Liver failure caused by prolonged state of malnutrition following bariatric surgery

Bariatric surgery is an effective tool in the treatment of patients with morbid obesity. In these case reports we describe 2 patients who developed liver failure after currently-practiced types of bariatric surgery, caused by a prolonged state of malnutrition provoked by psychiatric problems. Despite intensive guidance of a psychologist and dieticians after surgery, our patients deteriorated psychologically, resulting in a prolonged state of severe malnutrition and anorexia. Finally, a state of starvation was reached, passing a critical level of the liver capacity. Patients who present with signs of severe protein malnutrition after bariatric surgery should be closely monitored and checked for nutritional status. Specific attention should be given to patients who develop psychiatric problems post-bariatric surgery. If refeeding does not result in clinical improvement, reversal surgery should be considered in a timely manner.

In recent years the occurrence of civilisation related diseases such as diabetes and obesity is increasing at a dramatic rate. This results in a growing consumer awareness of relationships between diet and health. Industry is respond ing to this trend by introducing products with a healthy image, including products with dietary fibre. Every year about 500 products are being launched globally with statements regarding their fibre content. Dietary fibre is usually defined as "the edible parts of plants or analogous carbohydrates that are resistant to digestion and absorption in the human small intestine with complete or partial fermentation in the large intestine. Dietary fibre includes polysaccharides, oligosaccharides, lignin, and associated substances. Dietary fibres promote beneficial physiological effects including laxation, and/or blood cholesterol attenuation, and/or blood glucose attenuation" (AACC, 2001).

Waste disposal management and the energy crisis are important challenges facing most *** fruit-processing industry generates daily several tons of wastes,of which the major share comes from banana *** digestion(AD)technology has been applied to the treatment of wastewater,animal slurry,food waste,and agricultural residues,with the primary goals of energy production and waste *** study examines the effect of organic loading(OL)and cow manure(CM)addition on AD performance when treating banana peel waste(BPW).The maximum daily biogas production rates of banana peels(BPs)with a CM content of 10%,20%,and 30%at 18 and 22 g of volatile solids(gvs)per liter were 50.20,48.66,and 62.78 mL (gvs.d)^-1 and 40.49,29.57,and 46.54 mL (gvs.d)^-1,***,the daily biogas yield showed no clear interdependence with OL or CM *** addition,a kinetic analysis using first-order and cone models showed that the kinetic parameters can be influenced by the process parameters.

The current irrational use of fossil fuels and the impact of greenhouse gases on the environment are driving research into renewable energy production from organic resources and waste. The global energy demand is high, and most of this energy is produced from fossil resources. Recent studies report that anaerobic di- gestion （AD） is an efficient alternative technology that combines biofuel production with sustainable waste management, and various technological trends exist in the biogas industry that enhance the production and quality of biogas. Further investments in AD are expected to meet with increasing success due to the low cost of available feedstocks and the wide range of uses for biogas （i.e., for heating, electricity, and fuel）. Bio- gas production is growing in the European energy market and offers an economical alternative for bioenergy production. The objective of this work is to provide an overview of biogas production from lignocellulosic waste, thus providing information toward crucial issues in the biogas economy.

The auxin signaling molecule controls a variety of growth and developmental processes in land plants. Auxin regulates gene expression through a nuclear auxin signaling pathway (NAP) consisting of the ubiquitin ligase auxin receptor TIR1/AFB, its Aux/IAA degradation substrate, and DNA-binding ARF transcription factors. Although extensive qualitative understanding of the pathway and its interactions has been obtained, mostly by studying the flowering plant Arabidopsis thaliana, it remains unknown how these translate to quantitative system behavior in vivo, a problem that is confounded by the large NAP gene families in most species. Here, we used the minimal NAP of the liverwort Marchantia polymorpha to quantitatively map NAP protein accumulation and dynamics in vivo through the use of knockin fluorescent fusion proteins. Beyond revealing the dynamic native accumulation profile of the entire NAP protein network, we discovered that the two central ARFs, MpARF1 and MpARF2, are proteasomally degraded. This auxin-independent degradation tunes ARF protein stoichiometry to favor gene activation, thereby reprogramming auxin response during the developmental progression. Thus, quantitative analysis of the entire NAP has enabled us to identify ARF degradation and the stoichiometries of activator and repressor ARFs as a potential mechanism for controlling gemma germination.

BACKGROUND Individuals with benign kidney disorders undergoing nephrectomy have three possibilities:Autotransplantation,with a certain risk of complications,but without a clear benefit;discarding the kidney;or living kidney *** To investigate whether patients with benign kidney disorders and a medical indication for nephrectomy are suitable as unspecified live kidney *** We searched all clinical data from 1994-2019 for unspecified donors and their transplant recipients(n=160).Nine of these 160 donors had pre-existing kidney disorders necessitating nephrectomy and had decided to donate their kidney anonymously after discussing the possibility of kidney *** studied the clinical course of these nine donating patients and their transplant *** Seven of nine donating patients indicated unbearable loin pain as the main complaint,one donating patient refused ureterocutaneostomy and one had two aneurysms of the renal ***,seven donating patients described absence of pain and one a significant reduction after the *** average 1-year creatinine level in the donating patients was 88μmol/L and after a median of 6.9 years the average creatinine level was 86.6μmol/*** the transplant recipients,one major complication occurred which led to death and in one transplant recipient graft function failed to normalize at first but has been stable for nine years ***,all transplant recipients are off *** Our data show that patients undergoing nephrectomy as part of treatment in selected kidney disorders can function as live kidney donors.

This study examined concentrations of 15 perfluoroalkyl acids（PFAAs） in tissues from male Mozambique tilapia（Oreochromis mossambicus） collected at Loskop Dam, Mpumalanga,South Africa in 2014 and 2016. Nine of the 15 PFAAs were detected frequently and were included in statistical analysis and included two of the most commonly known PFAAs,perfluorooctanesulfonic acid（PFOS）（median, 41.6 ng/g） and perfluorooctanoic acid（PFOA）（median, 0.0825 ng/g）. Of the tissues measured, plasma（2016 and 2014 median, 22.2 ng/g）contained the highest PFAA burden followed by（in descending order）： liver（median,11.6 ng/g）, kidney（median, 9.04 ng/g）, spleen（median, 5.92 ng/g）, adipose（median, 2.54 ng/g）, and muscle（median, 1.11 ng/g）. Loskop Dam tilapia have been affected by an inflammatory disease of the adipose tissue known as pansteatitis, so this study also aimed to investigate relationships between PFAA tissue concentrations and incidence of pansteatitis or fish health status. Results revealed that healthy tilapia exhibited an overall higher（p-value 〈 0.05） PFAA burden than pansteatitis-affected tilapia across all *** analysis showed that organs previously noted in the literature to contain the highest PFAA concentrations, such as kidney, liver, and plasma, were the organs driving the difference in PFAA burden between the two tilapia groups. Care must be taken in the interpretations we draw from not only the results of our study, but also other PFAA measurements made on populations（human and wildlife alike） under differing health status.
