The experience of caring for a dying client with intellectual disabilities. A qualitative study with direct-caregivers

Background: Caregivers experience problems when caring for people with intellectual disabilities who are terminally ill. Aim: The aim of this study is to gain a deeper understanding of the experience of the caregiver when caring for a person with intellectual disabilities who is terminally ill and what influences this experience. Design: A qualitative research was carried out following the principles of Grounded Theory (GT) method, through fifteen interviews with caregivers in a living facility with continuous care. Results: The results show how caregivers strive to create meaningful moments for their client during the last period of his life. Since very few of the clients can clearly express their wishes and feelings, continuous observation is required of the caregivers for the interpretation of small signals of their clients. In that way, the appropriateness of their actions can be made certain. The results show that because of their involvement, caregivers are able to recognize clients’ signals. When a client is dying, the special atmosphere created on the ward contributes to coping with the situation and a positive experience. Conclusion: Direct-caregivers caring for a client with an intellectual disability who is terminally ill, experience an intense period of insecurity and also a special period meaningful when caring a dying client with intellectual disabilities. Their relationship with the client makes them to be more able to strive for client’s comfort. The results also show that some factors such as enough staff, clear and open communication with the physician and support of the manager contribute to the quality of palliative care on the ward.

Early Postoperative Anticoagulation by Enoxaparin after Mechanical Aortic Valve Replacement

Background: The use of low molecular weight heparin for early anticoagulation after mechanical aortic valve replacement is still a matter of debate even more that the early postoperative phase is associated with maximum of thrombo-embolic and bleeding risks. The objective of this study is to verify the efficacy and the safety of low molecular weight heparin for the early anticoagulation after mechanical aortic valve replacement. Methods and Results: It is a prospective study conducted over 6 months and interested 40 consecutive patients (32 male and 8 female) with a mean age 53.83 ± 16.93 years (19-75 years) who underwent a mechanical aortic valve replacement and received enoxaparin as bridging therapy between continuous unfractionated heparin and fully effective vitamin K antagonist therapy. There was no in-hospital death and no in-hospital thromboembolic events. We report 2 major bleeding events (5%). Conclusion: The use of low molecular weight heparin should be an alternative to explore for early anticoagulation after valve heart surgery and the results of our study must be verified by large randomized studies before drawing any hasty conclusions.

Modelling Freshwater Availability Using SWAT Model at a Catchment-Scale in Ivory Coast

Sustainable management of Ivory Coast’s freshwater reserve at a catchment scale is an essential way in the policies of land use sustainable management. Thus, the implementation of physical conceptual semi-distributed SWAT model required a good knowledge of the watershed and a large number of physic-chemical data available that have been prior adapted to Ivory Coast’s climatic and soil conditions. The whole simulation span was divided into calibration set (1982-1986) and validation set (1987-1990). The SUFI-2 algorithm was used for parameters optimization. The sensitivity analysis focused on 8 parameters related to runoff, soil, evaporation, main channel and groundwater. The performance criteria were based on the P-factor, R-factor and the two objective functions which are Nash-Sutcliffe coefficient and the coefficient of determination. Although Taabo river basin like African basins suffers from a significant lack of data, the objective functions showed the robustness of the model to climate variability. The calibration launched during a wet period gave objective functions higher than 0.7 while validation performed in less humid period gave performance criteria around 0.6. During the simulation period, Taabo river basin daily green water ranged from 0.044 to 50.257 mm/day with a total average of 3,090.9 mm per year. As for blue water, it is ranged from 0.032 to 0.552 mm/day with an annual total average of 29.19 mm.

