AIM: To investigate the prevalence of advanced polyps in asymptomatic Chinese and to determine the risk of proximal advanced colonic polyps in subjects with and without polyps in the distal ***: Data were collected prospectively during colonoscopic examinations performed in 5 973 subjects as part of health evaluation at our unit from December 1997 to December 2003. Polyps were considered advanced, if they were larger than 10 mm or were tubovillous, villous or malignant. Proximal colon was defined as the splenic flexure and more proximal portions of the ***: Colon polyps were detected in 971 (16.3%)subjects (613 males and 358 females) with their mean age being 56.6±10.7 years. Advanced polyps were noted in 199 (3.3%) individuals. Subjects were sub-classified according to the location of polyps into three groups: distal (569, 58.6%), proximal (284, 29.2%), and combined proximal and distal (118, 12.2%) groups. Subjects with advanced polyps in these three groups were 95 (9.8%),56 (5.8%), and 48 (4.9%) respectively. In the 48 subjects with advanced combined polyps, 13 advanced polyps were distributed at the distal colon, 17 at the proximal colon,and 18 at both. Eighteen colon cancers including 12 at sigmoid and 6 at ascending colon were confirmed by final pathology. The relative risk for advanced proximal polyp according to distal findings was 3.1 (95%CI: 1.3-7.4) for hyperplastic polyp, 2.7 (95%CI: 1.4-5.3) for tubular polyp and 13.5 (95%CI: 5.1-35.4) for advanced polyp as compared to that for no polyp. However, 56 (28.2%) of 199 subjects with advanced polyps had no index polyps at the distal colon and might go undetected under sigmoidoscopic ***: Although distal lesions can predict the risk of advanced proximal polyps, a substantial portion of Chinese with advanced proximal polyps is not associated with any distal sentinel lesions. These data have implications for screening policy of colon cancers in Taiwan Residents Chinese.

BACKGROUND Hepatitis C virus (HCV) infection is a documented risk factor for chronic kidney disease (CKD) and progression to end-stage renal disease (ESRD). However, to date there are no reports on the long-term hard endpoints (ESRD and death) of anti-HCV therapy [interferon-based therapy (IBT) or new direct-acting antivirals] in CKD patients. Direct-acting antivirals are not available in Taiwan’s singlepayer national health insurance database currently released for research. Therefore, we hypothesized that a retrospective analysis of the long-term outcomes of IBT in CKD patients will serve as a proxy for direct-acting antivirals to increase our understanding of progression to ESRD following HCV infection. AIM To evaluate the long-term outcomes (ESRD and death) of anti-HCV therapy, especially IBT, in HCV-infected patients with stage 1-5 CKD. METHODS We analyzed 93894 Taiwan Residents adults diagnosed with CKD and without HBV infection. Of these, 4.9% were infected with HCV. Of the 4582 HCV-infected CKD patients, 482 (10.5%) received IBT (treated cohort). They were matched 1:4 with 1928 untreated HCV-infected CKD patients (untreated cohort) by propensity scores and year, which further matched 1:2 by propensity scores with 3856 CKD patients without HCV infection (uninfected cohort). All participants were followed until the occurrence of ESRD, death, or the end of 2012. The association between HCV infection, IBT use, and risks of ESRD and death was analyzed using competing risk analysis. RESULTS Taking the uninfected cohort as a reference, the adjusted hazard ratios for ESRD, after adjusting for competing mortality, were 0.34 (0.14-0.84, P = 0.019) and 1.28 (1.03-1.60, P = 0.029) in the treated and untreated cohorts, respectively. The treated cohort had a 29%(0.54-0.92, P = 0.011) decrease in mortality compared to the untreated cohort, in which the mortality was 31%(1.18-1.45, P < 0.001) higher than in the uninfected cohort. Background： To understand an effective golf swing, both swing speed and impact precision must be thoroughly and simultaneously examined. The aim of this study was to perform both swing speed test and impact precision test to ascertain what swing type determines an effective impact. Methods： Seven golfers from a college team （handicap： 0--12） were recruited to complete a swing speed test and impact precision test using a 5-iron club, A force plate and electromyography （EMG） system were used to collect data in the swing speed test to compare the difference between two motion sequences. High speed video cameras were used to determine the displacement of rotation center for impact precision test. Results： The results showed a significant difference （p 〈 0.01） in clubhead speed with different motion sequences and muscle contraction patterns. In the impact precision test, the displacement of the rotation center which defined as the inner center point of the C7 was significantly different （p 〈 0.05） between different ball impacted marks on club face. Conclusion： The vertical peak ground reaction force on left foot occurring before impact and the left latissimus dorsi contracting prior to the right pectoralis major represent a superior skill by allowing the club to strike the ball with normal collision at a faster speed.

