The theory of microstrip antennas has motivated us to design a highly improved gain antenna under this category. It is a microstrip monopole antenna characterized by omni-directional radiation as well as a high radiation gain. A review of different methodologies to designing antennas with broad/ultra-wide band performance for various applications is enriched by our original antenna design. This is an original model analyzed over different substrate materials and finally optimized for the bandwidth of (3.3 - 5.8) GHz just below ?10 dB of return loss (RL). The antenna is judged for high gain when the ground plane size is reduced to nearly half that of substrate. The impact of the substrate materials is discussed in this article. The master design tool is Ansoft High Frequency Simulator Structure (HFSS), one of Finite Element Method (FEM) based software tools. The antenna would be printed on a 1.524 mm thick Rogers (RO3003C) substrate;overall size of 33.4 × 33.4 squared millimeters. At the optimal resonance frequency of 3.8 GHz, simulation results perfectly agree with the standards of UWB antennas, with a high radiation gain and impedance matching status.

Objective: To report the experience of surgical caravans for urogenital fistulas care. Methodology: This was a retrospective study covering the period from January 2014 to December 2014 and which took place on 7 sites of fistula care. The epidemiological, anatomoclinical, therapeutic and evolutionary aspects were studied. Results: 346 patients were operated during 14 caravans. Their average age was 33.11 years (range: 12 to 70 years). Most of these patients were without remunerative activities (80%) and without education (63.3%). The average duration of fistula progression was 6.08 years (range: 0 to 42 years). Obstetrical etiology was predominant (87.9%). According to the classification of Kees Waaldijk, fistulas were divided into type I (67.4%), type II (21.1%), and type III (11.5%). The most common surgical approach was the transvaginal route (82.1%). The vesicovaginal splitting with separated suture of the bladder and the vagina was the basic technique (94.7%). The therapeutic results were judged after a follow-up of 1 month and 3 months. Across the cohort, 80 patients (23.1%) were lost of sight for follow-up at 1 month and 245 (70%) at 3 months. The success rates evaluated in patients reviewed at 1 month and 3 months were respectively 70% and 64%. Conclusion: The incidence of urogenital fistulas is still high in Cote d'Ivoire. Various anatomoclinical varieties have been identified and treated with satisfactory results in poorly equipped local structures.

Epidemiological Aspects of Metastatic Relapse of Breast Cancer in an African Context

Objectives: This study aimed to describe the epidemiological characteristics of metastatic relapse in breast cancer at the University and Hospital Center of Treichville. Methodology: This is a cohort and retrospective study conducted from January 2000 to December 2015 on 178 patients with metastaticrelapse in breast cancer. Results: The prevalence of metastatic relapses was 49% and the patients had an average age of 53.4 years and among them, 91.6% received school education and 88.2% did not have a high socioeconomic level. The majority of initial tumors were in Stage III (47.8%), SBR III (47.2%), High Ki 67 (46.5%), Negative RH (79.3%), and HER2 negative (58%). Metastases were generally unique (62.9%), dominated by bone sites (36.6%), with an average occurrencedelay of 2.4 years. After the occurrence of metastases, the cumulative survival rate was 5% at 3 years, and nil at 5 years. Conclusion: Metastatic relapses were common with poor prognosis.

Perioperative Local Cooling Reduce Significantly Early Pain after Open Inguinal Hernia Repair:A Prospective Randomized Study

Pain and local complications are the major determinants of outcome after inguinal hernia repair. To evaluate the respective impact of peri-operative cooling of surgical site and usual care after open inguinal hernia repair, we performed a prospective randomized study. Methods: One hundred and eight consecutive patients with primary unilateral inguinal hernia were included the study. Repair was performed by local direct access during ambulatory surgery. The first study group underwent standard pre- and postoperative local care (control group). In the second group (cold compress group), a single-use disposable sterile cold compress was applied on the surgical site for at least 30 minutes before and 2 hours after surgery. Primary endpoints were immediate postoperative pain using a visual analogue scale, and local complications. Secondary endpoints included: analgesic drug consumption, length of hospital stay, delay to return to normal activity and patient satisfaction. Results: There was no difference concerning operative time (36.3 ± 14.0 vs 39.6 ± 7.2 minutes) and early (one-week) complications, although there was a non significant reduced incidence of hematoma and ecchymosis (0/54 versus 4/54) for the cold compress group. Analgesic drug consumption was significantly (p = 0.01) reduced. During the day of surgery and the first postoperative day, the visual analogue scale was significantly lower after cooling. There was a non-significant reduction in length of hospital stay (150 ± 37 versus 210 ± 47 min), and time to return to normal activity was shorter in the cold compress group. Conclusion: For open inguinal hernia repair, immediate pre- and post operative surgical site cooling, targeting a controlled temperature between 12?C and 15?C significantly reduced postoperative pain, analgesic drug consumption and resulted in improved immediate outcomes. This technique is safe, simple, easy to use, inexpensive and well tolerated by the patient.

