Risk of ischaemic stroke in patients with transient global amnesia: a propensity-matched cohort study

Background The exact pathophysiological mechanism of transient global amnesia(TGA)is *** is debatable whether TGA is a risk factor for ***,here we investigated the possibility of TGA as a risk factor for stroke in a real-world setting using large-scale nationwide health claims *** We used health claims data from the Korean National Health Insurance Service(NHIS).Patients diagnosed with TGA between 2007 and 2013 were *** initially extracted patients without TGA who were preferentially matched for age and sex with the patients with TGA at a ratio of 10:1 from the whole ***,we performed 1:2 propensity score matching analysis to balance the baseline characteristics between the two *** the propensity score-matched dataset,we performed multivariable Cox regression analysis to investigate the association between TGA and stroke type,including ischaemic,haemorrhagic and all stroke *** Patients with TGA(n=14673)were selected from the NHIS *** extracting from the whole database(n=140486)and propensity score matching their data at a 1:2 ratio,a total of 10448 and 20442 patients were finally assigned to the TGA and control groups,*** multivariable Cox regression analysis demonstrated that the TGA group had a higher risk of ischaemic stroke and all types of stroke(adjusted HR=1.194;95%CI:1.043 to 1.368;and HR=1.197;95%CI:1.056 to 1.357,respectively).Conclusions Analysis of the nationwide claims database showed that TGA could be an important risk factor for stroke,especially for ischaemic stroke.

Risk stratification for disease progression in pT3 prostate cancer after robot-assisted radical prostatectomy

The aim of this study is to identify optimal patients for adjuvant radiation therapy （ART） in pT3 prostate cancer. The role of ART for patients with adverse pathologic features after radical prostatectomy （RP） has been demonstrated, but over- or under-treatment remains a significant concern. Two-hundred and five patients with pT3NOMO who underwent robot-assisted RP without ART were analyzed. Multivariate Cox proportional regression analyses were used to identify predictors of biochemical recurrence （BCR） and clinical progression （CP）. The estimated 5-year BCR-free survival （BCRFS） and CP-free survival （CPFS） were 52.8% and 85.6%, respectively. Preoperative prostate-specifc antigen （PSA）：〉10 ng m1-1 （hazard ratio [HR]： 3.288-6.027; P = 0.003）, pathologic Gleason score （pGS）：〉8 （HR： 4.146; P = 0.014）, and lymphovascular invasion （LVI）（HR： 2.167; P = 0.026） were associated with BCR. Based on these factors, a risk stratification tool was developed. Patients with no risk factors （PSA 〈10 ng m1-1, pGS 6, and absent LVI） showed excellent BCRFS and CPFS at 5 years （91.9% and 100.0%, respectively）, but those with two or more risk factors （PSA ：〉10 ng ml-1, pGS ≥8, or present LVI） had poor BCRFS and CPFS （12.1% and 54.6%, respectively）. In addition, the multivariate analysis revealed that pathologic stage pT3b （HR： 5.393; P = 0.025） was the only predictor of CP. Our study demonstrated the heterogeneity of oncologic outcomes in patients with pT3 prostate cancer. The proposed risk stratification can be used to identify patients who are at risk for disease progression and may aid in identifying the best patients for ART.

There have been numerous works proposed to merge augmented reality/mixed reality(AR/MR)and Internet of Things(IoT)in various ***,they have focused on their specific target applications and have limitations on interoperability or reusability when utilizing them to different domains or adding other devices to the *** paper proposes a novel architecture of a convergence platform for AR/MR and IoT systems and *** proposed architecture adopts the oneM2M IoT standard as the basic framework that converges AR/MR and IoT systems and enables the development of application services used in general-purpose environments without being subordinate to specific systems,domains,and device *** implement the proposed architecture utilizing the open-source oneM2M-based IoT server and device platforms released by the open alliance for IoT standards(OCEAN)and Microsoft HoloLens as an MR device *** also suggest and demonstrate the practical use cases and discuss the advantages of the proposed architecture.

