Effect of sedative agent selection on morbidity, mortality and length of stay in patients with increase in intracranial pressure

BACKGROUND: To identify the effects of sedative agent selection on morbidity, mortality, and length of stay in patients with suspected increase in intracranial pressure. Recent trends and developments have resulted in changes to medications that were previously utilized as pharmacological adjuncts in the sedation and intubation of patients with suspected increases in intracranial pressure. Medications that were previously considered contraindicated are now being used with increasing regularity without demonstrated safety and effectiveness. The primary objective of this study is to evaluate and compare the use of Ketamine as an induction agent for patients with increased intracranial pressure. The secondary objective was to evaluate and compare the use of Etomidate, Midazolam, and Ketamine in patients with increased intracranial pressure. METHODS: we conducted a retrospective chart review of patients transported to our facility with evidence of intracranial hypertension that were intubated before trauma center arrival. Patients were identifi ed during a 22-month period from January 2014 to October 2015. Goals were to evaluate the impact of sedative agent selection on morbidity, mortality, and length of ***: During the review 148 patients were identifi ed as meeting inclusion criteria, 52 were excluded due to incomplete data. Of those the patients primarily received; Etomidate, Ketamine, and Midazolam. Patients in the Ketamine group were found to have a lower mortality rate after injury stratifi cation. CONCLUSION: Patients with intracranial hypertension should not be excluded from receiving Ketamine during intubation out of concern for worsening outcomes.

Simulation of mode conversion at the magnetopause

Two-dimensional(2-D)and three-dimensional(3-D)hybrid simulations are carried out for mode conversion from fast mode compressional wave to kinetic Alfvn waves(KAws)at the inhomogeneous magnetopause *** cases in which the incident fast wave propagates in the xz plane,with the magnetopause normal along x and the background magnetic field pointing along z,the 2-D (xz)simulation shows that KAws with large wave number kxρi～1 are generated near the Alfve′n resonance surface,whereρi is the ion Larmor *** nonlinear wave properties are manifest in the mode conversion *** of the driver frequency are *** a result of nonlinear wave interaction,the mode conversion region and its spectral width are *** the 3-D simulation,after this first stage of the mode conversion to KAws with large kx,a subsequent generation of KAw modes of finite ky is observed in the later stage,through a nonlinear parametric decay *** the nonlinear cascade to ky can lead to massive transport at the magnetopause,the simulation results provide an effective transport mechanism at the plasma boundaries in space as well as laboratory plasmas.

An analytical study on the second-order resonance system of tide in an ideal partially-enclosed bay

In the numerical studies of a real tide M4 resonance system, the Xiangshan Port which is a partially-closed bay, Dong et al. [1999. Acta Oceanologica Sinica, 21 （3）： 1-6] found the interesting phenomenon that the advection plays an important role in inhibiting the growth of the amplitude of the tidal second-order resonance response （M4）. This result is contrary to the general traditional ideas for a non-resonance system. How this phenomenon is interpreted and what internal mechanism is behind the phenomenon are the main focuses of this study. The followings are examined：（1） the dynamic features of a second-order resonance system of tide; （2） the dominating factors on the second-order resonance responses; （3） the effects of both the friction and the advection on the second-order resonance responses; and （4） their roles in dominating the second-order resonance response and internal mechanisms by using the analytical methods. The respective results show that：（1） Both the bottom friction and the advection play significant roles in dominating the magnitude of the amplitude of the second-order resonance responses; （2） the effect of the friction on the second-order resonance response depends on the distribution ratio of the work-done of the system to friction force exhausted into between the damping of the first-order system and the inner excitation of the second-order system; （3） the advection plays a positive role in increasing the amplitude of the second-order non-resonance response in the second order non-resonance of tide; （4） in a second-order resonance system of tide, the effect of the advection may be either to increase or to decrease the amplitudes of the second-order resonance responses of tide, which depends on the distribution ratio mentioned above.

Geochemical Modelling of Fluoride Concentration in Hard Rock Terrain of Madhya Pradesh, India

The aim of the present study is to locate and decipher the groundwater quality, types, and hydrogeochemicai reactions, which are responsible for elevated concentration of fluoride in the Chhindwara district in Madhya Pradesh, India. Groundwater samples, quality data and other ancillary information were collected for 26 villages in the Chhindwara District, M.P. India during May 2006. The saturation index was computed for the selected samples in the region, which suggest that generally most of the minerals are saturated with respect to water. The concentration of fluoride in the region varies from 0.6 to 4.74 rag/l, which is much higher as per the national and international water quality standards. The study also reveals that the fluoride bearing rock formations are the main source of the higher concentration of fluoride in groundwater along with the conjuncture of land use change. Moreover, the area is a hard rock terrain and consists of fractured granites and amygdaloidal and highly jointed compact basalt acting as good aquifer, which is probably enriching the high content of fluoride in groundwater. High concentration of fluoride is found in deeper level of groundwater and it is possible due to rock-water interaction, which requires further detailed investigation. The highly alkaline conditions indicate fluorite dissolution, which works as a major process for higher concentration of fluoride in the study area. The results of this study will ultimately help in the identification of risk areas and taking measures to mitigate negative impacts related to fluoride pollution and toxicity.

