Improvement of Survival in Patient with Primary Metastatic Breast Cancer over a 10-Year Periode: Prospective Analyses Based on Individual Patient Date from a Multicenter Data Bank

Approximately 6% of patients with breast cancer have distant metastases at the time of the initial diagnosis. The aim of this analysis was to examine the overall survival rate over time and to investigate the effect of new therapy options. Methods: This retrospective analysis was performed based on the data bank of the Clinic for Gynaecological Oncology/Dr. Horst Schmidt Klinik, Wiesbaden and the Clinic for Gynaecological Oncology and Senology/Kliniken Essen Mitte, Essen. The patients with primary metastatic breast cancer (pmBC) who were diagnosed and treated at the accredited breast cancer centres of these clinics were enrolled between 1998 and 2007. The date of diagnosis was used to define 2 specifically chosen 5-year periods: 1998-2002 and 2003-2007. The follow-up time was on average 76 months. The Breslow Test was used to evaluate changes in the median survival time and to detect factors associated with the increase in survival rates. Results: Two hundred sixteen patients with complete baselines were analysed. Ninety patients were diagnosed between 1998 and 2002, and 126 patients received their diagnosis of pmBC between 2003 and 2007. The tumour-biological factors were the same in both groups, whereas the therapeutic concepts were different—the later group (2003-2007) received more aromatase inhibitors, taxane-based chemotherapy and trastuzumab. This finding resulted in an increased median survival time from 31 months in the years 1998-2002 to 44 months in the group with the first diagnosis between 2003 and 2007. Conclusions: Primary metastatic breast cancer occurred at constant rates over last 10 years. The tumour findings did not change in the time between the two examined groups;however, the treatment options in the 2003-2007 group included newly approved therapies. Selective recovery of rare earth elements by smelting of magnets

Rare earth elements(REEs)play a crucial role in many technologies from daily appliances in cell phones to more advanced wind turbines and electric *** magnets account for a quarter of total global REEs production and have high recycling *** this study,smelting process was used to selectively oxidize REEs in the permanent magnets by adding Fe_(2)O_(3).This separates REEs into a slag phase from an iron-rich metallic phase.B_(2)O_(3) was also added to the system as a flux to lower the slag melting *** minimizes REEs loss to the metallic phase and allows a more efficient phase *** effect of flux and oxidizing agent addition was investigated on both regular and cerium-rich NdFeB(NdCeFeB)*** 1350℃and for 1 h,the slag phase was successfully separated from the metallic phase with the addition of 0.8 stoichiometric amount of Fe_(2)O_(3) and 40 wt%of B_(2)O_(3).Scanning electron microscopy-energy dispersive X-ray spectroscopy(SEM-EDX)analysis reveals that REEs in the magnet do not migrate to the metal phase while the REE-rich slag phase contains almost no *** the selective removal of iron into the metallic phase,REEs are recovered from the slag phase through an acid leaching process allowing>99%of REEs *** in the magnet can also be recovered as useful boric acid by evaporation and crystallisation *** proposed process in this study is reagent and energy-efficient with almost complete valorisation of both NdCeFeB and NdFeB magnets.

Trilayer anisotropic structure versus randomly oriented structure in heart valve leaflet tissue engineering

It has been hypothesized that leaflet substrates with a trilayer structure and anisotropicmechanical properties could be useful for the production of functional and long-lasting tissue-engineered *** investigate the influence of the anisotropic structural and mechanical characteristics of a substrate on cells,in this study,we electrospun trilayer anisotropic fibrous substrates and randomly oriented isotropic fibrous substrates(used as controls)from polycaprolactone ***,the random substrates had higher radial and lower circumferential tensile properties than the trilayer substrates;however,they had similar flexural *** valvular interstitial cells cultured on both substrates produced random and trilayer cell-cultured constructs,*** trilayer cell-cultured constructs had more anisotropic mechanical properties,17%higher cellular proliferation,14%more extracellular matrix(i.e.,collagen and glycosaminoglycan)production,and superior gene and protein expression,suggesting that more cells were in a growth state in the trilayer constructs than in the random ***,the random and radial layers of the trilayer constructs had more vimentin,collagen,transforming growth factor-beta 1(TGF-ß1),transforming growth factor-beta 3(TGF-ß3)gene expression than in the circumferential layer of the *** study verifies that the differences in structural,tensile,and anisotropic properties of the trilayer and random substrates influence the characteristics of the cells and ECM in the constructs.