Hydrocarbon Potential of Two Coastal Basins (Cameroon)

The problem related to the occurrence of oil accumulations in a sedimentary basin requires knowledge of the different geological structures present in this basin. The aim of this article is to show that the geological structure of sedimentary basins has an impact on the generation of oil accumulations. The case of Cameroon’s coastal basins has been studied: the Douala/Kribi-Campo basin (DKC) and the Rio Del Rey basin (RDR), which are producing basins in Cameroon. The work carried out has enabled to classify the DKC and RDR basins as passive margin basins. The lithology and geological structures present in the Douala/Kribi-Campo basin suggest the existence of source rocks (RM-1, RM-2, RM-3, RM-4 and RM-5), seal rocks (Mundeck clay, Logbabaclay...), stratigraphic, structural and mixed traps;the best oil potential is identified in its eastern part. On the other hand, the sandy levels are abundant, clean, and thick with a great porosity, which makes them excellent hydrocarbon reservoirs. In the Rio Del Rey basin, the lithology and geological structures present suggest the occurrence of source rocks (Akata clay and Agbada base clay), seal rocks (Akata clay) and multilayered reservoir rocks sandy or silty Agbada Formation and the freshwater sands of the Benin Formation. Unlike the Douala/Kribi-Campo basin, the best oil potential in the Rio Del Rey Basin lies in the center, in the so-called “deltaic alternation” formations dated from the late Miocene to the Pliocene.

随着社会经济和建设技术的发展,新的建筑结构形式不断涌现。工业厂房结构体系采用混凝土柱与折线形钢梁轻型房屋结构的形式越来越普遍,该结构形式能充分发挥钢筋混凝土柱刚度大、抗压强度高、防火防腐性能好的特点和折线形钢梁抗弯能力好、自重轻的优势,并且采用钢筋混凝土柱时,可以配以轻质砌体墙,能够解决房屋保温和墙体耐久性的问题;上部采用轻质高强、便于加工和安装的钢梁及轻型屋面,可提高施工速度,降低施工成本。但是,现行的建筑结构规范对这种新结构体系的受力特点、设计及应用等没有明确规定,给设计和施工造成困难。因此,正确理解与分析钢筋混凝土柱与折线形钢梁结构体系的力学性能是我国建筑结构领域中需要解决的重要问题。钢筋混凝土柱与折线形钢梁结构体系的研究符合国家“低碳、节能、环保”的理念,对中国绿色建筑工业化的发展具有十分重要的现实意义和战略性意义。折线形钢梁的安全可靠是保证这种新的结构体系正常工作的必要条件,它是结构体系的核心,深入研究折线形钢梁结构体系的受力性能是合理设计与应用应此类构件的前提。虽国内外学者对混凝土柱与钢梁混合结构进行了一些研究,但目前关于折线形钢梁结构体系的研究甚少。钢梁截面正应力分析计算是钢梁设计与计算中必要的计算过程,本文在张治新、黄军超于2015年国内外首次做的5种不同坡度折线形钢梁的试验结果基础上,对折线形钢梁在竖向荷载作用下的正应力状态进行试验,并结合ANSYS有限元模拟分析,对两端铰接折线形钢梁在竖向荷载作用下的正应力状态进行了进一步验证,具体研究分析结果如下:1、通过在集中荷载作用下折线形钢梁试验过程中的正截面应变状态,分析得出在集中荷载作用下折线形钢梁的跨中截面应变状态为接近线性变化关系、且基本符合平截面假定,在折线型钢梁跨中截面的中和轴位置区别于无曲折直线形钢梁,位于截面中心线以下。2、基于现有的折线形钢梁水平推力的研究成果,从理论角度进行竖向荷载作用下的折线形钢梁截面应力状态分析;通过折线形钢梁三个不同部位的截面应力分析得出,应力分布状态跟试验分析结果较吻合,其最大正应力产生位置不一定是跨中位置,在具体工程设计计算时要引起重视。3、以有限元软件ANSYS Workbench为研究平台建立分析模型,进行了仿真模拟分析,通过分析试验时的应变状态和理论分析的应变状态结果,进一步验证实际受力状态下的折线形钢梁截面正应力计算方法,为折线形钢梁计算与设计提供借鉴。

Long Term Evaluation of Green Vegetation Cover Dynamic in the Atacora Mountain Chain(Togo) and its Relation to Carbon Sequestration in West Africa

