A Case Series of Patients with Pancreatic Cancer and Cholangiocarcinoma Treated with <i>nab</i>-Paclitaxel at a Single Institution

Background: locally advanced or metastatic pancreatic cancer patients have a poor prognosis with median survival less than 12 months. Nanoparticle albumin bound (nab)-paclitaxel is a novel agent that has demonstrated antitumor effects in cancers that overexpress the albumin binding protein SPARC (secreted protein acidic and rich in cysteine), which includes pancreatic cancer. A recent phase III trial comparing nab-paclitaxel and gemcitabine to gemcitabine alone demonstrated a survival advantage in patients with previously untreated metastatic pancreatic cancer. Here we present our local experience with this drug in patients with gastrointestinal malignancies. Methods: Patients treated with nab-paclitaxel for gastrointestinal malignancies at the Cross Cancer Institute in Edmonton, Alberta, Canada were identified and these patient’s medical records were interrogated for data. Results: Three patients with pancreatic cancer and two with cholangiocarcinoma have been treated with nab-paclitaxel at the Cross Cancer Institute. Three patients achieved stable disease, while one had a partial response, and one had progressive disease after the first assessment. Median time to progression was 3.7 months. Median overall survival (OS) was 32.5 months. Median OS from initiation of nab-paclitaxel was 7.2 months. Patients tolerated treatment with nab-paclitaxel well with only one patient requiring treatment modification due to neutropenia. Conclusion: The experience at this single center supports published evidence that nab-paclitaxel is a safe and effective therapy in pancreatic cancer, but also suggests that it may have activity in cholangiocarcinoma, which to our knowledge is the first published evidence of this in humans.

目的:由于技术原理的限制,目前尚不能对所有的HlA等位基因进行严格的区分,特别是以往没有发现的新基因序列只能通过测序的方法解决,然而,当遇到等位基因杂合时,测序给出的结果仍然无法确认新的序列改变发生在等位基因的哪一侧,这时需要用分子生物学方法分离杂合子然后进行测序才能确定新的基因序列。采用基因克隆方法确认HlA新等位基因。方法:实验于2006-01/05在河南省红十字血液中心HlA实验室,美国海军骨髓库HlA实验室完。成造血干细胞血样由中华骨髓库提供。采用荧光微珠HlA分型方法对中华骨髓库捐献者血样进行HlA分型检测,无法给出确切结果的摸棱两可结果标本用基因克隆(TOPO TA Cloning)、DNA测序的方法确认新的HlA基因序列。结果:通过克隆分离杂台等位基因,再进行测序确认发现新的序列与B~*3709相比,出现4个核苷酸改变:1.355nt C>A,2.363nt C>G,3.412ntG>A,4 477nt C>G,而且均发生在HlA-B基因外显子3(exon 3)。4处改变引起氨基酸编码改变:①编码95 CTC>ATC,氨基酸改变l>I(亮氨酸>异亮氨酸)。②97 AGC>AGG,S>R(丝氨酸>精氨酸)。③114 GAC>AAC D>N(天门冬氨酸>天冬酰胺)。④135 GCC>GCG A=A无氨基酸改变。结论:①新的基因序列已经在GenBnak注册,被WHO的HlA因子命名委员会得到正式命名为HlA-B~*3712基因。②基因克隆是确认HlA新基因的根本方法。

In the absence of photosynthesis, ATP is imported into chloroplasts and non-green plastids by ATP/ADP transporters or formed during glycolysis, the latter requiring continuous regeneration of NAD＋, supplied by the plastidial isoform of NAD-MDH. During screening for T-DNA insertion mutants in the plNAD-MDH gene of Arabidopsis, only heterozygous plants could be isolated and homozygous knockout mutants grew only after complementation. These heterozygous plants show higher transcript levels of an alternative NAD＋-regenerating enzyme, NADH-GOGAT, and, remarkably, improved growth when ammonium is the sole N-source. In situ hybridization and GUS-histochemical stain- ing revealed that plNAD-MDH was particularly abundant in male and female gametophytes. Knockout plNAD-MDH pollen exhibit impaired tube growth in vitro, which can be overcome by adding the substrates of NADH-GOGAT. In vivo, knockout pollen is able to fertilize the egg cell. Young siliques of selfed heterozygous plants contain both green and white seeds corresponding to wild-type/heterozygous （green） and homozygous knockout mutants （white） in a （1：2）：1 ratio. Embryos of the homozygous knockout seeds only reached the globular stage, did not green, and developed to tiny wrinkled seeds. Complementation with the gene under the native promoter rescued this defect, and all seeds developed as wild-type. This suggests that a blocked major physiological process in plNAD-MDH mutants stops both embryo and endosperm development, thus avoiding assimilate investment in compromised offspring.

Responses of Macroinvertebrate Community Metrics to a Wastewater Discharge in the Upper Blue River of Kansas and Missouri, USA

The Blue River Main wastewater treatment facility (WWTF) discharges into the upper Blue River (725 km2), and is recently upgraded to implement biological nutrient removal. We measured biotic condition upstream and downstream of the discharge utilizing the macroinvertebrate protocol developed for Kansas streams. We examined responses of 34 metrics to determine the best indicators for discriminating site differences and for predicting biological condition. Significant differences between sites upstream and downstream of the discharge were identified for 15 metrics in April and 12 metrics in August. Upstream biotic condition scores were significantly greater than scores at both downstream sites in April (p = 0.02), and in August the most downstream site was classified as non-biologically supporting. Thirteen EPT taxa (Ephemeroptera, Plecoptera, Trichoptera) considered intolerant of degraded stream quality were absent at one or both downstream sites. Increases in tolerance metrics and filtering macroinvertebrates, and a decline in ratio of scrapers to filterers all indicated effects of increased nutrient enrichment. Stepwise regressions identified several significant models containing a suite of metrics with low redundancy (R2 = 0.90 - 0.99). Based on the rapid decline in biological condition downstream of the discharge, the level of nutrient removal resulting from the facility upgrade (10% - 20%) was not enough to mitigate negative effects on macroinvertebrate communities.

Ground-level ozone is a harmful air pollutant associated with several health issues. Ozone concentrations have exceeded the National Ambient Air Quality Standards (NAAQS) in the Chicago metropolitan area on hot summer days for many years because of nitrogen oxide and volatile organic compound emissions. Annual fourth highest 8-hour ozone concentrations have been between 0.070 and 0.084 ppm at several monitoring sites in Cook county, during the 2016-2018 time period. The continuous measurement of nitrogen dioxide (NO2) and ozone (O3) was conducted in several communities in Chicago in 2017. The air pollution impacts the health of all who live in the area. The data were used to analyze correlations between the O3 distribution and its association with ambient concentrations of NO2 from transportation emissions. Higher concentrations in NO2 and O3 occurred in succession in the daytime. The diurnal variation of O3 concentration was analyzed. The daily cycle of NO2 concentration reaches a maximum in the late morning and has smaller nighttime concentrations. The daily cycle of ozone concentration reaches the maximum in the afternoon and also becomes smaller for nighttime concentrations. In addition, relationships were found between O3 and NO2. Monthly variations of ozone and NO2 are presented. Some options to reduce ozone pollution are presented.
