Role of ascites adenosine deaminase in differentiating between tuberculous peritonitis and peritoneal carcinomatosis

AIM： To investigate the usefulness of tumor markers and adenosine deaminase in differentiating between tuberculous peritonitis （TBP） and peritoneal carcinoma- tosis （PC）. METHODS： A retrospective analysis of data was per- formed on consecutive patients who underwent perito- neoscopic and abdominal computed tomography （CT） evaluations. Among 75 patients at the Seoul National University Hospital from January 2000 to June 2010 who underwent both tests, 27 patients （36.0%） and 25 patients （33.3%） were diagnosed with TBP and PC, re- spectively. Diagnosis was confirmed by peritoneoscopic biopsy. RESULTS： Serum c-reactive protein （7.88：1：6.62 mg/ dL vs 3.12 ＋ 2.69 mg/dL, P = 0.01）, ascites adenos- ine deaminase （66.76：1：32.09 IU/L vs 13.89 ：l： 8.95 IU/L, P 〈 0.01）, ascites lymphocyte proportion （67.77 ：1： 23.41% vs 48.36 ＋ 18.78%, P 〈 0.01）, and serum- ascites albumin gradient （0.72 ＋ 0.49 g/dL vs 1.05 ＋ 0.50 g/dL, P = 0.03） were significantly different be- tween the two groups. Among tumor markers, serum and ascites carcinoembryonic antigen, serum carbohy- drate antigen 19-9 showed significant difference be- tween two groups. Abdominal CT examinations showed that smooth involvement of the parietal peritoneum was more common in the TBP group （77.8% vs 40.7%） whereas nodular involvement was more common in the PC group （14.8% vs 40.7%, P = 0.04）. From receiver operating characteristic （ROC） curves ascites adeno- sines deaminase （ADA） showed better discriminative capability than tumor markers. An ADA cut-off level of 21 IU/L was found to yield the best results of differ- ential diagnosis; sensitivity, specificity, positive predic- tive value, and negative predictive value were 92.0%, 85.0%, 88.5% and 89.5%, respectively. CONCLUSION： Besides clinical and radiologic findings, ascitic fluid ADA measurement is helpful in the differen- tial diagnosis of TBP and PC.

White matter changes in 80 mild cognitive impairment patients using magnetic resonance imaging

BACKGROUND： Many studies have suggested that one possible etiology of mild cognitive impairment is small vessel cerebrovascular disease, which is associated with small subcortical infarcts and white matter abnormalities. These white matter changes have been detected as white matter hyperintensity （WMH） using magnetic resonance imaging. WMH may be associated with frontal lobe dysfunction. OBJECTIVE： To examine white matter changes in mild cognitive impairment patients of different subtypes, and to evaluate the correlation between white matter changes and neuropsychological characteristics, demographic information, vascular risk factors, and mild cognitive impairment subtypes. DESIGN, TIME AND SETTING： The neurophysiological, comparison study was performed at the Department of Neurology Memory Clinic, Ulsan University Hospital, South Korea, between March 2007 and March 2008. PARTICIPANTS： Out of a total of 83 subjects with clinically diagnosed mild cognitive impairment at the out-patient clinic, 3 subjects with severe WMH were excluded. A total of 80 subjects were included in this study. No patients suffered from cognitive impairment induced by neurological diseases, mental disorders, or somatic diseases. In accordance with magnetic resonance imaging results, the patients were assigned to two subtypes： 56 subjects without WMH and 24 subjects with WMH. METHODS： All patients were subjected to a standard neuropsychological battery using the Korean version of the Mini-Mental State Examination, Clinical Dementia Rating, and comprehensive Seoul Neuropsychological Screening Battery. The Clinical Dementia Rating reflected general cognitive function of patients. Results from the Seoul Neuropsychological Screening Battery reflected attention, language function, visuospatial function, verbal memory, nonverbal memory, long-term memory, and frontal/executive function. Silicon is an essential alloying element in quenching and partitioning(Q&P)steels,because it is known to suppress carbide precipitation during partitioning step and promote carbon partitioning to stabilize *** 2 wt%Si was added to 10Mn-2Al-0.2C steel,the size and fraction of the carbides formed during partitioning became smaller than in the Si-free ***,the suppression of carbide formation promoted C partitioning into austenite as ***,austenite stability was always lower with Si under the equivalent partitioning condition because Si effectively decreased the stacking fault energy of *** partitioning progressed,the both yield and tensile strengths of the Si-added steel exceeded that of the Si-free steel with the similar ductility *** was because Si was an effective solid solution strengthener,and the austenite in the Si-added steel exhibited the appropriate stability to gradually transform into martensite throughout the *** resulting strengthening effect compensated for the softening caused by martensite ***,strain hardening rate decreased continuously throughout deformation,which resulted in high tensile strength and ductility.

What is the appropriate strategy for diagnosing NAFLD using ultrasonography in obese children?

