Metabolic syndrome,levels of androgens,and changes of erectile dysfunction and quality of life impairment 1 year after radical prostatectomy

Robust data evaluating the association of preoperative parameters of the patients with quality of life after radical prostatectomy are *** investigated whether clinical and biological preoperative characteristics of the patients were associated with impaired patient-reported quality of life(QoL)and sexual outcomes 1 year after radical *** evaluated patient-reported outcomes among the 1343 men participating in the AndroCan trial(NCT02235142).QoL and erectile dysfunction(ED)were assessed before and 1 year after radical prostatectomy using validated self-assessment questionnaires(Aging Male's Symptoms[AMS]and the 5-item abridged version of the International Index of Erectile Function[IIEF5]).At baseline,1194 patients(88.9%)accepted to participate.A total of 750(55.8%)patients answered the 1-year postoperative *** of them,only 378(50.4%of responders)provided answers that could be used for *** year after prostatectomy,ED had worsened by 8.0(95%confidence interval[CI]:7.3–8.7;P<0.0001)out of a maximum of *** global AMS score has worsened by 2.8(95%CI:1.7–3.8;P<0.0001).ED scores 1 year postsurgery were positively correlated with preoperative age and percentage of fat mass,and negatively correlated with total cholesterol,dehydroepiandrosterone(DHEA),and androstenediol(D5);AMS were poorly correlated with preoperative *** and sexual symptoms significantly worsened after radical *** bioavailable testosterone levels were significantly correlated with smaller changes on AMS somatic subscores *** findings may be used to inform patients with newly diagnosed prostate cancer.

This study aims to investigate whether clinical and biological preoperative characteristics of patients who were to undergo radical prostatectomy were associated with impairment in patient-reported quality of life(QoL)and erectile dysfunction immediately before *** evaluated patient-reported outcomes among 1019 patients(out of 1343)of the AndroCan study,willing to score the Aging Male Symptom(AMS)and the International Index of Erectile Function 5-item(IIEF-5)*** linear regression and robust multiple regression were used to ascertain the relationship between demographic,clinical,and hormonal parameters and global AMS or IIEF-5 *** a result,most patients(85.1%)of the Androcan cohort agreed to complete *** higher IIEF-5 global scores were found in non-Caucasian and obese patients,with larger waist circumference,metabolic syndrome,diabetes mellitus,cardiovascular disease,hypertension,high blood sugar,concomitant medications,and hypogonadism,while the AMS global score was significantly higher in patients with larger waist circumference,metabolic syndrome,high blood pressure,raised glycemia,and concomitant *** IIEF-5 global score was correlated to age,dehydroepiandrosterone(DHEA),fat mass percentage,and androstenediol(D5).The AMS global score was significantly correlated to DHEA,D5,and DHEA ***,the multivariate models showed that QoL and erectile function were significantly affected,before surgery,by symptoms and signs that are usually considered as pertaining to the metabolic syndrome,while sexual hormones are essentially correlated to erectile dysfunction.

A substantial body of research has been conducted with the objective of enhancing the understanding of Non-Timber Forest Products (NTFP) in Central Africa. A significant proportion of the studies focus on medicinal plants, in contrast to those that examine food products. Nevertheless, studies on edible wild mushrooms as NTFPs and cultivated mushrooms, on their economic potential and value chains in the Great Lakes Region, remain scarce and largely absent for some NTFPs. In light of the aforementioned considerations, the present study aims to examine the economic potential and value chain of the production sector of wild edible mushrooms (WEM) and cultivated mushrooms (CM) within the Virunga National Park (PNVi) and its surrounding areas in North Kivu, Democratic Republic of the Congo (DRC). To this end, a sample of 432 respondents was selected, distributed equitably among the actors of the Goma-Kitshanga-Mweso value chain, the Goma-Rutshuru chain, the Beni-Mutwanga chain, and the Beni-Mangina chain. A digital questionnaire consisting of open and closed questions was administered, and observations were carried out concurrently. The quantities of mushrooms sold were weighed and photographed. The results demonstrated that, when the eight-month fruiting period of wild and cultivated mushrooms was considered as a single year for all actors within the value chain, a picker could sell 6.1 kg at $2 and earn $12. A seller could sell 16 kg at $4 and earn $64, while a grower could sell 502 kg at $3 and earn $1506. Consequently, the CM grower generates a significantly higher profit than other participants in the value chain, despite the expenses he incurs. Ultimately, the domestication of WEM, highly prized by the local population of the Virunga National Park, has the potential to enhance their household incomes, as evidenced by the findings of this study.

