Recovery of brachial plexus injury after bronchopleural fistula closure surgery based on electrodiagnostic study:A case report and review of literature

BACKGROUND Axillary thoracotomy and muscle flap are muscle-and nerve-sparing methods among the surgical approaches to bronchopleural fistula(BPF).However,in patients who are vulnerable to a nerve compression injury,nerve injury may *** this report,we present a unique case in which the brachial plexus(division level),suprascapular,and long thoracic nerve injury occurred after BPF closure surgery in a patient with ankylosing spondylitis and concomitant multiple joint *** SUMMARY A 52-year-old man with a history of ankylosing spondylitis with shoulder joint contractures presented with right arm weakness and sensory impairment immediately after axillary thoracotomy and latissimus dorsi muscle flap surgery for BPF *** the surgery,the patient was positioned in a lateral decubitus position with the right arm hyper-abducted for approximately 6 *** resonance imaging and ultrasound revealed subclavius muscle injury or myositis with brachial plexus(BP)compression and related *** electrodiagnostic study confirmed the presence of BP injury involving the wholedivision level,long thoracic,and suprascapular nerve *** was treated with medication,physical therapy,and ultrasound-guided *** steroid injection at the BP,hydrodissection with 5%dextrose water at the BP and suprascapular nerve,and intra-articular steroid and hyaluronidase injection at the glenohumeral joint were *** postoperative day 194,the pain and arm weakness were resolved,and a follow-up electrodiagnostic study showed marked *** Clinicians should consider the possibilities of multiple nerve injuries in patients with joint contracture,and treat each specific therapeutic target.

BACKGROUNDAutonomic dysreflexia (AD) can be a life-threatening condition in patients withspinal cord injury. It is important to prevent bladder overdistension in thesepatients as it may trigger AD. Sensation-dependent bladder emptying (SDBE), asa method of bladder management, improves the quality of life and allowsphysiologic voiding. In this study, we report disruption of the SDBE habit afterbladder overdistension leading to AD with chest *** SUMMARYA 47-year-old male with a diagnosis of C4 American Spinal Cord InjuryAssociation impairment scale A had been emptying his bladder using the cleanintermittent catheterization method with an itchy sensation in the nose as asensory indication for a full bladder for 23 years, and the usual urine volume wasabout 300-400 mL. At the time of this study, the patient had delayed catheterizationfor approximately five hours. He developed severe abdominal pain andheadache and had to visit the emergency room for bladder overdistension (800mL) and a high systolic blood pressure (205 mmHg). After control of AD, ahypersensitive bladder was observed despite using anticholinergic agents. Thesensation indicating bladder fullness changed from nose itching to pain in theabdomen and precordial area. Moreover, the volume of the painful bladder fillingsensation became highly variable and was noted when the bladder urine volumeexceeded only 100 mL. The patient refused intermittent clean catheterization Finally, a cystostomy was performed, which relieved the *** using physiologic feedback, such as SDBE, for bladder management arerecommended to avoid bladder overdistension.

BACKGROUND Immunoglobulin G4-related disease(IgG4-RD)is a complex immune-mediated condition that causes fibrotic inflammation in several organs.A significant clinical feature of IgG4-RD is hypertrophic pachymeningitis,which manifests as inflammation of the dura mater in intracranial or spinal *** IgG4-RD can affect multiple areas,the spine is a relatively rare site compared to the more frequent involvement of intracranial *** SUMMARY A 70-year-old male presented to our hospital with a two-day history of fever,altered mental status,and generalized *** initial brain magnetic resonance imaging(MRI)revealed multiple small infarcts across various cerebral *** the second day after admission,a physical examination revealed motor weakness in both lower extremities and diminished sensation in the right lower *** evaluation revealed findings consistent with acute motor sensory *** initial management with intravenous immunoglobulin for presumed Guillain-Barrésyndrome,the patient exhibited progressive worsening of motor *** the 45th day of hospitalization,an enhanced MRI of the entire spine,focusing specifically on the thoracic 9 to lumbar 1 vertebral level,raised the suspicion of IgG4-related spinal ***,the patient was administered oral prednisolone and participated in a comprehensive rehabilitation program that included gait training and lower extremity strengthening *** IgG4-related spinal pachymeningitis,diagnosed on MRI,was treated with corticosteroids and a structured rehabilitation regimen,leading to significant improvement.

This paper presents an unbalance response analysis for a rotor bearing system that utilizes a high-density fuel as its working fluid. The extensive simulations with a LP spool rotor indicate that the unbalance response can be significantly varied by manipulation of the fluid supply pressure. It suggests that use of semi-active fluid bearing can be very effective in attenuation of unbalance response of the spool rotor, when the fluid supply pressure is properly manipulated. For comparison, the characteristics of the conventional design using rolling element bearings are also analyzed.