Recurrent Autoencoder Ensembles for Brake Operating Unit Anomaly Detection on Metro Vehicles

The anomaly detection of the brake operating unit (BOU) in thebrake systems on metro vehicle is critical for the safety and reliability ofthe trains. On the other hand, current periodic inspection and maintenanceare unable to detect anomalies in an early stage. Also, building an accurateand stable system for detecting anomalies is extremely difficult. Therefore,we present an efficient model that use an ensemble of recurrent autoencodersto accurately detect the BOU abnormalities of metro trains. This is the firstproposal to employ an ensemble deep learning technique to detect BOUabnormalities in metro train braking systems. One of the anomalous caseson metro vehicles is the case when the air cylinder (AC) pressures are less thanthe brake cylinder (BC) pressures in certain parts where the brake pressuresincrease before coming to a halt. Hence, in this work, we first extract the dataof BC and AC pressures. Then, the extracted data of BC and AC pressuresare divided into multiple subsequences that are used as an input for bothbi-directional long short-term memory (biLSTM) and bi-directional gatedrecurrent unit (biGRU) autoencoders. The biLSTM and biGRU autoencodersare trained using training dataset that only contains normal subsequences. Fordetecting abnormalities from test dataset which consists of abnormal subsequences, the mean absolute errors (MAEs) between original subsequences andreconstructed subsequences from both biLSTM and biGRU autoencoders arecalculated. As an ensemble step, the total error is calculated by averaging twoMAEs from biLSTM and biGRU autoencoders. The subsequence with totalerror greater than a pre-defined threshold value is considered an *** carried out the experiments using the BOU dataset on metro vehiclesin South Korea. Experimental results demonstrate that the ensemble modelshows better performance than other autoencoder-based models, which showsthe effectiveness of our ensemble model for detecting BOU anomalies onmetro trains.

Dysregulation of the Hippo pathway has been frequently identified in various human cancers. The Hippo pathway is a highly complex pathway involving multiple types of proteins, and the activation of YAP by LATS kinase is the final effector step in the transcription process. In this study, we linked the roles of the multifunctional adapter proteins β-arrestins (β-arrestin 1 and 2) in cooperation with other signaling pathways such as GPCR and Wnt to essential cellular functions involved in carcinogenesis, including the regulation of cell proliferation, migration, and differentiation as well as stem cell properties.1 Although β-arrestin 1 and 2 have high structural similarities, they have different roles in carcinogenesis as β-arrestin 1 aids cancer cell survival and metastasis2 and β-arrestin 2 inhibits tumor growth.1 In the Hippo signaling pathway, β-arrestins function as scaffold proteins that mediate the phosphorylation of key molecules, and their association with human cancers is a major research topic. In this study, we demonstrated that β-arrestin 2 inhibits YAP activation through the formation of the β-arrestin 2-LATS-YAP trimeric complex, which results in the promotion of the kinase activity of LATS in cancer cell lines and patient-derived colon cancer organoids.

Identification and comparison of the local physicochemical structures of transition metal-based layered double hydroxides for high performance electrochemical oxygen evolution reactions

Layered double hydroxides(LDHs) have attracted considerable attention as a cost effective alternative to the precious iridium-and ruthenium-based electrocatalysts for an oxygen evolution reaction(OER),a bottleneck of water electrolysis for sustainable hydrogen *** their excellent OER performance,the structural and electronic properties of LDHs,particularly during the OER process,remain to be poorly *** this study,a series of LDH catalysts is investigated through in situ X-ray absorption fine structure analyses and density functional theory(DFT) *** experimental results reveal that the LDH catalyst with equal amounts of Ni and Fe(NF-LDH) exhibits the highest OER activity and catalytic life span when compared with its counterparts having equal amounts of Ni and Co(NC-LDH)and Ni only(Ni-LDH).The NF-LDH shows a markedly enhanced OER kinetics compared to the NC-LDH and the Ni-LDH,as proven by the lower overpotentials of 180,240,and 310 mV,respectively,and the Tafel slopes of 35.1,43.4,and 62.7 mV dec^(-1),*** DFT calculations demonstrate that the lowest overpotential of the NF-LDH is associated with the active sites located at the edge planes of NF-LDH in contrast to those located at the basal planes of Ni-LDH and *** current study pinpoints the active sites on various LDHs and presents strategies for optimizing the OER performance of the LDH catalysts.