The research was done in the Atacora Mountain chain in Togo which tended to assess the change of vegetation cover during a 24-year *** also aims to evaluate the dynamic of the net primary productivity(NPP) of the living plants over the same *** Landsat imagery covering three different periods(1987, 2000, and 2011) was pre-processed to correct atmospheric and radiometric parameters as well as gapfilling the 2011 SCL-off ***, the vegetation indices such as NDVI(normalized difference vegetation index), SR(simple ratiovegetation index), SAVI(soil-adjusted vegetation index), and CASA(carnegie- ames- stanford approach)model for NPP were applied on these images after masking the study *** results showed a quiet decrease in the vegetation *** vegetation loss was more significant from 2000 to 2011 than from1987 to 2000, and anthropogenic activities can be deemed as the main cause of the vegetation *** biomass assessment by NPP computation also showed a decrease over the *** to the change of the vegetation cover, the ecosystem net productivity was very low in 2011 compared to 2000 and *** seems that the general health condition of thevegetation, including its potentiality in carbon sinking,was negatively affected in this area, which has already been under threatened.A perpetual monitoring of these ecosystems by means of efficient techniques could enhance the sustainable management tools of in the framework of reducing emissions from deforestation and forest degradation(REDD).

Evaluation of Agricultural Practices Scenarios for Reducing Erosion in Buyo Lake Catchment (Sassandra;Cote d’Ivoire) by Use of GIS

This study has used RUSLE model factors within a GIS component to evaluate eroded soil and developed viable agricultural practices scenarios to reduce soil loss in Buyo Lake catchment in the Western region of Côte d’Ivoire. Consequently, five study cases were tested as following: scenario 1—association of dense forest and crops with a high percentage of mulch;scenario 2—corn and sorghum combination by considering a high yield without conventional tillage;scenario 3—establishment of an herbaceous meadow;and scenario 4—association of palm, coffee, and cocoa with green cover. Scenario P was the setting up of straw mulch after corn or sorghum harvest. The erosion map resulting from these study cases showed by estimation, an average of soil loss of about 95 t/ha/year. The respective values obtained for the scenarios 1, 2 and 3 are 28 t/ha/year, 66 t/ha/year and 30 t/ha/year. This indicates a decrease of 68%. By scenario 4, the estimated average was 2 t/ha/year corresponding to a decrease of 98%. The combination of scenario 2 with scenario P gave soil losses average of 5 t/ha/year, i.e. a 95% reduction. This study shows that the association of palm, coffee, and cocoa with green cover would be very effective in reducing soil loss.

With the increase of gas consumption and the expansion of the associated distribution network, Gaz de France set up a research program to develop a methodology and a library of models to study the stability of any type of pressure regulator. The pressure level is controlled by pressure regulating stations. The objective of this study is to point out the working conditions that lead to instabilities. Some experiments and numerical simulations have been carried out to identify the relative influence of several parameters on the amplitude of oscillations. It turned out from measurements and simulations that the amplitudes of the downstream pressure are especially sensitive to the upstream pressure and to the size of the downstream volume.

Synthesis, Structural Characterization and DFT Studies of Silver(I) Complex Salt of Bis(4,5-dihydro-1<i>H</i>-benzo[g]indazole)

A new silver complex salt [Ag(N2C11H10)2]NO3 (where N2C11H10 = 4,5-dihydro-1H-benzo[g]indazole), has been synthesized and characterized by elemental and thermal analyses, IR and 1HNMR spectroscopies, single crystal X-ray structure determination and DFT studies. Its molecular structure comprises of a silver center coordinated to two nitrogen atoms from two 4,5-dihydro-1H-benzo[g]indazole molecule giving rise to a cationic complex entity, [Ag(N2C11H10)2]+ with as counter ion. The bulk structure is consolidated by N–H…O, C–H…π, Ag…π and Ag…O intermolecular interactions, thus generating a pseudo-helical network. The optimized structure, frontier molecular orbitals (HOMO and LUMO) and global reactivity descriptors were investigated by performing DFT calculations.
