Clinical significance of hepatitis B e antigen level measurement during long-term lamivudine therapy in chronic hepatitis B patients with e antigen positive

AIM： To determine the changes of quantitative hepatitis B e antigen （HBeAg） that predicts early detection of non-response or breakthrough to long-term lamivudine （LAM） therapy. METHODS： Among HBeAg positive chronic hepatitis B patients who failed to achieve HBeAg seroconversion within 12 too, we retrospectively analyzed 220 patients who had received LAM more than 24 too. RESULTS： The mean duration of LAM therapy was 36 （range, 24-72） mo. HBeAg seroconversion after the first 12 mo of LAM therapy was achieved in 53 （24.1%） patients. Viral breakthrough was observed in 105 （47.7%） patients. To find out whether the changing patterns of HBeAg levels can predict the outcome of LAM therapy, we analyzed the reduction rates of HBeAg levels during LAM therapy. Using the decrease more than 90% of pretreatment HBeAg levels, the sensitivity and specificity of response were 96.2% and 70.1%, respectively. Patients were divided into 3 groups according to the reduction patterns of the decrease of quantitative HBeAg： decrescendo, decrescendo-crescendo, no change or fluctuating groups. The optimal time to predict non-response or breakthrough was the first 9 mo of therapy. At 9 mo of therapy, 49 （92.5%） of 53 patients who had achieved HBeAg seroconversion were included in the decrescendo group. On the contrary, in the no change or fluctuating group, only four （7.5%） had achieved HBeAg seroconversion. Among patients who did not show the continuous decrease of HBeAg levels at 9 too, 95.2% （negative predictive value） failed to achieve HBeAg seroconversion. CONCLUSION： Almost all patients who failed to show a continuous decrease of HBeAg levels at 9 mo of LAM therapy were non-response or breakthrough. Therefore, monitoring changes of HBeAg levels during LAM therapy in HBeAg positive chronic hepatitis B may be valuable for identifying patients who are at high risk of non-response or breakthrough.

A cobalt-silica hybrid nanocatalyst bearing small cobalt particles of diameter -5 nm was prepared through a hydrothermal reaction and hydrogen reduction. The resulting material showed very high CO conversion （〉82%） and high hydrocarbon productivity （-1.0gHC·gcat^-1·h^-1 ） with high activity （-8.5 × 10^-5 ***^-1.s-1） in the Fischer-Tropsch synthesis reaction.

Objective:To investigate the mechanism of antibacterial activity of luteoiin(LUT) against methicillin-resistant Staphylococcus aureus(MRSA).Methods:The mechanism of anti-MRSA activity of LUT was analyzed by the viability assay in membrane permeabilizing agent ATPase inhibitors,and peptidoglycan(PGN) derived from Staphylococcus aureus(***).Also,transmission electron microscopy was used to monitor survival characteristics and changes in *** ***:Compared to the LUT alone,the optical density of suspensions treated with the combination of 125 μg/mL Tris and 230 μg/mL DCCD were reduced to 60%and 46%,***(15.6 μg/mL) gradually impeded the activity of LUT,and PGN(62.5 μg/mL) completely blocked the activity of LUT on ***:Increased susceptibility to LUT with me Tris and DCCD combinations is evident in all tested MRSA *** results indicate LUT synergy in increasing cytoplasmic membrane permeability and inhibiting *** PGN directly blocks the antibacterial activity of LUT,suggesting the direct binding of LUT with *** findings may be validated for the development of antibacterial agent for low MRSA resistance.

Evaluation of recurrence in gastric carcinoma: Comparison of contrast-enhanced computed tomography and positron emission tomography/computed tomography

