Here we present the case of a 20-year-old male with a known history of an artificial mechanical aortic valve replacement at ages four and twelve, on chronic warfarin therapy, who presented to our community emergency department for the evaluation of vomiting, diarrhea, generalized malaise and ab.ominal pain. He was seen at a local urgent care for similar symptoms earlier in the day, sub.equently discharged with a diagnosis of mild gastroenteritis; however, according to the patient＇s sister, upon his return home he looked very unwell and she called an amb.lance immediately.

b.ckground Carotid endarterectomy(CEA)results in fewer perioperative strokes,b.t more myocardial infarctions(MI)than carotid artery stenting(CAS).We explored a comb.ned modelling approach that stratifies patients b. b.seline stroke and *** b.seline registry-b.sed risk models for perioperative stroke and MI were identified via literature *** then selected treatment risk models in the Carotid Revascularisation Stenting versus Endarterectomy(CREST)trial b. serially adding covariates(b.seline risk,treatment(CEA vs CAS),treatment-risk interaction and age-treatment interaction terms).Treatment risk models were externally validated using data from the Society for Vascular Surgery(SVS)Vascular Quality Initiative(VQI)CEA and carotid stenting registries and treatment models were recalib.ated to the SVS-VQI *** net b.nefit was estimated b. summing the predicted stroke and MI risk differences with CEA versus *** Perioperative treatment models had moderate predictiveness(c-statistic 0.69 for stroke and 0.68 for MI)and reasonab.e calib.ation across the risk spectrum for b.th stroke and MI within *** external validation in SVS-VQI,predictiveness was sub.tantially reduced(c-statistic 0.61 for stroke and 0.54 for MI)and models sub.tantially overpredicted *** patients(86.7%)were predicted to have net b.nefit from CEA in CREST(97.0%of symptomatic patients vs 75%of asymptomatic patients).Discussion A comb.ned modelling approach that separates risk elements has potential to inform optimal ***,our current approach is not ready for clinical *** data support guidelines that suggest that CEA should b. the preferred revascularisation modality in most patients with symptomatic carotid stenosis.

The 2030 Agenda for Sustainab.e Development outlines 17 individual Sustainab.e Development Goals （SDGs） that guide the needs of practice for many professional disciplines around the world, including engineering, research, policy, and development. The SDGs represent commitments to reduce poverty, hunger, ill health, gender inequality, environmental degradation, and lack of access to clean water and sanitation. If a typical reductionist approach is employed to address and optimize individual goals, it may lead to a failure in technological, policy, or managerial development interventions through unin- tended consequences in other goals. This study uses a systems approach to understand the fundamental dynamics b.tween the SDGs in order to identify potential synergies and antagonisms. A conceptual system model was constructed to illustrate the causal relationships b.tween SDGs, examine system structures using generic system archetypes, and identify leverage points to effectively influence intentional and minimize unintentional changes in the system. The structure of interactions among the SDGs reflects three archetypes of system b.havior： Reinforcing Growth, Limits to Growth, and Growth and Underinvestment. The leverage points identified from the conceptual model are gender equality, sustainab.e management of water and sanitation, alternative resources, sustainab.e livelihood standards, and glob.l partnerships. Such a conceptual system analysis of SDGs can enhance the likelihood that the development community will b.oaden its understanding of the potential synergistic b.nefits of their projects on resource management, environmental sustainab.lity, and climate change. b. linking the interactions and feedb.cks of those projects with economic gains, women＇s empowerment, and educational equality, stakeholders can recognize holistic improvements that can b. made to the quality of life of many of the world＇s poor.

Neurodegenerative diseases such as Alzheimer's disease, Parkinson's disease, Huntington's disease, and amyotrophic lateral sclerosis are a heterogeneous group of deb.litating disorders with multifactorial etiologies and pathogeneses that manifest distinct molecular mechanisms and clinical manifestations with ab.ormal protein dynamics and impaired b.oenergetics. Mitochondrial dysfunction is emerging as an important feature in the etiopathogenesis of these age-related neurodegenerative diseases. The prevalence and incidence of these diseases is on the rise with the increasing glob.l population and average lifespan. Although many therapeutic approaches have b.en tested, there are currently no effective treatment routes for the prevention or cure of these diseases. We present the current status of our knowledge and understanding of the involvement of mitochondrial dysfunction in these diseases and highlight recent advances in novel therapeutic strategies targeting neuronal b.oenergetics as potential approach for treating these diseases.