Background:The aim of this study is to identify obese children who are candidates for a potential diagnosis of non-alcoholic fatty liver disease (NAFLD).Methods:We enrolled 242 obese children (122 boys and 120 girls) aged 7-16 years who were examined with abdominal ultrasonography in our pediatric obesity *** compared patients in the normal group with those in the NAFLD group (mild disease,moderate to severe disease) and identified the optimal anthropometric parameters among height,weight,body mass index (BMI),waist circumference,hip circumference,waist to height ratio (WHtR),and waist to hip ratio to predict NAFLD using a receiver operating characteristic curve *** also investigated risk factors associated with NAFLD for the anthropometric parameters and the biochemical model using logistic ***:The high-and low-risk groups for hepatic steatosis relative to a WHtR of 0.56 as the standard point showed significant differences in hepatic steatosis severity grade (P0.56 (2.25,1.05-4.81),and waist circumference >90th percentile (20.22,9.21-44.36) in the anthropometric parameter model and elevated alanine aminotransferase levels (boys >25.8 U/L,girls >22.1 U/L) (6.93,2.52-19.03),hypertriglyceridemia (>110 mg/dL) (3.80,1.23-11.75),and triglyceride to HDL cholesterol ratio >3 (9.23,2.95-8.83) in the biochemical parameter ***:A diagnostic approach to hepatic steatosis is recommended as part of the proper screening and stratification of risk factors in obese *** is a simple and convenient method of effectively identifying obese children who are candidates for hep

Improvement of uniformity in cultivation environment and crop growth rate by hybrid control of air flow devices

A complete control type plant factory has high efficiency in terms of cultivation area by constructing vertical multiple layered cultivation ***,it has a problem of irregular crop growth due to temperature deviation at upper and lower beds and increases in energy consumption by a prolonged cultivation *** this work,air flow rate inside a facility was improved by a hybrid control of air flow devices like air conditioning and air circulation fan with an established wireless sensor network to minimize temperature deviations between upper and lower beds and to promote crop *** performance of proposed system was verified with an experimental environment or Case A wherein air conditioning device was operated without a control algorithm and Case B wherein air conditioning and circulation fans were alternatively operated based on the hybrid control *** planting leafy vegetables under each experimental condition,crops were cultivated for 21 *** a result,Case B wherein AC(air conditioning) and ACF(air-circulation fan) were alternatively operated based on the hybrid control algorithm showed that fresh mass,number of leaves,and leaf length for the crops grown were increased by 40.6%,41.1%,and 11.1%,respectively,compared to Case A.

Development of 3D bicontinuous metal-intermetallic composites through subsequent alloying process after liquid metal dealloying

This study presents a novel process for the fabrication of metal-intermetallic composites with a 3D bicontinuous structure, achieved through a combination of liquid metal dealloying(LMD) and subsequent alloying. Initially, porous Ti structures are produced using the LMD process, followed by immersion in a molten Mg-3Al(wt%) metal. Due to the higher thermodynamic miscibility of Al with Ti compared to Mg, the concentration of Al in the Ti matrix increases as the immersion time increases. This results in a sequential phase transition within the Ti matrix: α-Ti → Ti_(3)Al → Ti Al. The phase transition considerably affects the hardness and strength of the composite material,with the Mg-Ti_(3)Al-Ti Al composite exhibiting a maximum hardness nearly twice as high as that of the conventional Mg-Ti composite. This innovative process holds potential for the development of various bicontinuous metal-intermetallic composites.

Decomposition of Nitrogen Trifluoride Using Low Power Arc Plasma

The low power arc plasma is characterized by extremely high enthalpy and temper- ature and it is easy to generate and control, and thus thermal decomposition process based on the plasma torch is receiving a great attention for decomposing non-degradable greenhouse gases. In order to elevate the economic feasibility, the effects of input power, waste gas flow rate and additive gases on the destruction and removal efficiency （DRE） of NF3 are examined. Specific energy density （SED） deceases as the flow rate increases, and accordingly, the DRE is reduced. The DRE is basically determined by the specific energy density. The highest DRE of NF3 was 97% for the waste gas flow rate of 100 L/min at a low input power level of 2 kW with the help of hydrogen additional gas. The inlet and outlet concentration of NF3 was analyzed using Fourier transform infrared spectroscopy （FT-IR） for DRE of NF3 evaluation. As a result, large amount of NF3 can be efficiently decomposed by low power arc plasma systems.

Optimal design of auxiliary-teeth to solve circulation current reduction of permanent magnet linear synchronous motor

A discontinuity of magnetic circuits according to the end effect is generated in the permanent magnet linear synchronous motor （PMLSM）. Due to the unbalanced back electro-motive force （EMF） and impedance produced, unbalanced current is generated. The cireulatin8 current, which is caused by a decrease in the thrust, is generated by the unbalanced current. The optimal design of auxiliary-teeth at the end of the mover was carried out to solve the unbalance of phase by using design of experiment （DOE）, and compared with the basic model through finite element analysis （FEA）. As a result, the auxiliary-teeth model compensates for the decrease of thrust caused by the unbalanced phase. Also, this model is proven to reduce the detent force by the vibration and noise of the PMLSM and copper loss caused by the circulating current.
