Role of magnetic resonance cholangiopancreatography in diagnosing choledochal cysts: Case series and review

AIM:To determine the merits of magnetic resonance cholangiopancreatography(MRCP) as the primary diagnostic test for choledochal cysts(CC’s).METHODs:Between 2009 and 2012,patients who underwent MRCP for perioperative diagnosis were *** information,clinical characteristics,and radiographic findings were *** results were compared with intraoperative findings.A PubMed search identified studies published between 1996-2012,employing MRCP as the primary preoperative imaging and comparing results with either endoscopic retrograde cholangiopancreatography(ERCP) or operative *** rates for CC’s and abnormal pancreaticobiliary junction(APBJ) were *** addition detection rates for clinically related biliary pathology like choledocholithiasis and cholangiocarcinomas in patients diagnosed with CC’s were also ***:Eight patients were identified with CC’*** patients out of them had typeⅣCC’s,1 had type I and 1 had a new variant of choledochal cyst with confluent dilatation of the common bile duct(CBD) and cystic *** patients had an APBJ and 3 of those had a long *** were found in 2 patients,1 had a CBD stone,and 1 pancreatic-duct stone was also *** all cases,MRCP successfully identified the type of CC’s,as well as APBJ with ductal *** analyzing the literature,we found that MRCP has 96%-100% detection rate for CC’***,we found that the range for sensitivity,specificity,and diagnostic accuracy was 53%-100%,90%-100% and 56%-100% in diagnosing ***’s detection rate was 100% for choledocholithiasis and 87% for cholangiocarcinomas with concurrent CC’***:After initial ultrasound and computed tomography scan,MRCP should be the next diagnostic test in both adult and pediatric *** should be reserved for patients where therapeutic intervention is needed.

This paper reviews the progress on nano-aperture vertical-cavity surface-emitting lasers （VCsELs）. The design, fabrication, and polarization control of nano-aperture VCsELs are reviewed. With the nanoaperture evolving from conventional circular and square aperture to unique C-shaped, H-shaped, I-shaped, and bowtie-shaped aperture, both the near-field intensity and near-field beam confinement from nanoaperture VCsELs are significantly improved. As a high-intensity compact light source with sub-100- nm spot size, nano-aperture VCsELs are promising to realize many new near-field optical systems and applications.

Glass fi ber reinforced gypsum(GFRG) wall panels are prefabricated panels with hollow cores, originally developed in Australia and subsequently adopted by India and China for use in buildings. This paper discusses identifi cation and calibration of a suitable hysteretic model for GFRG wall panels fi lled with reinforced concrete. As considerable pinching was observed in the experimental results, a suitable hysteretic model with pinched hysteretic rule is used to conduct a series of quasi-static as inelastic hysteretic response analyses of GFRG panels with two different widths. The calibration of the pinching model parameters was carried out to approximately match the simulated and experimental responses up to 80% of the peak load in the post peak region. Interestingly, the same values of various parameters(energy dissipation and pinching related parameters) were obtained for all fi ve test specimens.

Hepatitis B remains a leading cause of cirrhosis, hepatocellular carcinoma and liver transplantation worldwide. Management of chronic hepatitis B during pregnancy is challenging. Transmission of hepatitis B to infants still occurs perinataily although immunoprophylaxis is widely available for infants born to mothers with chronic hepatitis B infection. The emerging data suggest that initiation of antiviral therapy in the beginning of the third trimester in highly viremic mothers can prevent immunoprophylaxis failure in their infants. The available drug safety data show that lamivudine, telbivudine and tenofovir are generally safe to be used during the pregnancy. In order to minimize the fetal exposure to the antiviral medication, antiviral therapy during the pregnancy should be limited to a selected group of patients with cirrhosis, high hepatitis B viral load, or prior history immunoprophylaxis failure. An elective Caesarean section may reduce the risk of perinatal transmission. For those females planning for pregnancy or in early stage of pregnancy, communication and follow-up among obstetrician, gastroenterologist, and primary care physician are important. In this article, we will review the features of hepatitis B infection before, during and after the pregnancy; the risk factors that increase mother-tochild transmission; safety data on antiviral drug use during pregnancy; and the potential role of Caesarean section in selected cases.
