Origin of Magnetic Fields of Stellar Objects in the Universe Based on the 5D Projection Theory

Beginning with a 5D homogeneous universe [1], we have provided a plausible explanation of the self-rotation phenomenon of stellar objects previously with illustration of large number of star samples [2], via a 5D-4D projection. The origin of such rotation is the balance of the angular momenta of stars and that of positive and negative charged e-trino pairs, within a 3D ⊗1D?void of the stellar object, the existence of which is based on conservation/parity laws in physics if one starts with homogeneous 5D universe. While the in-phase e-trino pairs are proposed to be responsible for the generation of angular momentum, the anti-phase but oppositely charge pairs necessarily produce currents. In the 5D to 4D projection, one space variable in the 5D manifold was compacted to zero in most other 5D theories (including theories of Kaluza-Klein and Einstein [3] [4]). We have demonstrated, using the Fermat’s Last Theorem [5], that for validity of gauge invariance at the 4D-5D boundary, the 4th space variable in the 5D manifold is mapped into two current rings at both magnetic poles as required by Perelman entropy mapping;these loops are the origin of the dipolar magnetic field. One conclusion we draw is that there is no gravitational singularity, and hence no black holes in the universe, a result strongly supported by the recent discovery of many stars with masses well greater than 100 solar mass [6] [7] [8], without trace of phenomena observed (such as strong gamma and X ray emissions), which are supposed to be associated with black holes. We analyze the properties of such loop currents on the 4D-5D boundary, where Maxwell equations are valid. We derive explicit expressions for the dipolar fields over the whole temperature range. We then compare our prediction with measured surface magnetic fields of many stars. Since there is coupling in distribution between the in-phase and anti-phase pairs of e-trinos, the generated mag-netic field is directly related to the angular

Era of direct acting antivirals in chronic hepatitis C: Who will benefit?

In the era of highly effective direct acting antiviral(DAA) drugs for the treatment of chronic hepatitis C(CHC) infection, where eradication is almost ensured with minimal side effects, all hepatitis C carriers should benefit theoretically. In the real world setting however, only a small proportion will benefit at this time point due to the multiple barriers to accessing therapy. Given that universal treatment is unlikely, treatment with DAAs will likely be restricted to those with the highest health benefits, and for those who can afford the high expense of a treatment course. Those with the highest unmet needs include those who have failed previous interferon-based therapy or who are interferon-ineligible with evidence of active disease, those with advance liver disease, and those with recurrence of hepatitis C after liver transplantation. In the future, the focus should be on increasing access to treatment for those infected with CHC.

Clinical application of liver stiffness measurement using transient elastography in chronic liver disease from longitudinal perspectives

Accurate determination of the presence and degree of fibrosis in liver is of great importance, because the prognosis and management strategies for chronic liver disease depend mainly on these factors. To date, liver biopsy (LB) remains the "gold standard" for assessing the severity of liver fibrosis; however, LB is often limited by its invasiveness, sampling error, and intra/ inter-observer variability in histological interpretation. Furthermore, repeated LB examinations within a short time interval are indeed ineligible in a real clinical practice. Thus, due to the pressing need for non-invasive surrogates for liver fibrosis, transient elastography (TE),as a novel ultrasound based technology, has allowed a noninvasive measurement of liver stiffness and has gained in popularity over recent years. In the past few years, additional roles for transient TE beyond the initial purpose of a non-invasive surrogate for LB have included the prediction of the most two critical consequences of fibrosis progression: the development of portal hypertension-related complications and hepatocellular carcinoma. This indicates that the role of transient TE is not merely limited to reducing the need for LB, but transient TE can enable the establishment of tailored management strategies by providing more detailed prognostic information. In particular, under the concept in which the clinical course of liver fibrosis is dynamic and bidirectional, especially when appropriate intervention is commenced, transient TE can be used to track the dynamic changes in fibrotic burden during antiviral or antifibrotic treatment. This review discussed extended applications of transient TE in prediction of the development of real clinical endpoints from a longitudinal perspective.

