Usefulness of antero-posterior radiograph and variability of management in non-major thoracolumbar injuries: a single centre pilot study and review of literature

Background: Most adult trauma protocols suggest that where there has been a dangerous mechanism of injury or the patient exhibits abnormal physiology, CT scan is the primary radiological investigation. Other patients who may have suffered thoraco-lumbar (T-l) trauma initially have antero-posterior (AP) and lateral plain X-rays performed. Our clinical experience suggests AP views are not particularly useful in the management of these relatively low-velocity injuries. This is the first study intended to determine the contribution made by AP X-rays in these cases. Methods: Adults with a history of T-l trauma referred to our tertiary spinal service over 20 weeks were reviewed. Those with a CT scan performed prior to X-rays were excluded. Four spine surgeons and four neuroradiologists were independently shown lateral X-rays along with the clinical details and asked to provide a management plan. Then they were shown the AP X-rays and asked if they would like to change their advice. Results: Fifty-two patients were identified. Thirty-four sets of supine and 40 sets of erect X-rays were included (four people only had lateral X-rays performed), yielding 1152 film views. Average patient age was 58.3 years with 30 (58%) males. Forty-five (87%) were AO type A (compression-type) fractures. Seven (13%) had been erroneously referred with a diagnosis of acute fracture, which on review was not considered to be the case. Fifty-four percent of fractures were between T11 and l2. Forty-six percent appeared osteoporotic. In no instance did evaluation of the AP X-ray change the management plan which had been suggested following the evaluation of the lateral X-ray alone. However, there was significant variation in advice on further management between consultants. Conclusions: Our results suggest AP X-rays do not contribute to the management of low-velocity thoraco-lumbar traumas. larger studies are required to support these findings, but there appears to be a potential to reduce both cost

Exotic bush honeysuckles (Lonicera spp.) are becoming increasingly common in the eastern and mid-western United States, but little is known about their impacts on invertebrates. We used a modified leaf vacuum to sample invertebrates in the shrub strata and understory of three shrub types (and open plots in the understory): single Morrow’s honeysuckle (L. morrowii A. Gray) shrubs, single native southern arrowwood (Viburnum recognitum Fernald) shrubs, and dense thickets of Morrow’s honeysuckle, in southwestern PA, USA during 2004 and 2005. We also assessed the degree of lE_lINK26">lE_lINK25">herbivory on the two species of shrubs. Within the shrub strata, invertebrate biomass was lower in southern arrowwood shrubs, but there was no difference in invertebrate abundance or family richness. Invertebrate abundance and richness were lowest in August, but there was no difference in biomass among the months. Background: Coarctation of the aorta (CoA) is associated with a generalized arteriopathy and long-term complicationsdespite repair. Data on invasive exercise hemodynamics in this population are lacking. Accordingly, wereviewed adults with CoA undergoing exercise catheterization to assess 1. hemodynamic profile;2. feasibilityfor assessment of CoA severity. Methods: Twenty patients undergoing exercise cardiac catheterization (12 armadduction and 8 supine cycle ergometry) at a quaternary care center between 2004 and 2021 were identified. Restingand exercise hemodynamic data were abstracted from the procedure logs. Results: Mean age was 43.6 ±12.0 years. Eleven patients (55%) had resting pulmonary arterial wedge pressure (PAWP) >15 mmHg;amongthose undergoing arterial catheterization, left ventricular end-diastolic pressure was >15 mmHg in 63%. Elevenpatients (55%) had pulmonary hypertension: 7 (35%) combined and 4 (20%) isolated post-capillary. At peak exercise,ΔPAWP/Δcardiac output (CO) ≥2 and Δmean pulmonary artery pressure/ΔCO ≥3 mmHg/l/min were foundin 7 (78%) and 6 (67%) patients, respectively;the composite of exercise PAWP ≥25 mmHg or ΔPAWP/ΔCO>2 was seen in 12 (86%). CoA peak-to-peak gradients at baseline (n = 14) and during exercise (n = 9) were 12(3–16) and 16 mmHg (9–28), respectively. Only 2 patients had an increase in CoA gradient to >20 mmHg withexercise. Conclusions: Diastolic dysfunction and pulmonary hypertension were highly prevalent, with exerciseunmasking abnormal diastolic and pulmonary vascular reserve in some individuals. Most patients failed to showsignificant increases in CoA peak-to-peak gradients with exercise. Further studies are warranted to establish thebest diagnostic method for CoA severity assessment.

