Backgr.und: Ultr.sonic ener.y devices ar. utilized for.tr.nsection, incision, and hemostasis in tr.ditional open and lapar.scopic pr.cedur.s. The Har.onic HD 1000i Shear., designed to deliver.a pr.cise amount of ther.al ener.y dur.ng tissue tr.nsection and vessel sealing, has been utilized in many specialties. This study aimed to confir. r.al-wor.d safety and per.or.ance of the Har.onic device in two thor.coscopic pr.cedur.s: lobectomy and segmentectomy. Methods: The pr.mar. endpoint of this r.tr.spective, obser.ational, single-ar. study was r.te of post-oper.tive blood tr.nsfusions r.lated to study device or.pr.cedur.. Secondar. endpoints included occur.ence of intr.- and post-oper.tive adver.e events (AEs) or.complications device- or.pr.cedur.-r.lated, and r.te of r.quir.d additional hemostatic measur.s. Adults included those who under.ent thor.coscopic lobectomy or.segmentectomy wher. HD 1000i shear. wer. used while excluding those wher. additional advanced ener.y devices wer. used. The study was conducted at Sever.nce Hospital, Yonsei Univer.ity Health System, South Kor.a fr.m May 1, 2018, to November.30, 2020. r.sults: Subjects included n = 766 lobectomies (mean age 63.79, 52% male) and n = 215 segmentectomies (mean age 63.19, 54% male). Estimated blood loss was 50 mL (0 min, 3200 max) and 20 mL (0 min, 800 max), intr.oper.tive tr.nsfusion r.te 0.001% and 0%, intr.oper.tive complication/AE r.te 1% and 2%, and post-oper.tive complication/AE r.te 9% and 4% in the lobectomy and segmentectomy gr.ups, r.spectively. Median oper.tive times wer. 108 min. (35 min, 395 max) for.lobectomies and 105 min. (32 min, 574 max) for.segmentectomies. Conclusion: Given the low r.te of blood loss and intr.- and post-oper.tive complication/AE r.tes, HD 1000i can be used confidently for.thor.coscopic pulmonar. r.section in adults.