Basic theorem as representation of heterogeneous concept lattices

We propose a method for representing heteroge- neous concept lattices as classical concept lattices. Particu- larly, we describe a transformation of heterogeneous formal context into a binary one, such that corresponding concept lattices will be isomorphic. We prove the correctness of this transformation by the basic theorem for heterogeneous as well as classical concept lattices.

Preparation of LDPE-Acetylated/Butyrylated Starch Blend Blow Films and Characterization

Development of modified plastics has been studied through the LDPE-acylated starch blend films to examine the effect of different acyl groups and degrees of substitution （DS） on properties of films. Corn starch was modified with acetyl and hutyryl groups and films were prepared by blending acylated starch with low density polyethylene （LDPE）. Systematic studies were done to observe the effect of acyl groups, DS and starch concentration on the properties and biodegradability of the blend films. It was observed that blend films containing 5% acetylated and butyrylated starches of high DS （2.5, 1.7） maintained 75% and 83% of tensile strength of LDPE films. Thermal analysis results indicated that acetylated and butyrylated starch blend films decomposed at 370℃ and 389℃ which were higher than the decomposition temperature of native starch film （349℃）. Scanning electron micrographs of blend films containing high DS acylated starch showed well dispersed starch particles due to improvement in dispersion between starch and LDPE. Water absorption capacity of high DS acetylated and butyrylated starch blend films （4.18% and 3.76%, respectively） was lower than that of native starch films （5%）. This study has an advantage because of blown films prepared can be integrated with the present manufacturing systems without any other requirement.

Evaluation of the expression of sperm proteins in normozoospermic and asthenozoospermic men using monoclonal antibodies

Recent studies have shown that infertility affects estimated 15% of all couples. Male infertility is the primary or contributory cause in 60% of these cases. Consequently, the application of assisted reproduction is increasing. These methods could benefit from an extended evaluation of sperm quality. For this reason, we analyzed sperm proteins from 30 men with normal spermiograms and 30 men with asthenozoospermia. Ejaculates of both groups were tested by flow cytometry （FCM） and fluorescence with a set of well-characterized anti-human sperm Hs-monoclonal antibodies （MoAbs）, which were generated in our laboratory. No statistically significant differences were found between normospermics and asthenospermics in the expression of the sperm surface protein clusterin, evaluated with Hs-3 MoAb, and semenogelin, evaluated with Hs-9 MoAb. However, FCM revealed quantitative differences in the acrosomal proteins between normozoospermic and asthenozoospermic men, namely, in glyceraldehyde-3-phosphate dehydrogenase, evaluated with Hs-8 MoAb, valosin-containing protein, evaluated with Hs-14 MoAb, and ATP synthase （cAMP-dependent protein kinase II, PRKAR2A）, evaluated with MoAb Hs-36. Asthenozoospermic men displayed a highly reduced expression of intra-acrosomal proteins, with a likely decrease in sperm quality, and thus a negative impact on successful reproduction. Asthenozoospermia seems to be a complex disorder involving intra-acrosomal proteins.

Online Induction of Probabilistic Real-Time Automata

The probabilistic real-time automaton （PRTA） is a representation of dynamic processes arising in the sciences and industry. Currently, the induction of automata is divided into two steps： the creation of the prefix tree acceptor （PTA） and the merge procedure based on clustering of the states. These two steps can be very time intensive when a PRTA is to be induced for massive or even unbounded datasets. The latter one can be efficiently processed, as there exist scalable online clustering algorithms. However, the creation of the PTA still can be very time consuming. To overcome this problem, we propose a genuine online PRTA induction approach that incorporates new instances by first collapsing them and then using a maximum frequent pattern based clustering. The approach is tested against a predefined synthetic automaton and real world datasets, for which the approach is scalable and stable. Moreover, we present a broad evaluation on a real world disease group dataset that shows the applicability of such a model to the analysis of medical processes.