Rwanda是一个内陆多山的国家,被称为千山之国,其主要经济基础是农业,其人口中90%以上是种植者。SINA-gerard ENTERPRISE是Rwanda最著名的企业之一,拥有13个永久贸易场所,在当地和国际上获得了许多奖励。本文基于Rwanda全国现有的交易站点距离为基础,对Sina-gerard企业配送中心选址进行优化,以减少其分支机构之间的总运输距离。研究目标是使得配送中心与其他分支机构的总距离最小,在确定交易场所间距离的基础上,采用P-中值法和简单LP法进行选址。所使用的基本数据通过问卷调查和Google地图收集获得,对各分支机构之间从一个站点到另一个站点的距离进行建模计算,并使用excel和lingo等工具得到可行的解决方案,研究发现,通过将配送中心设在Nyabugogo,距离缩短了240公里。设施位置分析与针对现有设施定选址一处或多处新设施以优化某些经济标准有关,建议在NYABUGOGO建立配送中心。企业应考虑配送中心的选址优化,以缩短整体配送距离。

成年动物中枢神经系统中的NG2细胞具有异质性。脊髓损伤(SCI)后,具有少突胶质前体细胞(OPCs)功能的NG2细胞亚型如何产生反应性变化及对其正常的发育过程有何影响目前还不清楚。本文采用少突胶质细胞系细胞表达EGFP的CNP-EGFP小鼠,研究正常和SCI后的NG2细胞的特征。在正常小鼠的白质中,对EGFP+NG2+双极细胞和EGFPnegNG2+多极细胞进行鉴别。SCI后,白质损伤边缘区中的EGFP+NG2+细胞3 d增殖达高峰,损伤中心区的EGFPnegNG2+细胞增殖在损伤后7 d达高峰。损伤后EGFP+NG2+细胞的Olig2、Sox10、Sox17等转录因子、基本的膜电生理参数以及钾电流表型均与发育过程增殖的OPCs一致。EGFPnegNG2+细胞不表达少突胶质发生中的转录因子。EGFP+CC1+少突胶质细胞6周时包含在EGFP+NG2+细胞增殖峰时表达BrdU的细胞。EGFPnegCC1+少突胶质细胞未被观察到。神经胶质生长因子2和成纤维细胞生长因子2处理促进少突胶质发生,提高EGFPnegNG2+细胞数量。因此,通过EGFP和转录因子表达,时空增殖类型及对生长因子的反应,两种类型的NG2+细胞对SCI刺激发生反应。SCI后EGFP+NG2+细胞经受细胞和生理变化特征,与在早期出生后的少突胶质发生过程中NG2+细胞中的变化类似。

为高速列车而设计的中等跨度多肋铁路桥的桥梁上,混凝土桥面是提供横向刚度的主要构件,但横梁上的作用力是未知的。由于横梁需要现场操作和焊接,所以增加了桥梁的成本。研究了一种横隔板的影响,这种横隔板应用在法国的一个典型的多跨度铁路桥梁上。第一个目的是为了提出一种合适的建模方法,可以被用于评估在这种交通荷载呈横向分布的结构上应用横隔板的作用和有效性。第二个目的是为了探究应用横隔板的必要性。为解决这些问题,建立了一个三维模型和非线性材料本构关系,分析了带有横向横隔板的和不带横隔板的桥面在不同的静力荷载条件下(自重,UIC荷载,TGV)的反应。考虑到桥面的动力特性,TGV采用移动荷载模拟。在动力分析中还考虑到了火车的极限速度。静力和动力分析结果表明:横隔板可能并不是必须采用的,除非桥梁受到意外的侧向力作用。因此,在这些桥梁上也许可以不需要安装横隔板。

This paper brings the comparison of performances of CO_(2)conversion by plasma and plasma-assisted catalysis based on the data collected from literature in this field,organised in an open access online *** tool is open to all users to carry out their own analyses,but also to contributors who wish to add their data to the database in order to improve the relevance of the comparisons made,and ultimately to improve the efficiency of CO_(2)conversion by *** creation of this database and database user interface is motivated by the fact that plasma-catalysis is a fast-growing field for all CO_(2)conversion processes,be it methanation,dry reforming of methane,methanolisation,or *** a result of this rapid increase,there is a need for a set of standard procedures to rigorously compare performances of different ***,this is currently not possible because the fundamental mechanisms of plasma-catalysis are still too poorly understood to define these standard *** however,the accumulated data within the CO_(2)plasma-catalysis community has become large enough to warrant so-called“big data”studies more familiar in the fields of medicine and the social *** enable comparisons between multiple data sets and make future research more effective,this work proposes the first database on CO_(2)conversion performances by plasma-catalysis open to the whole *** database has been initiated in the framework of a H_(2)0_(2)0 European project and is called the“PIONEER Data Base”.The database gathers a large amount of CO_(2)conversion performance data such as conversion rate,energy efficiency,and selectivity for numerous plasma sources coupled with or without a *** data set is associated with metadata describing the gas mixture,the plasma source,the nature of the catalyst,and the form of coupling with the *** the database itself,a data extraction tool with direct visualisation features or advance
