Obj.ctives: To describe thoracic endovascular aortic repair (TEVAR) outcomes at the Royal Brisbane and Women’s Hospital between 2001-2010. Design: Prospective cohort study. Results: We successfully treated 95 of 97 patients for a 98% procedural success rate. Of the treated patients, 68 (72%) were males and mean age was 61 ± 17 years. Average follow up was 3.6 ± 2.0 years. Pathologies treated were: dissection (n = 35), aneurysmal disease (n = 32), traumatic disease (n = 19), coarctation (n = 5) and miscellaneous (n = 4). There was one peri-procedural myocardial infarction. Renal complications occurred in 7 patients (3 with doubling creatinine and 4 requiring temporary renal replacement therapy). Stroke occurred in 2 patients and paresis in 2 patients (permanent in one). Six patients died during index hospitalization and 17 deaths occurred during follow-up, 2 of which were confirmed secondary to aortic pathology. Age (HR 1.08 per year, p < 0.01) and ASA class (HR 2.2 per class, p = 0.02) were independently associated with mortality. There were 25 re-interventions in 22 patients. Eighteen of these re-interventions in 16 patients were related to the management of complications of TEVAR of which 13 were for endoleaks;eight type I, four type II, one type III. Conclusion: TEVAR can be used to treat thoracic aortic pathology but questions remain regarding long-term durability.

Modern societal pressures and expectations over the past several decades have resulted in the tendency for couples to delay conception. While women experience a notable decrease in oocyte production in their late thirties, the effect of age on spermatogenesis is less well described. While there are no known limits to the age at which men can father children, the effects of advanced paternal age are incompletely understood. This review summarizes the current state of knowledge regarding advanced paternal age and its implications on semen quality, reproductive success and offspring health. This review will serve as a guide to physicians in counseling men about the decision to delay paternity and the risks involved with conception later in life.

AIM: To conduct a retrospective study in 400 chronic hepatitis B patients in order to identify hepatitis B viral factors associated with complications of liver disease or development of hepatocellular ***: The mean follow-up time was 83.6 ± 39.6 mo. Alpha-fetoprotein test and abdominal ultrasound were used for cancer surveillance. Hepatitis B basal core promoter mutants, precore mutants, genotypes, hepatitis B viral DNA (HBV DNA) level and hepatitis B e antigen (HBeAg) were measured. Univariate analysis and logistic regression were used to assess odds ratios for viral factors related to liver deaths and hepatocellular carcinoma development. RESULTS: During follow-up, 38 patients had liver deaths not related to hepatocellular carcinoma. On multivariate analysis, older age [odds ratio: 95.74 (12.13-891.31); P < 0.0001], male sex [odds ratio: 7.61 (2.20-47.95); P = 0.006], and higher log10 HBV DNA [odds ratio: 4.69 (1.16-20.43); P < 0.0001] were independently predictive for these liver related deaths. Also, 31 patients developed hepatocellular carcinoma. Multivariate analysis showed that older age [odds ratio: 26.51 (2.36-381.47); P = 0.007], presence of precore mutants [odds ratio: 4.23 (1.53-19.58); P = 0.02] and presence of basal core promoter mutants [odds ratio: 2.93 (1.24-7.57); P = 0.02] were independent predictors for progression to hepatocellular ***: Our results show that high levels of baseline serum HBV DNA are associated with non- hepatocellular carcinoma-related deaths of liver failure, while genetic mutations in the basal core promoter and precore regions are predictive for development of hepatocellular carcinoma.

Multi-obj.ctive optimization of process parameters for ultra-narrow gap welding based on Universal Kriging and NSGA Ⅱ

The successful confinement of the arc by the flux band depends on the welding process parameters for achieving single-pass,multi-layer, and ultra-narrow gap welding. The sidewall fusion depth, the width of the heat-affected zone, and the line energy are utilized as comprehensive indications of the quality of the welded j.int. In order to achieve well fusion and reduce the heat input to the base *** welding process characteristics were chosen as the primary determinants, including welding voltage, welding speed, and wire feeding speed. The metamodel of the welding quality index was built by the orthogonal experiments. The metamodel and NSGA-Ⅱ(Non-dominated sorting genetic algorithm Ⅱ) were combined to develop a multi-obj.ctive optimization model of ultra-narrow gap welding process parameters. The results showed that the optimized welding process parameters can increase the sidewall fusion depth, reduce the width of the heataffected zone and the line energy, and to some extent improve the overall quality of the ultra-narrow gap welding process.

Purpose j.mp-landing assessments provide a means to quantify an individual’s ability to attenuate ground reaction forces,generate lower limb explosive power and maintain j.int *** order to identify risk factors that can be targeted through appropriate training interventions,it is necessary to establish which(scalar)obj.ctive kinetic,kinematic,and performance measures are most associated with lower-extremity *** Online searches of MEDLINE,SCOPUS,EBSCOHost,SPORTDiscus and PubMed databases were completed for all articles published before March 2020 in accordance with PRISMA *** 40 articles investigating nine j.mp-landing assessments were included in this *** 79%of studies using drop j.mp(n=14)observed an association with future inj.ry,while only 8%of countermovement j.mp studies(n=13)observed an association with inj.ry *** 57%of studies using unilateral assessments found associations with risk of inj.ry(n=14).Studies using performance measures(j.mp height/distance)as outcome measure were only associated with inj.ry risk in 30%of ***,those using kinetic and/or kinematic analyses(knee abduction moment,knee valgus angle,knee separation distance,peak ground reaction force)found associations with inj.ry in 89%of *** The landing element of j.mp-landing assessments appears to be superior for identifying individuals at greater risk of inj.ry;likely due to a closer representation of the inj.ry ***,j.mp-landing assessments that involve attenuation of impact forces such as the drop j.mp appear most suited for this purpose but should involve assessment of frontal plane knee motion and ground reaction forces.