with the exception of mature erythrocytes, cells within the human hematopoietic system are characterized by the cell surface expression of the pan-leukocyte receptor CD45. Here, we identify a novel subset among mononuclear cord blood cells depleted of lineage commitment markers (Lin) that are devoid of CD45 expression. Surprisingly, functional examination of Lin"CD45" cells also lacking cell surface CD34 revealed they were capable of multipotential hematopoietic progenitor capacity. Co-culture with mouse embryonic limb bud cells demonstrated that Lin-CD45-CD34- cells were capable of contributing to cartilage nodules and differentiating into human chondrocytes. BMP-4, a mesodermal factor known to promote chondrogenesis, significantly augmented Lin-CD45-CD34- differentiation into chondrocytes. Salicylic Acid Signaling Controls the Maturation and Localization of the Arabidopsis Defense Protein ACCELERATED CELL DEATH6

ACCELERATED CELL DEATH6 （ACD6） is a multipass membrane protein with an ankyrin domain that acts in a positive feedback loop with the defense signal salicylic acid （SA）. This study implemented biochemical approaches to infer changes in ACD6 complexes and localization. In addition to forming endoplasmic reticulum （ER）- and plasma membrane （PM）-Iocalized complexes, ACD6 forms soluble complexes, where it is bound to cytosolic HSP70, ubiquitinated, and degraded via the proteasome. Thus, ACD6 constitutively undergoes ER-associated degradation. During SA signaling, the soluble ACD6 pool decreases, whereas the PM pool increases. Similarly, ACD6-1, an activated version of ACD6 that induces SA, is present at low levels in the soluble fraction and high levels in the PM. However, ACD6 variants with amino acid substitutions in the ankyrin domain form aberrant, inactive complexes, are induced by a SA agonist, but show no PM localization. SA signaling also increases the PM pools of FLAGELLIN SENSING2 （FLS2） and BRI1-ASSOClATED RECEPTOR KINASE 1 （BAK1）. FLS2 forms complexes ACD6; both FLS2 and BAK1 require ACD6 for maximal accumulation at the PM in response to SA signaling. A plausible scenario is that SA increases the efficiency of productive folding and/or complex formation in the ER, such that ACD6, together with FLS2 and BAK1, reaches the cell surface to more effectively promote immune responses.

Phenology-based delineation of irrigated and rain-fed paddy fields with Sentinel-2 imagery in Google Earth Engine

Paddy rice agriculture is practiced in both rain-fed and irrigated ecosystems in the ***,small farms are prevalent in the region,and current satellite-based mapping techniques do not distinguish between the two ecosystems at farm *** study developed an approach to rapidly map irrigated and rain-fed paddy rice in Iloilo,Philippines at 10 m resolutions using Google Earth *** approach used an ensemble of classifiers based on time-series vegetation indices to produce dry and wet seasonal maps for the entire *** showed a predominance of rain-fed rice areas in both seasons,with irrigated rice making up only onefourth of the total rice *** overall accuracy was achieved at 68%for the dry season and 75%for the wet season based on ground-acquired points and very high-resolution *** two types of paddies were classified at accuracies up to 87%.Furthermore,the land cover maps showed a strong agreement with the municipal *** resultant maps complement current official statistics and demonstrate the prowess of phenology-based mapping to create paddy inventories in a timely manner to inform food security and agricultural policies.

Degradation of wetlands on the Qinghai-Tibet Plateau:A Comparison of the Effectiveness of Three Indicators

The Qinghai-Tibet Plateau encompasses a large quantity of wetlands, some of which have been degraded to varying severity levels. In the literature, a number of degradation indicators have been proposed to evaluate ecological health of wetlands, but their effectiveness in the plateau environment remains unknown. In this study, we assessed the effectiveness of three degradation indicators, soil moisture content at lo em deep, vegetative cover, and density of pika burrows. The degradation severity of wetlands in Maduo County on the Qinghai-Tibet Plateau is enumerated at four levels, intact, slight, moderate and severe. Analysis of xo6 samples collected in the field demonstrates that the density of pika burrows is the least reliable indicator. By comparison, vegetative cover and underlying soil moisture content are more reliable, even though neither is a perfect indicator as the difference among adjacent levels of severity as revealed by t-test is not always statistically significant. The imperfection of vegetative cover as an indicator is due to its variation among different types of wetlands. The limitation of moisture content is attributed to its non-linear relationship with wetland degradation. Above the threshold of about 50% in moisture content wetlands are unlikely to be degraded. It is recommended that moisture be measured at the point near the surface and vegetative cover be further differentiated by species in order to improve their effectiveness.