AIM To compare the value of contrast-enhanced abdominal computed tomography(CT) and fluorodeoxyglucose(FDG) positron emission tomography/computed tomography(PET/CT) for detecting gastric carcinoma *** We retrospectively examined data from 2475 patients who underwent both contrast-enhanced abdominal CT and FDG PET/CT for the surveillance of gastric carcinoma curative resection. Patients had an interval of less than 1 mo between their CT and PET/CT scans. Sixty patients who had recurrence were enrolled. Among 1896 patients who did not have recurrence, 60 were selected by simple random sampling. All CT and PET/CT images were reviewed retrospectively by two reviewers blinded to all clinical and pathologic information except curative resection due to gastric carcinoma. RESULTS The pathological stage of the recurrence group was statistically significantly higher than that of the control group(P < 0.001). In the 60 patients who had recurrence, there were 79 recurrent lesions. Fortyfour patients had only one location of recurrence, 13 patients had two locations, and 3 patients had three. In the detection of patient-based overall recurrence, no statistically significant differences existed between the two modalities(P = 0.096). However, for peritoneal carcinomatosis, CT had a statistically significantly higher sensitivity compared to PET/CT(96% vs 50%, P = 0.001). Adenocarcinoma was the most common type of gastric carcinoma. On the pathology-based analysis, CT also had a statistically significantly higher sensitivity compared to PET/CT(98% vs 80%, P = 0.035).CONCLUSION Contrast-enhanced CT was superior to PET/CT in the detection of peritoneal carcinomatosis and pathologic type of adenocarcinoma.

Gyejibokryeong-Hwan improves recovery of injured muscles in mouse model of muscle contusion

OBJECTIVE: To investigate the effects of Gyejibokryeong-Hwan(Guizhifuling-wan, GBH) on muscle injury in a mouse model of muscle ***: C57/BL6 mouse biceps femoris muscles were injured using the drop-mass method and injured animals were treated orally with GBH(50,100, or 500 mg/kg) once a day for 7 d. Open field and treadmill running tests were performed to assess functional recovery from muscle injury. The production of pro-inflammatory cytokines was examined by enzyme-linked immunosorbent assay and Western blotting analysis. Expression of the muscle regeneration biomarkers, myoblast determination(Myo D), myogenic factor 5(Myf5), and smooth muscle actin(α-SMA), in the biceps femoris muscle was investigated at the protein and m RNA level by Western blotting and real time-PCR, respectively. Histological analysis was performed using hematoxylin and eosin staining. Finally, myosin heavy chain production was investigated in differentiated C2C12 myoblasts in the presence of ***: GBH treatment markedly improved locomotion and running behavior. GBH significantly inhibited the secretion of monocyte chemoattractant protein-1 into the bloodstream in muscle-contused animals. The levels of Myo D, Myf5, and α-SMA protein and m RNA were significantly up-regulated by GBH in injured muscle tissue. Histological studies suggested that GBH facilitated recovery from muscle damage. However, GBH did not induce the production of myosin heavy chain in ***: Overall, the present study suggested that GBH improves the recovery of the injured muscles in the mouse model of muscle contusion.

Increased incidence of chemoport-related thrombosis in patients with colorectal cancer receiving bevacizumab: A single-institutional experience

Objective： Chemoport-related thrombosis （CRT） is a serious complication that causes morbidities and interruptsadministration of intravenous cancer therapy. We investigated the incidence and risk of CRT in colorectal cancer（CRC） patients treated with bevacizumab （BEV）.Methods： We retrospectively reviewed 1,534 CRC patients who received chemotherapy with or without BEVusing a chemoport between 2014 and ***： The participants had a median age of 58 （18-85） years, and 60.3% were male. All participants werestratified into three groups： adjuvant chemotherapy （AC）（n=670）, palliative chemotherapy （PC） without BEV（n=356）, and PC with BEV （n=508）. The median follow-up was 20.19 （interquartile range, 14.07-27.19） *** occurred in 3.8% of all patients; incidence of symptomatic and asymptomatic CRT was 2.9% and 0.9%,respectively. CRT occurred more in patients with BEV （5.7%） than in patients without BEV （2.9%, P=0.008）. Thecumulative incidence of CRT in patients administered PC with BEV was significantly higher than that in thoseadministered AC （P=0.011） and there was a trend toward increased CRY in patients administered PC with BEVcompared with those administered PC without BEV （P=0.044）. Multivariate analysis found that BEV treatment wasthe only variable that was significandy associated with CRT （hazard ratio, 2.06; 95% confidence interval, 1.24-3.43;P=0.006）.Conclusions： BEV treatment was significandy associated with increased incidence of CRT in CRC patients.

