目的儿童青少年重度抑郁症(MDD)是一种在全球范围内以高复发率、高致残率为特点的疾病,但其病理机制尚未得到充分阐明并且有效治疗方式有限。方法利用超高效液相色谱-四极杆飞行时间质谱对血浆进行了代谢组学分析,以寻找儿童青少年抑郁症的潜在生物标志物。我们共招募了134名儿童和青少年,包括52名首发DN-MDD患者(男性/女性:27/25)、32名DT-MDD患者(男性/女性:15/17)和50名HCs患者(男性/女性:27/23),分别采集其血浆样本进行代谢谱和生物信息学分析。结果发现了几种紊乱的通路与儿童青少年抑郁症密切相关,包括脂肪酸代谢(尤其是多不饱和脂肪酸代谢)和嘌呤代谢,共鉴定出35种代谢物(DN-MDD vs. HCs), 36种代谢物(DT-MDD vs. HCs), 6种代谢物(DT-MDD ***-MDD)组间差异具有统计学意义。为进一步探索识别儿童和青少年MDD潜在生物诊断标志物的可能性,我们进行了二元逻辑回归分析,证明肌苷是潜在的MDD独立诊断生物标志物。通过ROC分析,在区分未用药的MDD患者和健康对照组,其ROC曲线下面积达到0.999,并且区分药物治疗后的MDD患者和健康对照组的曲线下面积为0.866。同时,我们还发现了儿童青少年MDD病理生理学与成人MDD不同,后者主要是与色氨酸代谢相关。结论通过代谢组学分析,发现代谢紊乱与儿童青少年MDD的病理机制和诊断生物标志物之间存在紧密联系。