On the Origin of Mass and Angular Momentum of Stellar Objects

The consequence of the 5D projection theory [1] is extended beyond the Gell-Mann Standard Model for hadrons to cover astronomical objects and galaxies. The proof of Poincare conjecture by Pe-relman’s differential geometrical techniques led us to the consequence that charged massless spinors reside in a 5D void of a galactic core, represented by either an open 5D core or a closed, time frozen, 3D × 1D space structure, embedded in massive structural stellar objects such as stars and planets. The open galactic core is obtained from Ricci Flow mapping. There exist in phase, in plane rotating massless spinors within these void cores, and are responsible for 1) the outward spiral motion of stars in the galaxy in the open core, and 2) self rotations of the massive stellar objects. It is noted that another set of eigen states pertaining to the massless charged spinor pairs rotating out of phase in 1D (out of the 5D manifold) also exist and will generate a relatively weak magnetic field out of the void core. For stars and planets, it forms the intrinsic dipole field. Due to the existence of a homogeneous 5D manifold from which we believe the universe evolves, the angular momentum arising from the rotation of the in-phase spinor pairs is proposed to be counter-balanced by the rotation of the matter in the surrounding Lorentz domain, so as to conserve net zero angular momentum. Explicit expression for this total angular momentum in terms of a number of convergent series is derived for the totally enclosed void case/core, forming in general the structure of a star or a planet. It is shown that the variables/parameters in the Lorentz space-time domain for these stellar objects involve the object’s mass M, the object’s Radius R, period of rotation P, and the 5D void radius Ro, together with the Fermi energy Ef and temperature T of the massless charged spinors residing in the void. An HPC Based Real-time Path Rating Calculation Tool for Congestion Management with High Penetration of Renewable Energy

Every year, transmission congestion costs billions ofdollars for electricity customers. This clearly identifies the criticalneed for more transmission capacity and also poses big challengesfor power grid reliability in stressed conditions due to heavyloading and in uncertain situations due to variable renewableresources and responsive smart loads. However, it becomesincreasingly difficult to build new transmission lines, whichtypically involve both economic and environmental *** this paper, advanced computing techniques are developedto enable a non-wire solution that realizes unused transfercapabilities of existing transmission facilities. An integratedsoftware prototype powered by high-performance computing(HPC) is developed to calculate ratings of key transmission pathsin real time for relieving transmission congestion and facilitatingrenewable integration, while complying with the North AmericanElectric Reliability Corporation (NERC) standards on assessingtotal transfer capabilities. The innovative algorithms include: (1)massive contingency analysis enabled by dynamic load balancing,(2) parallel transient simulation to speed up single dynamicsimulation, (3) a non-iterative method for calculating voltagesecurity boundary and (4) an integrated package consideringall NERC required limits. This tool has been tested on realisticpower system models in the Western Interconnection of NorthAmerica and demonstrates satisfactory computational speedusing parallel computers. Various benefits of real-time path ratingare investigated at Bonneville Power Administration using realtime EMS snapshots, demonstrating a significant increase in pathlimits. These technologies would change the traditional goals ofpath rating studies, fundamentally transforming how the grid isoperated, and maximizing the utilization of national transmissionassets, as well as facilitating integration of renewable energy andsmart loads.

Patterns of in-hospital mortality and bleeding complications following PCI for very elderly patients： insights from the Dartmouth Dynamic Registry

