Background High deductible health plans(HDHPs)are associated with the avoidance of both necessary and unnecessary *** hernia repair(UHr)is a procedure that is frequently unnecessarily performed in young children,contrary to best practice *** hypothesized that children with HDHPs,as compared with other types of commercial health plans,are less likely to undergo UHr before 4 years of age but are also more likely to have UHr delayed beyond 5 years of *** Children aged 0-18 years old residing in metropolitan statistical areas(MSAs)who underwent UHr in 2012-2019 were identified in the IBM Marketscan Commercial Claims and Encounters Database.A quasi-experimental study design using MSA/year-level HDHP prevalence among children as an instrumental variable was employed to account for selection bias in HDHP ***-stage least squares regression modeling was used to evaluate the association between HDHP coverage and age at *** A total of 8601 children were included(median age 5 years,IQr 3-7).Univariable analysis revealed no differences between the HDHP and non-HDHP groups in the likelihood of UHr being performed before 4 years of age(27.7%vs 28.7%,p=0.37)or after 5 years of age(39.8%vs 38.9%,p=0.52).Geographical region,metropolitan area size,and year were associated with HDHP *** variable analysis demonstrated no association between HDHP coverage and undergoing UHr atrs of age(p=0.76)or>5 years of age(p=0.87).Conclusions HDHP coverage is not associated with age at pediatric *** studies should investigate other means by which UHrs in young children can be avoided.

Background: Pharyngeal and anorectal reservoirs of gonorrhea(GC) and chlamydia(CT) are increasingly recognized among heterosexual women. While a number of studies performed at sexually transmitted disease(STD) clinics have found a high prevalence of extragenital GC/CT infection, such screening is typically not offered during routine primary care visits for women. We sought to define the prevalence of and factors associated with extragenital GC/CT among women in the U.S. ***: We recruited servicewomen stationed in San Diego, California, between the ages of 18 and 25 who presented for an annual physical exam between January and September, 2017. Nucleic acid amplification testing was performed on swabs collected at endocervical, oropharyngeal and anorectal sites to assess the presence of GC/CT. An anonymous behavioral questionnaire was also administered to characterize sexual risk. Descriptive statistics were used to compare women with and without a prior history of any sexually transmitted infection(STI)(self-report) along with a current GC/CT diagnosis. This study was approved by the Institutional review Board of the Uniformed Services University of Health ***: Of the 75 patients who were approached, 60 subjects were enrolled in the study, including white 20(33.3%), black/African American 18(31.0%), Hispanic/Latina 13(21.7%) and Asian/Pacific Islander 9(15.5%) women. Among all the women, six(10.0%) were diagnosed with CT infection, all via endocervical exam. Of these, five(8.3%) had concurrent anorectal infection, including two cases(3.3%) accompanied by pharyngeal infection. Of the subjects, 15(25.0%) reported anal intercourse in their most recent sexual encounter, most of which was condomless(13/15, 86.7%). A high number of women who reported sex with a casual male partner(19/45, 42.2%) reported rarely or never using condoms;last, 41.7% consuming at least 3 drinks on a typical drinking day, and one-third of the reported drinking more than once

Objective To determine the prevalence and associations of general practice registrars’performing absolute cardio-vascular risk(ACVr)assessment(ACVra).Design A cross-sectional study employing data(2017–2018)from the registrar Clinical Encounters in Training project,an ongoing inception cohort study of Australian GP *** outcome measure was whether an ACVra was *** employed univariable and multivariable *** was conducted for all patient problems/diagnoses,then for an‘at-risk’population(specific problems/diagnoses for which ACVra is indicated).Setting Three GP regional training organisations(rTOs)across three Australian *** GP registrars training within participating *** 1003 registrars(response rate 96.8%)recorded details of 69105 problems either with Aboriginal and/or Torres Strait patients aged 35 years and older or with non-Indigenous patients aged 45 years and *** these problems/diagnoses,1721(2.5%(95%CI 2.4%to 2.6%))involved an *** ACVra was‘plausibly indicated’in 10384 problems/*** these,1228(11.8%(95%CI 11.2%to 12.4%))involved ***‘all problems/diagnoses’,on multivariable analysis female gender was associated with reduced odds of ACVra(Or 0.61(95%CI 0.54 to 0.68)).There was some evidence for Aboriginal and/or Torres Strait Islander people being more likely to receive ACVra(Or 1.40(95%CI 0.94 to 2.08),p=0.10).There were associations with variables related to continuity of care,with reduced odds of ACVra:if the patient was new to the registrar(Or 0.65(95%CI 0.57 to 0.75)),new to the practice(Or 0.24(95%CI 0.15 to 0.38))or the problem was new(Or 0.68(95%CI 0.59 to 0.78));and increased odds if personal follow-up was organised(Or 1.43(95%CI 1.24 to 1.66)).For‘ACVra indicated’problems/diagnoses,findings were similar to those for‘all problems/diagnoses’.Association with Aboriginal and/or Torres Strait Islander status,however,was significant at pr 1.60(95%CI 1.04 to 2.46))a

[背景]越来越多的证据显示,母亲空气污染暴露与对胎儿生长的不良影响之间存在关联;但是,现有的文献并不一致。[目的]研究9个国家14个中心的数据,量化母亲颗粒物空气污染暴露与足月出生体重和低出生体重(LBW)之间的关联,并探讨此关联中地点特征和暴露评估方法对中心之间异质性的影响。[方法]空气污染与妊娠结局国际合作(ICAPPO)的各中心采用共同的分析方案,对PM10和PM2.5(颗粒物≤10和2.5μm)与足月LBW和连续出生体重进行效应评估。采用meta分析将各个中心(~300万例出生)的效应评估结合起来,并采用meta回归分析在报告的效应评估中,中心特征和暴露评估方法对中心之间异质性的影响。[结果]随机效应meta分析显示,调整母亲的社会经济地位后,在整个妊娠期间,足月LBW与PM10[比值比(Or)=1.03;95%CI:1.01~1.05]和PM2.5[Or=1.10;95%CI:1.03~1.18]每增加10μg/m3呈正相关。在充分调整随机效应的meta分析中,作为一个连续性的结果,PM10暴露每增加10μg/m3与足月出生体重呈负相关(-8.9g;95%CI:-13.2^-4.6 g)。meta回归分析显示,具有较高平均PM2.5水平和PM2.5∶PM10比率的中心,以及采用时间暴露评估(与时空相比)的中心,所报告的关联更强。[结论]在研究人群中,母亲颗粒物污染暴露与足月LBW相关联。有3个地点特征和暴露评估方法学方面的不同导致各个中心报告的关联的差异。
