Community-acquired pneumonia(CAP)remains the leading cause of morbidity and mortality among children *** is critical for these patients to select and timely initiate appropriate empirical antimicrobial therapy against the causative pathogens[1].However,conventional pathogen-detecting methods,such as culture and serology,have no prospect of altering empiric therapy owing to their time delay in obtaining results and to their lower detection rates[2].Recent advances in molecular diagnostic assays,such as multiplex polymerase chain reaction PCR(mPCR)methods,have been used to detect multiple pathogens in CAP simultaneously within two hours and have dramatically improved the ability to diagnose respiratory pathogens[3].However,whether this molecular diagnosis method can reduce the use of antibiotics and can improve prognosis in severe CAP children,especially those less than 5 ye.rs old,remains to be explored.

The recommendation encouraging patients with cancer to k.ep a normal body mass index(BMI)is largely extrapolated from data on risk.of developing *** tested the prospective association between peri-diagnostic(within 1 ye.r post-diagnosis)BMI and all-cause mortality in patients with incident *** 7.2 ye.rs of follow-up,42%(48,340)of the 114430 patients with cancer *** analysis revealed that compared with a BMI of 22.5,a BMI lower than 22.5 was associated with increased risk.of all-cause mortality across 24 cancer types.A BMI higher than 22.5 was associated with reduced all-cause mortality,while a non-linear association was observed;the lowest risk.was found at a BMI of 29.6–34.2,and the risk.started to return to and above unity at very high BMI *** reduced mortality risk.of high BMI was observed in 23 of 24 cancer types and maintained after attempts to remove potential selection bias,confounding by smok.ng and comorbidities,and reserve causality.

Back.round The initiation and expansion of China＇s national free antiretroviral therapy program has led to significant improvement of survival among its participants. Success of further scaling up treatment coverage rests upon intensifying HIV screening and efficient link.ge of care. Timely CD4 cell count testing after HIV diagnosis is necessary to determine whether a patient meets criteria for antiretroviral treatment, and represents a crucial link.to engage HIV-infected patients in appropriate care, which has not been evaluated in *** We evaluated all patients ≥16 ye.rs who tested HIV positive from 2005 to 2009 in Yunnan and *** Logistic regression models were applied to identify factors associated with lack.of CD4 cell count testing within 6 months after HIV *** A total of 83 556 patients were included. Over the study period, 30 635 （37%） of subjects received a CD4 cell count within 6 months of receiving the HIV diagnosis. The rate of CD4 cell count testing within 6 months of HIV diagnosis increased significantly from 7% in 2005 to 62% in 2009. Besides the earlier ye.rs of HIV diagnosis, negative predictors for CD4 cell count testing in multivariate analyses included older age, not married or unclear marriage status,incarceration, diagnosis at sexual transmitted disease clinics, mode of HIV transmission classified as men who have sex with men, intravenous drug users or transmission route unclear, while minority ethnicity, receipt of high school or higher education, diagnosis at voluntary counseling and testing clinics, and having HIV positive parents were *** Significant progress has been made in increasing CD4 testing among newly diagnosed HIV positive patients in Yunnan and Guangxi from 2005-2009. 目的探讨母亲孕期体重增长是否存在不同模式,考察孕前体质指数(BMI)对孕妇体重轨迹变化的影响。方法广西壮族孕妇前瞻性出生队列作为研究基础,选择2015年2月—2018年4月收自平果、隆安、德保、田东等县级医院共2409人,获取研究对象基本人口学信息、产检信息,运用潜变量增长模型、Wald检验和χ^2检验进行分析。结果在不分组的模型拟合中,确定了4条不同的孕期体重增长轨迹(人数比例:A组566人,B组1047人,C组624人,D组172人),各组均在孕10周出现速率转折点,初始体重越高的孕妇,年龄越大(P<0.05)、城市居住(P<0.001)、较大孕前BMI(P<0.001)和规律运动(P=0.088)的比例越高。在按孕妇孕前BMI分组的模型拟合中,低体重组、正常组、超重组孕妇均可分别拟合多个亚型,曲线截距差异与斜率差异均有统计学意义(均P<0.001),均有初始体重越大,增速越快。结论广西壮族地区孕妇孕10周后体重增长速率加快;孕前体重越大的孕妇,孕早期体重增速越快。壮族孕妇孕期体重增加可能具有一定的民族和地方特点,在制定周体重增加率的推荐标准时应考虑拐点的不同。城市妇女、年龄越大,孕期体重增长速率越快。应加强对城市大龄超重孕妇孕期体重的监测。

＜正＞Recently DuPont,Nanjing University and BCEG began collaborative work.at a former chemical manufacturing site in Nanjing City,*** goal was to use this as a pilot ortest site to conduct a remediation *** relatively small former chemical manufacturing site is located adjacent to other chemical manufacturing operations,roadways,a housing development,and community vegetable *** the project began,old buildings have been razed,several phases of soil and groundwater sampling have been completed,and that work.has provided data sufficient to build apreliminary conceptual model and to conduct a screening-level human health risk.*** contamination in surficial and sub-surface soils appears to be relatively limited,some contaminants have been found in groundwater *** of the contaminants in groundwater are found at levels above risk.based screening levels suggesting the need for additional investigation to identify source material and understand fate and transport within the subsurface soil and *** addition,further sampling is underway in the small community gardens to determine if vegetables are being grown in contaminated soil. These current data indicate there will lik.ly be a need to evaluate remedial options that will prevent migration of contaminants through the subsurface,and which will mitigate potential human exposures to contaminants. In this presentation we will discuss the development of the conceptual model,initial results of the risk.assessment,and steps to begin evaluating potential remedial options.

需要日常照料的老年人数量正在急速增长。即使老年人自理能力不断减弱并越来越趋向于独居,当代社会的主流选择与政策目标仍着重于协助他们在原先的居所度过晚年。然而现阶段,美国和其他很多国家的住房均未针对日益严峻的老龄化挑战采取应对措施,为居家老年人提供的公共服务与支持相当匮乏。本文基于满足老年人在衰弱过程中的各项需求,从空间和社会的视角系统地检视了社区和住房层面的创新模式,内容包括服务强化型社区、集中照料型社区、全龄型社区,为老人服务型住房组团,小规模代际融合居住型住房,以及交通、输送和通信等方面技术手段的创新。这些对策目前尚未得到广泛应用,但有理由相信这些手段有助于推进居家和社区养老战略目标的实现。规划与设计人员应认识到老年人在生理、感官、认知障碍和独居时面临的挑战,利用设计手段改善公共空间及其连接性。
