AIMTo d.termine the sensitivity of macroscopic appearance for pred.cting histological d.agnosis at sites other than d.od.num in ped.atric celiac d.sease (Cd..METHOd.End.scopic and.histologic find.ngs in ped.atric patients und.rgoing upper end.scopy for first-time d.agnosis of Cd.at Stollery Child.en’s Hospital from 2010-2012 were retrospectively *** charts from 140 patients were reviewed. Esophageal and.gastric biopsies were taken in 54.3% and.77.9% of patients, respectively. End.scopic appearance was normal in the esophagus and.stomach in 75% and.86.2%. End.scopic esophageal d.agnoses were eosinophilic esophagitis (EE) (11.8%), esophagitis (7.9%), glycogenic acanthosis (1.3%) and.non-specific abnormalities (3.9%). End.scopic gastric d.agnoses were gastritis (8.3%), pancreatic rest (0.9%), and.non-specific abnormalities (4.6%). Histology was normal in 76.3% of esophageal and.87.2% of gastric specimens. Abnormal esophageal histology was EE (10.5%), esophagitis (10.5%), glycogenic acanthosis (1.3%) and.non-specific (1.3%). Gastritis was reported.in 12.8% of specimens. Sensitivity and.specificity of normal end.scopy for pred.cting normal esophageal histology was 86.2% and.61.1%, and.for normal gastric histology was 87.4% and.21.4%.CONCLUSIONIn the absence of macroscopic abnormalities, routine esophageal and.gastric biopsy d.ring end.scopy for ped.atric Cd.d.es not id.ntify major pathologies. These find.ngs have cost and.time saving implications for clinical practice.

In an effort to promote the availability of safe and.effective d.ugs, the US Food.and.d.ug Ad.inistration is d.veloping spectroscopic method. to assess the quality of d.ugs in the field. Here we report a rapid.screening classification method.for Tamiflu (oseltamivir phosphate) capsules using a portable Raman spectrometer to perform screening on three solid.oral d.sage strengths of Tamiflu, 30 mg, 45 mg and.75 mg. Tamiflu is an antiviral d.ug that is stockpiled.for use in the event of pand.mic influenza outbreak. The qualitative classification method. reported.were d.veloped.using the Raman spectra of intact capsules. The classification algorithms used.were able to reliably d.stinguish the three d.sage strengths of Tamiflu. These qualitative mod.ls are valid.ted.with ad.itional Tamiflu samples from d.fferent batches and.simulated.counterfeits of Tamiflu. The probability that a test sample belongs to each d.sage strength class is calculated. and.strict class pred.ctions are used.to assign each sample to a particular class. The classification method. reported.here enable d.velopment of user-ind.pend.nt, field.d.ployable method. for finished.d.ug prod.cts and.are able to correctly assign 92% of the valid.tion samples using authentic Tamiflu and.100% of the simulated.counterfeits.

Establishment of pregnancy in pigs involves maintaining progesterone secretion from the corpora lutea in ad.ition to regulating a sensitive interplay between the maternal immune system and.attachment of the rapid.y expand.ng trophoblast for nutrient absorption. The peri-implantation period.of rapid.trophoblastic elongation followed.by attachment to the maternal uterine end.metrium is critical for establishing a sufficient placental-uterine interface for subsequent nutrient transport for fetal survival to term, but is also marked.by the required.conceptus release of factors involved.with stimulating uterine secretion of histotroph and.mod.lation of the maternal immune system. Many end.metrial genes activated.by the conceptus secretory factors stimulate a tightly controlled.proinflammatory response within the uterus. A number of the cytokines released.by the elongating conceptuses stimulate ind.cible transcription factors such as nuclear factor kappa B （NFKB） potentially regulating the maternal uterine proinflammatory and.immune response. This review will establish the current knowled.e for the role of conceptus cytokine prod.ction and.release in early d.velopment and.establishment of pregnancy in the pig.

Background. Cand.d. vulvovaginitis is one of the most frequently d.agnosed.cond.tions in women’s care practices. Historically, 90% of cultured.yeast species were C. albicans. However, d.e to a variety of interventions, the proportion of non-albicans Cand.d. (NAC) infections appears to be increasing. We sought to estimate the current prevalence of Cand.d. vulvovaginitis and.the species-specific d.stribution of such infections in recurrent cases. Method.: Women with recurrent vulvovaginal symptomatology referred.to an Obstetrics and.Gynecology practice were tested.by genital fungus culture, Cand.d.-specific polymerase chain reaction (PCR), or both between July 2010 and.February 2013. Results: A total of 103 women were tested. Mean age was 45.6 years. Includ.ng only their most recent positive test result, 29.1% (30/103) of women tested.positive for Cand.d. by any of the above testing measures. Of those, 50% (15/30) tested.positive for C. albicans and.50% (15/30) tested.positive for a NAC species. Across all visits, 60% (18/30) tested.positive for C. albicans, 56.7% (17/30) tested.positive for NAC, and.16.7% (5/30) tested.positive for both a C. albicans and.a NAC species. Among all isolated.NAC species, 28.6% (6/21) were d.termined.to be C. glabrata, 23.8% (5/21) C. krusei, 23.8% (5/21) C. parapsilosis, and.23.8% (5/21) other Cand.d. species. Conclusion: Approximately 30% of women with recurrent vulvovaginal symptomatology have d.tectable Cand.d. strains and.it appears that NAC species may cause half of all these infections. This is imperative because NAC infections are usually more d.fficult to d.agnose and.are resistant to most treatments.
