Characterization and genetic manipulation of primed stem cells into a functional na?ve state with ESRRB

AIM to identify differences between primed mouse embryonic stem cells(ESCs) and fully functional naive ESCs; to manipulate primed cells into a naive state. MEtHODS We have cultured 3 lines of cells from different mouse strains that have been shown to be naive or primed as determined by generating germline-transmitting *** were put through a battery of tests to measure the different features. RNA from cells was analyzed using microarrays, to determine a priority list of the differentially expressed genes. these were later validated by quantificational real-time polymerase chain reaction. Viral cassettes were created to induce expression of differentially expressed genes in the primed cells through lentiviral transduction. Primed reprogrammed cells were subjected to in-vivo incorporation *** Most results show that both primed and naive cells have similar features(morphology, proliferation rates, stem cell genes expressed). However, there were some genes that were differentially expressed in the na?ve cells relative to the primed cells. Key upregulated genes in na?ve cells include ESRRB, ERAS, AtRX, RNF17, KlF-5, and MYC. After over-expressing some of these genes the primed cells were able to incorporate into embryos in-vivo, re-acquiring a feature previously absent in these cells. CONClUSION Although there are no notable phenotypic differences, there are key differences in gene expression between these na?ve and primed stem cells. these differences can be overcome through overexpression.

Each year approximately 360,000 people in the United States suffer a peripheral nerve injury （PNI）, which is a leading source of lifelong disability （Kelsey et al., 1997; Noble et al., 1998）. the most frequent cause of PNIs is motor vehicle accidents, while gunshot wounds, stabbings, and birth trauma are also common factors. Patients suffering from disabilities as a result of their PNIs are also burdensome to the healthcare system, with aver- age hospital stays of 28 days each year （Kelsey et al., 1997; Noble et al., 1998）.

AIM： to investigate if differences exist for patients＇ gastroesophageal reflux as measured by the Bravo ambulatory esophageal pH system between d 1 and d 2.MEtHODS： A retrospective study of 27 consecutive adult patients who underwent Bravo esophageal pH monitoring was performed. Patients underwent EGD under Ⅳ conscious sedation prior to Bravo placement. Acid reflux variables and symptom scores for d 1 were compared to d 2.RESUltS： the mean doses of fentanyl and midazolam were 90.4 μg and 7.2 mg, respectively. D 1 results were significantly more elevated than d 2 with respect to total time pH 〈 4, upright position reflux, and mean number of long refluxes. No statistical difference was noted between the two days for supine position reflux, number of refluxes, duration of longest reflux, episodes of heartburn, and symptom score.CONClUSION： Patients undergoing Bravo esophageal pH monitoring in association with EGD and moderate conscious sedation experience significantly more acid reflux on d i compared to d 2. the Ⅳ sedation may be responsible for the increased reflux on d 1. Performed this way, 48-h Bravo results may not be entirely representative of the patients＇ true GE reflux profile.

Using principal component analyses, this paper constructs two internationalization indices for the renminbi （RMB） and 32 other major currencies. We find that the RMB＇s currency internationalization degree index （CIDI） is still low, and far behind the 4 most important international currencies. In 2009, it was ranked 18th among all important international currencies. However, in terms of the currency internationalization prospect index （C1PI）, the RMB has remained the world ＂s fifih highest since 2006. Although it is still far behind the US dollar and the euro, surpassing the ranking of the yen and the pound is possible in the near future. the dramatic difference in the ranking between the CIDI and the CIPI is a result of China＇s tight capital account control, the usage continui01 of international currency due to network externalities, and the narrow foreign exchange and imperfect financial markets. Hence, to a large degree, the RMB ＇s potential as an international currency depends on China＇s capital account liberalization.

In the wake of the September 11,2001 terrorist attacks, there is a growing sense of insecurity felt by many citizens around the world. Sustainable security, with roots in the sustainable development and human security literature, seeks positive transformations for the co-evolving and mutually dependent human-environmental condition by integrating （and subsuming） national, human, environmental, and energy security concems and capitalizing on opportunities provided by human creativity, diplomatic openings, modernization and environmental change. the field of Sustainable Security Systems Engineering is proposed for protecting, restoring, designing, and implementing a set of integrated natural and man-made processes that equitably and responsibly meet the biophysical needs of human communities, while maintaining long-term security, respecting financial constraints, meeting ecological limits, and improving institutional arrangements for transparent and effective governance. Scenario planning is shown to help promote sustainable security by identifying the preconditions of instability and helping to proactively address them in an increasingly complex and uncertain world. the six papers published in this featured collection cross policy domains, geographic, political, and sectoral boundaries and were discussed at forums sponsored by the Systems Engineering and Global Policy group. Collectively, they demonstrate the quality, breadth and depth of systems engineering methodologies that are used to promote sustainable security.