A critical challenge hindering the practical application of lithium–oxygen batteries(LOBs)is the inevitable problems associated with liquid electrolytes,such as evaporation and safety *** study addresses these problems by proposing a modified polyrotaxane(mPR)-based solid polymer electrolyte(SPE)design that simultaneously mitigates solvent-related problems and improves ***-SPE exhibits high ion conductivity(2.8×10^(−3)S cm^(−1)at 25℃)through aligned ion conduction pathways and provides electrode protection ability through hydrophobic chain *** this mPR-SPE into solid-state LOBs resulted in stable potentials over 300 *** situ Raman spectroscopy reveals the presence of an LiO_(2)intermediate alongside Li_(2)O_(2)during oxygen *** situ X-ray diffraction confirm the ability of the SPE to hinder the permeation of oxygen and moisture,as demonstrated by the air permeability *** present study suggests that maintaining a low residual solvent while achieving high ionic conductivity is crucial for restricting the sub-reactions of solid-state LOBs.

Joint On-Demand Pruning and Online Distillation in Automatic Speech Recognition Language Model Optimization

Automatic speech recognition(ASR)systems have emerged as indispensable tools across a wide spectrum of applications,ranging from transcription services to voice-activated *** enhance the performance of these systems,it is important to deploy efficient models capable of adapting to diverse deployment *** recent years,on-demand pruning methods have obtained significant attention within the ASR domain due to their adaptability in various deployment ***,these methods often confront substantial trade-offs,particularly in terms of unstable accuracy when reducing the model *** address challenges,this study introduces two crucial empirical ***,it proposes the incorporation of an online distillation mechanism during on-demand pruning training,which holds the promise of maintaining more consistent accuracy ***,it proposes the utilization of the Mogrifier long short-term memory(LSTM)language model(LM),an advanced iteration of the conventional LSTM LM,as an effective alternative for pruning targets within the ASR *** rigorous experimentation on the ASR system,employing the Mogrifier LSTM LM and training it using the suggested joint on-demand pruning and online distillation method,this study provides compelling *** results exhibit that the proposed methods significantly outperform a benchmark model trained solely with on-demand pruning ***,the proposed strategic configuration successfully reduces the parameter count by approximately 39%,all the while minimizing trade-offs.

Optical properties of graded InGaN/GaN quantum well(QW)lasers are analyzed as improved gain media for laser diodes emitting near 500 *** results are compared with those of conventional InGaN/GaN QW *** heavy-hole effective mass around the topmost valence band is found to nearly not be affected by the inclusion of the graded *** graded InGaN/GaN QW structure shows a much larger matrix element than the conventional InGaN/GaN QW *** radiative current density dependences of the optical gain are similar to each ***,the graded QW structure is expected to have lower threshold current density than the conventional QW structure because the former has a lower threshold carrier density than the latter.

Analysis and Experiment on the Steering Control of a Water-running Robot Using Hydrodynamic Forces

Steering is important for the high maneuverability of mobile robots. Many studies have been performed to improve the maneuverability using a tail. The aim of this research was to verify the performance of a water-running robot steering on water using a tail. Kinematic analysis was performed for the leg mechanism and the interaction forces between the water and the feet to calculate the propulsive drag force of the water. This paper suggests a simplified planar two-link rigid body model to determine the dynamic performance of the robotic platform with respect to the effect of the tail＇s motion. Simulations based on a dynamic model were performed by applying a range of motions to the tail. In addition, a simulation with a Bang-bang controller was also performed to control the main frame＇s yawing locomotion. Finally, an experiment was conducted with the controller, and the simulation and experimental results were compared. These results can be used as a guideline to develop a steerable water-running robot.

Empirical Study on Shapes of the Foot Pad and Walking Gaits for Water-Running Robots

Recently, various kinds ofbiomimetic robots have been studied. Among these biomimetic robots, water-running robots that mimic the characteristics of basilisk lizards have received much attention. However, studies on the performance with respect to different geometric parameters and gaits have been lacking. To run on the surface of water, a water-running robot needs suffi- cient force with high stability to stay above the water. We experimentally measured the performance of the foot pads with different geometric parameters and with various gaits. We measured and analyzed the forces in the vertical direction and rolling angles of five different foot pad shapes： a circular shape, square shape, half-spherical shape, open half-cylinder shape, and closed half-cylinder shape. Additionally, the rolling stabilities of three kinds of gaits： biped, trotting, and tripod, were also empirically analyzed. The results of this research can be used as a guideline to design a stable water-running robot.

Unusual neural tract between injured fornix and pedunculopontine nucleus in a patient with traumatic brain injury

It has been known that four cholinergic nuclei are located in the septal region and basal forebrain （Ch 1： medial septal nucleus, Ch 2： vertical nucleus of the diagonal band, Ch 3： horizontal limb of the diagonal band and Ch 4： nucleus basalis of Meynert） and two are located in the brainstem （Ch 5： pedunculopontine nucleus and Ch 6： laterodorsal tegmental nucleus）（Selden et al., 1998; Lucas-Meunier et al., 2003; Mesulam, 2004; Nieuwenhuys et al., 2007）. The fornix transfers information on episodic mem- ory between the medial diencephalon and the medial temporal lobe （Afifi and Bergman, 2005; Wolk and Budson, 2010）. The fornix is known to obtain cholinergic innervation from Ch 1 and Ch 2, and to project to the hippocampal formation （Selden et al., 1998; Lucas-Meunier et al., 2003; Mesulam, 2004; Nieu- wenhuys et al., 2007; Dere et al., 2008; Naidich and Duvernoy, 2009; Isaacson and Pribram, 2013）.