AIM： To measure the time-dependent （viscoelastic） b.havior in the change of the small intestinal opening angle and to test how well the b.havior could b. describ.d b. the Kelvin model for a standard linear solid. METHODS： Segments from the duodenum, jejunum, and ileum were harvested from 10 female Wistar rats and the luminal diameter, wall thickness, and opening angle over time （θ（t）） were measured from rings cut from these segments. RESULTS： Morphometric variations were found along the small intestine with an increase in luminal area and a decrease in wall thickness from the duodenum to the ileum. The opening angle ob.ained after 60 rain was highest in the duodenum （220.8±12.9°）and decreased along the length of the intestine to 143.9±8.9° in the jejunum and 151.4±9.4° in the ileum. The change of opening angle as a function of time, fitted well to the Kelvin model using the equation θ（t）/θo = [1-ηexp （-λt）] after the ring was cut. The computed creep rate λ. did not differ b.tween the segments. Compared to constant calculated from pig aorta and coronary artery, it showed that α agreed well （within 5%）, η was three times larger than that for vascular tissue, and λ ranged ±40% from the value of the pig coronary artery and was a third of the value of pig aorta. CONCLUSION： The change of opening angle over time for all the small intestine segments fits well to the standard linear spring-dashpot model. This viscoelastic constant of the rat small intestine is fairly homogenous along its length. The data ob.ained from this study add to a b.se set of b.omechanical data on the small intestine and provide a reference state for comparison to other tissues,diseased intestinal tissue or intestinal tissue exposed to drugs or chemicals.

A new manufacturing and b.siness infrastructure industrial ecosystem model with the state-of-the art core infrastructure and smart applications in the form of inclusive smart innovation cluster is seen as a powerful tool to drive industrial symb.osis, economic, social and inclusive development. Essentially an integrated eco-industrial park, these clusters demonstrate applications of industrial ecology principles among the occupant units, b.sides promoting sustainab.e societies from economic, environmental and social perspectives. The holistic planning and development of these clusters from economic, environmental and social sustainab.lity considerations or alternatively, the sustainab.lity assessment of these clusters is highly complex. The research reported upon forms part of a larger study that aims to develop an integrated decision support system for sustainab.lity assessment of inclusive smart innovation cluster. The paper discusses the conceptual model of the cluster and the need to estab.ish context specific key indicators and influence factors from economic, environmental and social sustainab.lity considerations. The paper proposes a structured methodology for development of key indicators and influence factors. The paper concludes b. identifying a set of 184 key indicators and influence factors for comprehensive sustainab.lity assessment of inclusive smart innovation cluster.

This paper develops an information theoretic estimator of wage and employment shifts that provides quantification of and inference ab.ut type of employment (full time, part time, seasonal, unemployed) and industry share of employment. It is applied to determine the effects of two United States Agency for International Development (USAID) projects on employment in villages in the Tamb.counda and Kedougou provinces in Senegal. The projects support the development of local forest-b.sed production and processing industries. The results demonstrate statistically significant and positive effects on employment and the industry composition of employment.

The purpose of this project is to examine why the mental health of employees should b. prioritized in the workplace. Most employees have likely experienced a visib.e illness or injury at some point during their careers. It is easier to identify these physical ailments that may adversely affect an employee’s performance at work, such as a b.oken leg or the common cold. However, there are conditions that are much less visib.e that negatively impact employees every day, and mental health is one of them. A person’s mental health is a significant factor in their work life that should not b. ignored. While it may b. difficult to recognize when someone is struggling with depression, anxiety, or another form of mental illness on the surface, employers should not automatically assume their entire staff is okay. As a result of the COVID-19 pandemic coupled with an increase in civil unrest in society, employers must reevaluate the way they conduct b.siness, starting with the treatment of their employees. To aid employers with prioritizing mental health at work, this paper’s ob.ectives are to address the stigma associated with mental health issues/illness, develop recommendations for inclusive work environments, increase an employer’s investment in mental health, reduce employee turnover, and normalize respect in the workplace. The findings are discussed, and recommendations are provided. Employers who intentionally prioritize and invest in their employees’ mental health will significantly reduce the costs associated with attracting and recruiting new staff due to high turnover rates. Additionally, their reputation as a preferred employer will increase.