The design temperature of high plutonium concentration ZPPR fuel plates is 600°C. Cladding integrity of the 304 l stainless steel cladding is a significant concern with this fuel since even small holes can lead to substantial fuel degradation. Since the fuel has a higher coefficient of thermal expansion than the cladding, an investigation of the stress induced in the cladding due to the differential thermal expansion of fuel and cladding up to the design temperature was conducted. Small holes in the cladding envelope would be expected to lead to the fuel hydriding and oxidizing into a powder over a long period of time. This is the same type of chemical reaction chain that exists in the degradation of the high uranium concentration ZPPR fuel. Unfortunately, the uranium fuel was designed with vents which allowed this degradation to occur. The Pu cladding is sealed so only fuel with damaged cladding would be subject to this damage. The thermal stresses that can be developed in the fuel cladding have been calculated in this paper and compared to the ultimate tensile stress of the cladding. The conclusion is drawn that thermal stresses cannot induce holes in the cladding even for the highest storage temperatures predicted in calculations (292°C). In fact, thermal stress cannot cause cladding failure as long as the fuel temperatures are below the design limit of 600°C (1112°F).

Accounting for the Effects of Climate Variability in Regional Flood Frequency Estimates in Western Nigeria

Extreme flood events are becoming more frequent and intense in recent times, owing to climate change and other anthropogenic factors. Nigeria, the case-study for this research experiences recurrent flooding, with the most disastrous being the 2012 flood event that resulted in unprecedented damage to infrastructure, displacement of people, socio-economic disruption, and loss of lives. To mitigate and minimize the impact of such floods now and in the future, effective planning is required, underpinned by analytics based on reliable data and information. Such data are seldom available in many developing regions, owing to financial, technical, and organizational drawbacks that result in short-length and inadequate historical data that are prone to uncertainties if directly applied for flood frequency estimation. This study applies regional Flood Frequency Analysis (FFA) to curtail deficiencies in historical data, by agglomerating data from various sites with similar hydro-geomorphological characteristics and is governed by a similar probability distribution, differing only by an “index-flood”;as well as accounting for climate variability effect. Data from 17 gauging stations within the Ogun-Osun River Basin in Western Nigeria were analysed, resulting in the delineation of 3 sub-regions, of which 2 were homogeneous and 1 heterogeneous. The Generalized logistic distribution was fitted to the annual maximum flood series for the 2 homogeneous regions to estimate flood magnitudes and the probability of occurrence while accounting for climate variability. The influence of climate variability on flood estimates in the region was linked to the Madden-Julian Oscillation (MJO) climate indices and resulted in increased flood magnitude for regional and direct flood frequency estimates varying from 0% - 35% and demonstrate that multi-decadal changes in atmospheric conditions influence both small and large floods. Background: Interpretation of parathyroid hormone (PTH) measurements requires an understanding of its structural conformation. PTH reflects calcium intake and vitamin D status but is commonly measured in the fasted state using 2nd generation assays, known to cross-react with PTH 7-84 fragments, which is not observed with 3rd generation assays. The objective was to determine if plasma PTH in the fed and non-fed state differ while controlling for diurnal rhythm and the generation of PTH assay. Methods: Blood was sampled, 2 d apart, from Sprague-Dawley rats (30 males and 30 females) in both fed and non-fed states at 20 wk of age (weight: 470.2 ± 23.2 g) for measurement of ionized calcium (iCa), total intact (INT) and bioactive 1-84 (BIO) PTH. Differences between groups were tested using a GlIMMIX model with sex and feeding state as fixed effects and individual rats as a random effect. Results: Females had a lower iCa than males (F: 1.43 ± 0.01 vs M: 1.46 ± 0.01 mmol/l, P = 0.03). In males and females, there was no difference between fed and non-fed groups when PTH was assessed using the INT PTH assay (M Fed: 21.6 ± 1.3 vs M Non-fed: 22.6 ± 2.3 pmol/l, P = 0.59;F Fed: 19.3 ± 1.5 vs F Non-fed: 22.8 ± 2.9 pmol/l, P = 0.39). However, in females only, PTH measured using the BIO PTH, was significantly lower in the fed group versus the non-fed (Fed: 8.4 ± 0.8 vs Non-fed: 16.7 ± 3.4 pmol/l, P = 0.05). Conclusions:
