Objective:Intra-abdominal fat is a risk factor for pancreatic cancer(PC),but little is known about its contribution to PC precursors known as intraductal papillary mucinous neoplasms(IPMNs).Our goal was to evaluate quantitative radiologic measures of abdominal/visceral obesity as possible diagnostic markers of IPMN severity/***:In a cohort of 34 surgically-resected,pathologically-confirmed IPMNs(17 benign;17 malignant) with preoperative abdominal computed tomography(CT) images,we calculated body mass index(bMI) and four radiologic measures of obesity:total abdominal fat(TAF) area,visceral fat area(VFA),subcutaneous fat area(SFA),and visceral to subcutaneous fat ratio(V/S).Measures were compared between groups using Wilcoxon two-sample exact tests and other ***:Mean bMI for individuals with malignant IPMNs(28.9 kg/m^2) was higher than mean bMI for those with benign IPMNs(25.8 kg/m^2)(P=0.045).Mean VFA was higher for patients with malignant IPMNs(199.3 cm^2) compared to benign IPMNs(120.4cm^2),P=0.092.V/S was significantly higher(P=0.013) for patients with malignant versus benign IPMNs(1.25 vs.0.69 cm^2),especially among *** accuracy,sensitivity,specificity,and positive and negative predictive value of V/S in predicting malignant IPMN pathology were 74%,71%,76%,75%,and 72%,***:Preliminary findings suggest measures of visceral fat from routine medical images may help predict IPMN pathology,acting as potential noninvasive diagnostic adjuncts for management and targets for intervention that may be more biologically-relevant than *** investigation of gender-specific associations in larger,prospective IPMN cohorts is warranted to validate and expand upon these observations.

Clinical Pharmacist and Physician Team Collaboration to Improve Medication Safety and Cost Savings in an Inpatient Medicine Unit： A Prospective Cohort Study

background： To prevent adverse drug events and promote patient safety, medication reconciliation is critical in all patient care settings. The purpose of this study was to identify medication discrepancies occurring in an inpatient medicine unit and to analyze the clinical and economic benefit of clinical pharmacist and physician team collaboration. Methods： A prospective cohort study in which pharmacist attended daily team rounds and assisted with medication management and medication reconciliation on admission and discharge in an academic hospital with internal medicine residents. All interventions related to medication management were categorized based on error type, severity of harm, preventable, non-preventable and potential adverse drug events. The economic outcome associated with these medication errors was analyzed. Results： There were 160 admissions and 179 pharmacist recommendations with a 91% acceptance rate from physicians. There were 145 discharges during the study period of which 104 medication discrepancies were identified. Eighty nine of the medication discrepancies were corrected by the pharmacist within 72 hours of discharge. Pharmacist identified 11 actual adverse drug events. Cost savings from pharmacist interventions during the study period was $11,652 and cost avoidance from intercepting potential and actual adverse drug events was $256,806. Conclusion： Collaboration of pharmacist with a physician team improved medication safety and led to significant cost savings and cost avoidance.

Diabetes is a leading chronic disease of childhood and adolescence. In addition to the well-known auto-immune, insulin-dependent diabetes mellitus （type 1 diabetes （T 1D））, the past two decades have witnessed the emergence of type 2 diabetes （T2D） in children and adolescents, which previously was only seen in middle-aged or older adults. One of the key components of diabetes management is physical activity （PA）. The beneficial effects of increased PA and decreased sedentary behavior are extremely important in youth with diabetes because of the markedly increased long-term risk of cardiovascular disease in this population compared to persons without diabetes. This review aims to comprehensively summarize the epidemiologic, observational research published and listed in PubMed between 1970 and 2012 on PA and sedentary behaviors, as well as physical fitness in children and adolescents with T1D and T2D. Additionally, we describe briefly the state of knowledge on perceived barriers of PA in persons with diabetes, with a focus on hypoglycemia. Finally, we provide an overview of the epidemiological literature pertaining to health benefits of increased PA in youth with TID and T2D and briefly discuss the topic of exercise-related hypoglycemia, Copyright ~ 2012, Shanghai University of Sport. A Case Series of Patients with Pancreatic Cancer and Cholangiocarcinoma Treated with <i>nab</i>-Paclitaxel at a Single Institution

background: Locally advanced or metastatic pancreatic cancer patients have a poor prognosis with median survival less than 12 months. Nanoparticle albumin bound (nab)-paclitaxel is a novel agent that has demonstrated antitumor effects in cancers that overexpress the albumin binding protein SPARC (secreted protein acidic and rich in cysteine), which includes pancreatic cancer. A recent phase III trial comparing nab-paclitaxel and gemcitabine to gemcitabine alone demonstrated a survival advantage in patients with previously untreated metastatic pancreatic cancer. Here we present our local experience with this drug in patients with gastrointestinal malignancies. Methods: Patients treated with nab-paclitaxel for gastrointestinal malignancies at the Cross Cancer Institute in Edmonton, Alberta, Canada were identified and these patient’s medical records were interrogated for data. Results: Three patients with pancreatic cancer and two with cholangiocarcinoma have been treated with nab-paclitaxel at the Cross Cancer Institute. Three patients achieved stable disease, while one had a partial response, and one had progressive disease after the first assessment. Median time to progression was 3.7 months. Median overall survival (OS) was 32.5 months. Median OS from initiation of nab-paclitaxel was 7.2 months. Patients tolerated treatment with nab-paclitaxel well with only one patient requiring treatment modification due to neutropenia. Conclusion: The experience at this single center supports published evidence that nab-paclitaxel is a safe and effective therapy in pancreatic cancer, but also suggests that it may have activity in cholangiocarcinoma, which to our knowledge is the first published evidence of this in humans.

EUS guided fine needle aspiration is useful in diagnostic evaluation of indeterminate proximal biliary strictures

background and Aim: Determination of the etiology of proximal biliary strictures (PbS) still remains a challenge. Even though EUS-FNA is potentially useful, its role in diagnostic evaluation of PbS is still not established due to a lack of sufficient data. We sought to determine the performance characteristics of EUS-FNA in patients with obstructive jaundice (ObJ) and PbS. Patients and Methods: This is a retrospective analysis and included patients with ObJ and PbS with non-diagnostic brush cytology, who underwent EUS-FNA in our university based practice from March 2002 to February 2009. We evaluated the final diagnoses in study patients and the performance characteristics of EUS-FNA for identifying malignant PbS. Final diagnosis was based on surgical pathology or clinical follow-up of at least 12 months. Results: The study included 28 patients (17 male, 11 female) with mean age of 62.4 ± 14.9 years. The strictures ranged from 10 - 70 mm in length. CT scan identified a hilar mass in 9 patients. EUS-FNA identified malignancy in 12 patients. 18 patients were finally diagnosed to have a malignant stricture and 10 patients had a benign stricture. There were 6 false negative diagnoses. The accuracy of EUS-FNA for diagnosis of cholangiocarcinoma was 78.6%, with 66.6% sensitivity, 100% specificity, 62.5% NPV and 100% PPV. Conclusions: EUS-FNA can diagnose malignancy in 66.6% of jaundiced patients with PbS and non-diagnostic brush cytology and may have a role in their further diagnostic evaluation.

Hexabromocyclododecane(HbCDD)is listed under the Stockholm Convention on persistent organic pollutants,yet very few data are available on HbCDD concentrations in *** concentrations of total hexabromocyclododecanes(SHbCDDs)from soils from the UK(n=24)were 0.73 ng g^-1 dry weight(rangebiphenyls(PCbs)(average=4.7 ng SPCbs g^-1,range=0.39-21 ng g^-1)were determined in 19 UK samples and found to be statistically indistinguishable(p>0.05)from those of HbCDDs;thereby underlining the extent to which HbCDDs have migrated into the UK ***,PCb concentrations in this study are not markedly lower than those recorded in UK soils sampled in the mid-1980s indicating that the initial rapid decline in UK contamination with PCbs following bans on their manufacture and use,has not been *** products of HbCDD:pentabromocyclododecenes(PbCDs)and tetrabromocyclododecadienes(TbCDs)were detected in some UK soil samples with semi-quantitative concentrations ranging between 0.01 and 7.3 ng g^-1 for SPbCDs and 0.01e1.3 ng g^-1 for *** Australian soils only STbCDs were detected at concentrations ranging from 0.0023 to 0.45 ng g^-*** signatures of HbCDDs were racemic or non-racemic in all samples indicating minimal edaphic enantioselective degradation.A horizontal transect at the most contaminated UK location(a suburban garden)revealed a marked decrease in concentrations of HbCDDs with increasing distance from buildings.

High-temperature magnetic properties of anisotropic Mn bi/Nd Fe b hybrid bonded magnets

Anisotropic Mnbi/NdFeb （Mnbi contents of 0 wt%, 20 wt%, 40 wt%, 60 wt%, 80 wt%, and 100 wt%） hybrid bonded magnets were prepared by molding compression using Mnbi powders and commercial hydro-genation disproportionation desorption and recombination （HDDR） NdFeb powders. Magnetic measurements at room temperature show that with Mnbi content increasing, the magnetic properties of the Mnbi/NdFeb hybrid bonded magnets all decrease gradually, while the density of the hybrid magnets improves almost linearly. In a temperature range of 293-398 K, the coercivity temperature coefficient of the hybrid magnets improves gradually from -0.59 %.K^-1 for the pure NdFeb bonded magnet to -0.32 %.K^-1 for the hybrid bonded magnet with 80 wt% Mnbi, and the pure Mnbi bonded magnet exhibits a positive coercivity temperature coefficient of 0.61%-K^-1.