Modeling of 3-D Losses and Deviations in a Throughflow Analysis Tool

This contribution is dedicated to the modeling of the end-wall flows in a throughflow model for turbomachinery applications. The throughflow model is based on the Euler or Navier-Stokes equations solved by a Finite-Volume technique. Two approaches are presented for the end-wall modeling. The first one is based on an inviscid formulation with dedicated 3-D distributions of loss coefficient and deviation in the end-wall regions. The second approach is directly based on a viscous formulation with no-slip boundary condition along the annular end-walls and blade force modification in the region close to the end-walls. The throughflow results are compared to a series of 3-D Navier-Stokes calculations averaged in the circumferential direction. These 3-D calculations were performed on the three rotors of a low pressure axial compressor, for a series of tip clearance values.

Hepatocyte nuclear factor 4-alpha involvement in liver and intestinal inflammatory networks

Hepatocyte nuclear factor 4-alpha (HNF4-α) is a nuclear receptor regulating metabolism, cell junctions, differentiation and proliferation in liver and intestinal epithelial cells. Mutations within the HNF4A gene are associated with human diseases such as maturity-onset diabetes of the young. Recently, HNF4A has also been described as a susceptibility gene for ulcerative colitis in genome-wide association studies. In addition, specific HNF4A genetic variants have been identified in pediatric cohorts of Crohn’s disease. Results obtained from knockout mice supported that HNF4-α can protect the intestinal mucosae against inflammation. However, the exact molecular links behind HNF4-α and inflammatory bowel diseases remains elusive. In this review, we will summarize the current knowledge about the role of HNF4-α and its isoforms in inflammation. Specific nature of HNF4-α P1 and P2 classes of isoforms will be summarized. HNF4-α role as a hepatocyte mediator for cytokines relays during liver inflammation will be integrated based on documented examples of the literature. Conclusions that can be made from these earlier liver studies will serve as a basis to extrapolate correlations and divergences applicable to intestinal inflammation. Finally, potential functional roles for HNF4-α isoforms in protecting the intestinal mucosae from chronic and pathological inflammation will be presented.

Interendoscopist variability in proximal colon polyp detection is twice higher for serrated polyps than adenomas

AIM To assess the interendoscopist variability in the detection of colorectal polyps according to their location and histological *** This study was a retrospective analysis of prospectively collected data from a regional colorectal cancer(CRC) screening program; 2979 complete colonoscopies from 18 endoscopists were included. Variability in performance between endoscopists for detection of at least one adenoma(A), one proximal adenoma(PA), one distal adenoma(DA), and one proximal serrated polyp(PSP) was assessed by using multilevel logistic regression *** The observed detection rates among the 18 endoscopists ranged from 24.6% to 47.6%(mean = 35.7%) for A, from 19.1% to 39.0%(mean = 29.4%) for DA, from 6.0% to 22.9%(mean = 12.4%) for PA, and from 1.3% to 19.3%(mean = 6.9%) for *** adjusting for patient-level variables(sex, age), the interendoscopist detection rates variability achieved a significant level for A, PA, and PSP but not for DA(P = 0.03, P = 0.02, P = 0.02 and P = 0.08, respectively). Roadside vehicle particulate matter concentration estimation using artificial neural network model in Addis Ababa,Ethiopia
