits a large field of view to image single and colliding counter-propagating shocks with time-resolved,pointprojection X-ray backlighting(～20 μm source size,4.3 keV photon energy).single shocks were imaged up to 100 ns after the onset of the laser drive,allowing to probe the growth of spatial nonuniformities in the shock *** results are compared with experiments looking at counter-propagating shocks,showing a symmetric drive that leads to a collision and stagnation from～40 ns *** present a preliminary comparison with numerical simulations with the radiation hydrodynamics code ARWEN,which provides expected plasma parameters for the design of future experiments in this facility.

In this paper we present the science potential of the enhanced X-ray Timing and Polarimetry(eXTP) mission for studies of strongly magnetized objects. We will focus on the physics and astrophysics of strongly magnetized objects, namely magnetars, accreting X-ray pulsars, and rotation powered pulsars. We also discuss the science potential of eXTP for QED studies. Developed by an international Consortium led by the Institute of High Energy Physics of the Chinese Academy of sciences, the eXTP mission is expected to be launched in the mid 2020s.

We report the discovery of a peculiar X-ray transient,EP240408a,by Einstein Probe （EP） and follow-up studies made with EP,swift,NICER,GROND,ATCA and other ground-based multiwavelength *** new transient was first detected with Wide-field X-ray Telescope （WXT） on board EP on April 8th,2024,manifested in an intense yet brief X-ray flare lasting for *** flare reached a peak flux of 3.9×10-9erg cm-2s-1in 0.5-4 keV,300 times brighter than the underlying X-ray emission detected throughout the *** and more precise follow-up observations by EP/FXT,swift and NICER confirmed the finding of this new *** X-ray spectrum is non-thermal in 0.5-10 keV,with a power-law photon index varying *** X-ray light curve shows a plateau lasting for4d,followed by a steep decay till becoming undetectable10 d after the initial *** on its temporal property and constraints from previous EP observations,an unusual timescale in the range of 7-23 d is found for EP240408a,which is intermediate between the commonly found fast and long-term *** counterparts have been found in optical and near-infrared,with the earliest observation at 17 h after the initial X-ray detection,suggestive of intrinsically weak emission in these *** demonstrate that the remarkable properties of EP240408a are inconsistent with any of the transient types known so far,by comparison with,in particular,jetted tidal disruption events,gamma-ray bursts,X-ray binaries and fast blue optical *** nature of EP240408a thus remains an *** suggest that EP240408a may represent a new type of transients with intermediate timescales of the order of10 *** detection and follow-ups of more of such objects are essential for revealing their origin.

背景小儿围手术期心跳骤停（POCA）登记处（1994至1997年14）最初的调查结果显示，药物相关性原因是小儿围手术期心跳骤停最常见的原因，例如氟烷B}起的心血管抑制。小儿麻醉技术的变化可能已经使麻醉过程中心跳骤停的原因发生了改变。方法有将近80个为小儿提供麻醉的北美医疗机构自愿加入POCA登记处。这些机构均以标准化数据表格的形式，将每一例≤18岁的小儿围手术期心跳骤停病例匿名上报。据此我们分析了1998至2004年麻醉相关性心跳骤停的原因和相关因素。结果从1998至2004年，有193例（49％）心跳骤停与麻醉相关，其中药物相关性心跳骤停占所有心跳骤停的18％。而1994至1997年期间为37％（P〈0．05）。心血管方面是最常见的原因（占41％），其中失血导致的低血容量和输入库存血导致的高钾血症是最常见的、可确定的心血管原因。导致心跳骤停的呼吸性原因（27％）中，喉痉挛导致的气道梗阻最为常见。中心静脉导管置入过程中引起的血管损伤是设备相关性心跳骤停的最常见原因。在麻醉的不同阶段心跳骤停的原因也存在差异（P〈0．01）。心血管和呼吸方面的原因分别是手术中和手术后阶段最常见的原因。结论伴随氟烷在小儿麻醉中使用减少，与氟烷引起的心血管抑制相关的心跳骤停发生比例降低。通过预防措施可以降低心跳骤停原因各个分类中最常见病因的发生率。