Background the number of mobile stroke programmes has increased with evidence,showing they expedite intravenous *** questions include whether time savings extend to patients eligible for endovascular therapy and impact clinical *** Our mobile stroke unit(MSU),based at an academic medical centre in upstate New York,launched in October *** reviewed prospective observational data sets over 26 months to identify MSU and non-MSU emergency medical service(EMS)patients who underwent intravenous thrombolysis or endovascular thrombectomy for comparison of angiographic and clinical *** Over 568 days in service,the MSU was dispatched 1489 times(2.6/day)and transported 300 patients(20%of dispatches).Intravenous tissue plasminogen activator(tPA)was administered to 57 MSU patients and the average time from 911 call-to tPA was 42.5 min(±9.2),while EMS transported 73 patients who received tPA at 99.4 min(±35.7)(ptients(12%)received tPA from 3.5 hours to 4.5 hours since last known well and would likely have been outside the window with EMS *** thrombectomy was performed on 21 MSU patients with an average 911 call-to groin puncture time of 99.9 min(±18.1),while EMS transported 54 patients who underwent endovascular thrombectomy(Et)at 133.0 min(±37.0)(p=0.0002).there was no difference between MSU and traditional EMS in modified Rankin score at 90-day clinic follow-up for patients undergoing intravenous thrombolysis or endovascular thrombectomy,whether assessed as a dichotomous or ordinal *** Mobile stroke care expedited both intravenous thrombolysis and endovascular *** is an ongoing need to show improved functional outcomes with MSU care.

the past decade has brought substantial advances in the management of inflammatory bowel diseases(IBD). the introduction of tumor necrosis factor(tNF) antagonists, evidence for the value of combination therapy, the recog-nition of targeting lymphocyte trafficking and activation as a viable treatment, and the need for early treatment of high-risk patients are all fundamental concepts for current modern IBD treatment algorithms. In this article, authors review the existing data on approved biologicals and small molecules as well as provide insight on the current positioning of approved therapies. Patient stratification for the selection of specific therapies, therapeutic targets and patient monitoring will be discussed as well. the thera-peutic armamentarium for IBD is expanding as novel and more targeted therapies become available. In the absence of comparative trials, positioning these agents is becoming difficult. Emerging concepts for the future will include an emphasis on the development of algorithms which will facilitate a greater understanding of the positioning of novel biological drugs and small molecules in order to best tailor therapy to the patient. In the interim, anti-tNF therapy remains an important component of IBD therapy with the most real-life evidence and should be considered as first-line therapy in patients with complicated Crohn's disease and in acute-severe ulcerative colitis. the safety and efficacy of these ‘older' anti-tNF therapies can be optimized by adhering to therapeutic algorithms which combine clinical and objective markers of disease severityand response to therapy.

Incidences of head and neck squamous cell carcinoma(HNSCC)have been on the rise in the last few decades,with a significant risk factor being human papillomavirus(HPV)type-16/18 infection,particularly in the development of oropharyngeal ***(Rt)is an important treatment modality for HNSCC,where it promotes extensive cellular DNA damage leading to the therapeutic *** has been well-established that HPV-positive HNSCC display better response rates and improved survival following Rt compared to HPV-negative *** differential radiosensitivity has been largely associated with altered cellular DNA damage response mechanisms in HPV-positive HNSCC,and particularly with the signaling and repair of DNA double strand ***,other factors,particularly hypoxia present within the solid cancer,have a major impact on relative ***,recent approaches aimed at enhancing the radiosensitivity of HNSCC have largely centered on targeting key proteins involved in DNA repair,DNA damage checkpoint activation,and hypoxia *** studies have utilised in vitro and in vivo models of HPV-positive and HPV-negative HNSCC and examined the impact of specific inhibitors against the targets in combination with radiation in suppressing HNSCC cell growth and ***,accumulating evidence has shown that targeting enzymes including poly(ADP-ribose)polymerase,ataxia telangiectasia and Rad-3 related,DNA-dependent protein kinase catalytic subunit,and checkpoint kinase 1 can radiosensitise HNSCC cells which should be taken forward in further preclinical studies,with the goal of optimizing the future effective Rt treatment of HNSCC.