BACKGROUND Although the criteria for the indication of endoscopic submucosal dissection(ESD)for undifferentiated early gastric cancer(UD-EGC)have been recently proposed,accumulating reports on the non-negligible rate of lymph node metastasis(LNM)after indicated ESD raise questions on the reliability of the current *** To investigate the prevalence and risk factors of LNM in UD-EGC cases meeting the expanded indication for *** We retrospectively reviewed 4780 UD-EGC cases that underwent surgical resection between January 2008 and February 2019 at Asan Medical Center,a tertiary university hospital in *** identify the risk factors of LNM of UDEGC meeting the expanded criteria for ESD,we performed a case-control study by matching the cases with LNM to those without at a ratio of 1:*** reviewed the clinical,endoscopic,and histologic features of the cases to identify features with a significant difference according to the presence of *** and multivariate logistic regression analyses were performed to estimate the odds ratios(ORs).RESULTS Of the 4780 UD-EGC cases,1240(25.9%)were identified to meet the expanded indication for *** the 1240 cases,14(1.1%)cases had *** the various clinical,endoscopic,and histopathological features that were evaluated,mixed histology(tumors consisting of 10%-90%of signet ring cells)had a marginally significant association(P=0.059)with the risk of ***,diffuse blurring of the muscularis mucosae(MM)underneath the tumorous epithelium,a previously unrecognized histologic feature,had a significant association with the absence of LNM(P=0.028).Multivariate logistic regression analysis showed that the blurring of MM was the only explanatory variable significantly associated with a reduced risk of LNM(OR:0.12,95%CI:0.02-0.95;P=0.045).CONCLUSION The risk of LNM is higher than expected when using the current expanded indication for *** evaluation could provide useful clues for reducing the

Molecular mechanisms of apoptosis induced by Scorpio water extract in human hepatoma HepG2 cells

AIM: To clarify the mechanism underlying the anti-mutagenic and anti-cancer activities of Scorpio water extract (SWE). METHODS: Human hepatoma HepG2 cells were incubated with various concentrations of SWE. After 24-h incubation, cytotoxicity and apoptosis evaluations were determined by MTT and DNA fragmentation assay, respectively. After treatment with SWE, mitochondrial membrane potential (MMP) was determined by measuring the retention of the dye 3,3'-dihexyloxacarbocyanine (DiOC6(3)) and the protein expression including cytochrome C and poly-(ADPribose) polymerase (PARP) were measured by Western blotting. Caspase-3 and -9 enzyme activities were measured using specific fluorescence dyes such as Ac-DEVD-AFC and Ac-LEHD-AFC. RESULTS: We found that treatment with SWE induced apoptosis as confirmed by discontinuous DNA fragmentation in cultured human hepatoma HepG2 cells. Our investigation also showed that SWE-induced apoptosis of HepG2 cells were associated with intracellular events including disruption of MMP, increased translocation of cytochrome C from mitochondria to cytosol, activation of caspase-3, and PARP. Pre-treatment of N-acetyl-Asp-Glu-Val-Asp-CHO (Ac-DEVD-CHO), a caspase-3 specific inhibitor, or cyclosporin A (CsA), an inhibitor of MMP disruption, completely abolished SWE-induced DNA fragmentation. CONCLUSION: These results suggest that SWE possibly causes mitochondrial damage, leading to cytochrome C release into cytosol and activation of caspases resulting in PARP cleavage and execution of apoptotic cell death in HepG2 cells. These results further suggest that Scorpio may be a valuable agent of therapeutic intervention of human hepatomas.

Signal processing for real-time identification of similar metals by laser-induced breakdown spectroscopy

Laser-induced breakdown spectroscopy(LIBS) is regarded as a promising technique for realtime sorting of scrap metals due to its capability of fast multi-elemental and in-air analysis. This work reports a method for signal processing which ensures high accuracy and high speed during similar metal sorting by LIBS. Similar metals such as aluminum alloys or stainless steel are characterized by nearly the same constituent elements with slight variations in elemental concentration depending on metal type. In the proposed method, the original data matrix is substantially reduced for fast processing by selecting new input variables(spectral lines) using the information for the constituent elements of similar metals. Specifically, principal component analysis(PCA) of full-spectra LIBS data was performed and then, based on the loading plots, the input variables of greater significance were selected in the order of higher weights for each constituent element. The results for the classification test with aluminum alloy, copper alloy,stainless steel and cast steel showed that the classification accuracy of the proposed method was nearly the same as that of full-spectra PCA, but the computation time was reduced by a factor of 20 or more. The results demonstrated that incorporating the information for constituent elements can significantly accelerate classification speed without loss of accuracy.
