Background The optimal management of ipsilateral extracranial internal carotid artery(ICA)stenosis during endovascular treatment(EVT)is *** compared the outcomes of two different strategies:EVT with vs without carotid artery stenting(CAS).methods In this observational study,we included patients who had an acute ischaemic stroke undergoing EVT and a concomitant ipsilateral extracranial ICA stenosis of≥50%or occlusion of presumed atherosclerotic origin,from the Dutch multicenter Randomized Clinical Trial of Endovascular Treatment for Acute Ischemic Stroke in the Netherlands(mR CLEAN)Registry(2014-2017).The primary endpoint was a good functional outcome at 90 days,defined as a modified Rankin Scale score≤*** endpoints were successful intracranial reperfusion,new clot in a different vascular territory,symptomatic intracranial haemorrhage,recurrent ischaemic stroke and any serious adverse *** Of the 433 included patients,169(39%)underwent EVT with *** 123/168(73%)patients,CAS was performed before intracranial *** 42/224(19%)patients who underwent EVT without CAS,a deferred carotid endarterectomy or CAS was *** with and without CAS were associated with similar proportions of good functional outcome(47%vs 42%,respectively;adjusted OR(aOR),0.90;95%CI,0.50 to 1.62).There were no major differences between the groups in any of the secondary endpoints,except for the increased odds of a new clot in a different vascular territory in the EVT with CAS group(aOR,2.96;95%CI,1.07 to 8.21).Conclusions Functional outcomes were comparable after EVT with and without *** during EVT might be a feasible option to treat the extracranial ICA stenosis but randomised studies are warranted to prove non-inferiority or superiority.

multiple sclerosis(mS)is an inflammatory neurodegenerative disease of the central nervous system(CNS).Pathological characteristics of the disease include activation of CNS-intrinsic immune cells,such as microglia and astrocytes,and loss of neuronal connections,myelin and blood-brain barrier(BBB)integrity as well as peripheral immune cell infiltration into the *** has long been considered a predominantly immunological disease,which has led to the development of essentially only immune-directed *** this traditional“outside-in”mS hypothesis,a dysregulation of the peripheral immune system causes immune cell infiltration into the CNS,leading to autoreactivity against myelin sheath components and secondary BBB ***,recent findings indicate that overactivation of microglia and astrocytes represents an important first step in mS pathology,as appears to be the case for other neurodegenerative diseases such as Alzheimer’s disease(AD)and Parkinson’s disease(PD).

Background: Evidence regarding the effectiveness of prenatal nutritional supplements has mainly considered anthropometric pregnancy outcomes. The effect on markers of health and disease， such as offspring telomere length (TL) and mitochondrial DNA content (mtDNAc) is unknown. Objectives: We assessed the efficacy of maternal multiple micronutrient (mmN)-fortified balanced-energy protein (BEP) and iron-folic acid (IFA) supplementation on newborn TL as a secondary outcome and mtDNAc as a non-declared outcome. Design: We conducted a randomized controlled trial in rural Burkina Faso， among pregnant females (15-40 years old) enrolled at mothers received either mmN-fortified BEP and IFA (intervention) or IFA only (control) throughout pregnancy. Whole arterial blood samples were collected from the umbilical cord of 104 control and 90 intervention group infants， respectively. Average relative TL and mtDNAc were measured using quantitative polymerase chain reaction. Linear regression models were fitted to assess TL and mtDNAc differences across trial arms. Results: We found that a combined daily mmN-fortified BEP supplement and IFA tablet did not affect newborn TL [β = -0.010 (95% CI: -0.057， 0.036); P = 0.662] or mtDNAc [β = 0.065 (95% CI: -0.203， 0.073); P = 0.354]， as compared to an IFA tablet alone. These findings were confirmed (P > 0.05) by adjusting the regression models for potential prognostic factors of study outcomes at enrollment. Exploratory analyses indicated higher， but non-significantly different mtDNAc among children born either small-for-gestational age， low birthweight， or preterm. Conclusions: Newborns from mothers who received daily nutritional supplements across gestation did not have different relative TL or mtDNAc.

Can root trait diversity explain complementarity effects in a grassland biodiversity experiment?

Apple(malus)and pear(Pyrus)are economically important fruit crops well known for their unique textures,f lavours,and nutritional *** genera are characterised by a distinct pattern of secondary metabolites,which directly affect not only resistance to certain diseases,but also have significant impacts on the f lavour and nutritional value of the *** identical chromosome numbers,similar genome size,and their recent divergence date,together with DNA markers have shown that apple and pear genomes are highly *** study utilized comparative genomic approaches,including simple sequence repeats,high resolution single nucleotide polymorphism melting analysis,and single nucleotide polymorphism chip analysis to identify genetic differences among hybrids of malus and Pyrus,and F2 *** research has demonstrated and validated that these three marker types,along with metabolomics analysis are very powerful tools to detect and confirm hybridity of progeny derived from crosses between apple and pear in both cross ***,this work analysed the genus-specific metabolite patterns and the resistance to fire blight(Erwinia amylovora)in *** findings of this work will enhance and accelerate the breeding of novel tree fruit crops that benefit producers and consumers,by enabling marker assisted selection of desired traits introgressed between pear and apple.