Background Very elderly patients （age 〉 85 years） are a rapidly increasing segment of the population. As a group, they experience high rates of in-hospital mortality and bleeding complications following percutaneous coronary intervention （PCI）. However, the relationship between bleeding and mortality in the very elderly is unknown. Methods Retrospective review was performed on 17,378 consecutive PCI procedures from 2000 to 2015 at Dartmouth-Hitchcock Medical Center. Incidence of bleeding during the index PCI admission （bleeding requiring transfusion, access site hematoma 〉 5 cm, pseudoaneurysm, and retroperitoneal bleed） and in-hospital mortality were reported for four age groups （〈 65 years, 65-74 years, 75-84 years, and ≥ 85 years）. The mortality of patients who suffered bleeding complications and those who did not was calculated and multivariate analysis was performed for in-hospital mortality. Lastly, known predictors of bleeding were compared between patients age 〈 85 years and age ≥85 years. Results Of 17,378 patients studied, 1019 （5.9%） experienced bleeding and 369 （2.1%） died in-hospital following PCI. Incidence of bleeding and in-hospital mortality increased monotonically with increasing age （mortality： 0.94%, 2.27%, 4.24% and 4.58%; bleeding： 3.96%, 6.62%, 10.68% and 13.99% for ages 〈 65, 65-4, 75-84 and ≥ 85 years, respectively）. On multivariate analysis, bleeding was associated with increased mortality for all age groups except patients age ≥85 years [odds ratio （95% CI）： age 〈 65 years, 3.65 （1.99-6.74）; age 65-74 years, 2.83 （1.62-4.94）; age 75-84 years, 3.86 （2.56-5.82）, age ≥ 85 years 1.39 （0.49-3.95）]. Conclusions Bleeding and mortality following PCI increase with increasing age. For the very elderly, despite high rates of bleeding, bleeding is no longer predictive of in-hospital mortality following PCI.

Long-Term Trends in Pre-Summer Daytime and Nocturnal Extreme Hourly Rainfall in a Coastal City of South China

The long-term trends in the occurrence frequency of pre-summer daytime and nocturnal extreme hourly rainfall(EXHR) during 1988-2018 in Hong Kong and their spatial distributions are examined and analyzed. Despite a significant increasing trend observed in the occurrence frequency of pre-summer EXHRs during the investigated period,the increase in daytime and nocturnal EXHRs show distinct spatial patterns. Nocturnal EXHRs show uniform increasing trends over the entire Hong Kong. However, the increase in daytime EXHRs is concentrated over the northern or eastern areas of Hong Kong, indicating a downstream shift of pre-summer EXHRs in Hong Kong with regard to the prevailing southwesterly monsoonal flows in south China. The clustering of weather types associated with daytime and nocturnal EXHRs further reveals that the increase in EXHRs over Hong Kong are mainly contributed by the increase of the events associated with southwesterly monsoonal flows with relatively high speeds. During the past few decades, the southwesterly monsoonal flows at coastal south China have undergone a substantial weakening due to the increased surface roughness induced by the urbanization over the Guangdong-Hong Kong-Macao Greater Bay Area since 1990s,leading to enhanced low-level convergence and thus significant increase in EXHRs at coastal south China. Meanwhile,daytime sea-wind circulation at coastal south China is markedly enhanced during the investigated period, which is the main reason for the northward shift of daytime EXHRs in Hong Kong. In addition, the blocked southwesterly monsoonal flows at coastal south China are detoured eastward, leading to stronger convergence and increase in EXHRs at eastern coast of Hong Kong, especially during daytime, when the easterly sea winds prevail at the region.

It is largely unknown whether lower urinary tract symptoms(LUTS)or acute retention of urine(AROU)is linked to shorter life expectancy in *** conducted a multicenter,retrospective database analysis of patients undergoing transurethral resection of prostate(TURP)to study their *** Cox regression analysis and Kaplan-Meier analysis with stratification to age and indication of TURP were *** further performed an age-and sex-matched survival analysis with the general population using data from the Census and Statistics Department of the Hong Kong Special Administrative Region(Hong Kong,China).From January 2002 to December 2012,3496 patients undergoing TURP were included in our study,with 1764 patients in the LUTS group and 1732 patients in the AROU *** age,ischemic heart disease,cerebrovascular accident,and AROU were risk factors of *** aged80 *** to the general population,younger patients in the LUTS group appeared to have better survival(80 years,survival was similar with the general population in both the LUTS and AROU *** conclusion,AROU at young age was associated with mortality,while early detection and management of LUTS may improve survival.