Purpose:The purpose of this study was to compare the effects of an 8-week whole-body vibration training program in various frequency and amplitude settings under the same acceleration on the strength and power of the knee ***:Sixty-four young participants were randomly assigned to 1 of 4 groups with the same acceleration(4 g):high frequency and low amplitude(n = 16,32 Hz,1 mm) group,medium frequency and medium amplitude(n = 16,18 Hz,3 mm) group,low frequency and high amplitude(n = 16,3 Hz,114 mm) group,and control(n = 16,no vibration) *** participants underwent 8 weeks of training with body mass dynamic squats,3 sessions a ***:The results showed that the high frequency and low amplitude group increased significantly in isometric contraction strength and 120°/s isokinetic concentric contraction strength;the medium frequency and medium amplitude group increased significantly in 60°/s and 120°/s isokinetic strength of both concentric and eccentric contraction;and the low frequency and high amplitude group increased significantly in 60°/s and 120°/s isokinetic eccentric contraction ***:All frequency and amplitude settings in the 8-week whole-body vibration training increased muscle strength,but different settings resulted in various neuromuscular adaptations despite the same intensity.

Digital infrared thermography is suitable for monitoring the planar two-dimensional temperature distribution of curved surfaces of objects by sensing their infrared radiation. Cardiac infrared thermography also has a thermal coronary angiography alias. This study proposes a digital image processing methodology for locating blood clot blockage. This methodology contains four consequent processes. The two-dimensional gray scale infrared thermograph pixels are first binarized and classified as background or coronary arteries using multi-thresh adaptive *** coronary artery contours are extracted from segmented raw pixels using continuous pepper-like pixel removal,erosion,subtraction,recursive neighborhood visiting,contour point-list construction and short edge *** the third process one coronary artery branch is selected by physicians for calculating the longest curved central axis using morphological thinning and neighborhood analysis. In the last process the nearest left and right distances from each pixel along the directional central axis to its corresponding boundary contour are added as the coronary artery variable diameter at the current pixel's position. A variable diameter versus straighten length diagram along this axial curved path is plotted to provide useful physiological information to the physician. An obstruction rate equation is then defined to calculate the possible vascular blockage positions with the local minimal rates. Finally,preoperative cases are tested to prove the predictive positions are correct in comparison to individual patient myocardial perfusion imaging.

Diabetes mellitus affects people worldwide,and management of its acute complications or treatment-related adverse events is particularly important in critically ill *** reports have confirmed that hyperglycemia can increase the risk of mortality in patients cared in the intensive care unit(ICU).In addition,severe and multiple hypoglycemia increases the risk of mortality when using insulin or intensive antidiabetic *** innovation of continuous glucose monitoring(CGM)may help to alert medical caregivers with regard to the development of hyperglycemia and hypoglycemia,which may decrease the potential complications in patients in the *** major limitation of CGM is the measurement of interstitial glucose levels rather than real-time blood glucose levels;thus,there will be a delay in the treatment of hyperglycemia and hypoglycemia in ***,the European Union approved a state-of-art artificial intelligence directed loop system coordinated by CGM and a continuous insulin pump for diabetes control,which may provide a practical way to prevent acute adverse glycemic events related to antidiabetic therapy in critically ill *** this mini-review paper,we describe the application of CGM to patients in the ICU and summarize the pros and cons of CGM.

Objective： To investigate the effects of soluble factors secreted by acute myeloid leukemia （AML） cells on the phenotypical and functional properties of DCs derived from normal mononuclear cells. Methods： Mononuclear cells were cultured with interleukin-4 （IL-4） and granulocyte-macrophage colony-stimulating factor （GM-CSF）, in the presence or absence of 24 h culture supematants from fresh pdmary AML cells, to generate immature DCs. The maturation of DCs was induced by cytokines IL-lbeta, IL-6, tumor necrosis factor-alpha （TNF-alpha）, and prostaglandin-2 （PGE-2）. The phenotypic alterations of DCs and DCs-primed CD4＋ T cells were evaluated using flow cytometry. Precursor frequency （PF） was calculated to monitor the allostimulatory effects of DCs on CD4^＋ and CD8^＋ T cells. Results：AML cell supernatant-treated DCs showed significantly lower expression of co-stimulatory molecules CDS0 and CD86, and reduced response to cytokines IL-1beta, IL-6, TNF-alpha, and PGE-2. The allostimulatory effects of AML cell supematant-treated DCs on CD4^＋ and CD8^＋ T cells were significantly lower than those of normal mature DCs [PF：（1.8 ±0.5）% vs. （5.2 ± 1.6）% for CD4^＋ T cells, （2.1 ±0.6）% vs. （6.5 ± 2.0）% for CD8^＋ T cells, P 〈 0.01]. These AML supernatantoinduced DCs could also induce allogeneic CD4^＋ T cells to differentiate into CD4^＋CD25high T cells, which had immunophenotyping characteristics of regulatory T cells, i.e. they expressed Foxp3 but not active maker CD69. Conclusion： This study demonstrates that soluble factors secreted by AML cells can inhibit development and functions of DCs. In addition, AML supernatant-induced DCs can induce the generation of CD4^＋CD25^high T cells from CD4^＋ T cells, which may be a mechanism of increased prevalence of CD4^＋CD25^high regulatory T cells and immune dysfunction in AML patients.
