Clinical significance of isolated biliary candidiasis in patients with unresectable cholangiocarcinoma

BACKGROUND: The frequency of isolated biliary candidiasis is increasing in cancer patients. The clinical signiifcance of isolated biliary candidiasis remains unclear. We analyzed the risk factors of biliary candidiasis and outcomes of the patients with unresectable cholangiocarcinoma after percutaneous transhepatic biliary drainage (PTBD). METHODS: Among 430 patients who underwent PTBD between January 2012 and March 2015, 121 patients had unresectable cholangiocarcinoma. Bile and blood samples were collected for consecutive fungal culture. RESULTS: The study cohort included 49 women and 72 men with a median age of 71 years. Multivariate analysis showed that cancer progression (P=0.013), concurrent presence of another microorganism (P=0.010), and previous long-term (>7 days) antibiotic use (P=0.011) were potential risk factors of biliary candidiasis. Chemotherapy was not associated with overall biliary candidiasis (P=0.196), but was signiifcantly related to repeated biliary candidiasis (P=0.011). Patients with isolated biliary candidiasis showed remarkably reduced survival compared with those without (median overall sur-vival (OS): 32 vs 62 days,P=0.011)Subgroup analysis was also performed. Patients with repeated candidiasis had markedly decreased survival compared with those with transient candi-diasis (median OS: 30 vs 49 days,P=0.046). Biliary candidiasis was identiifed as a poor prognostic factor by univariate and multivariate analyses (P=0.033). Four cases of repeated can-didiasis (4/19, 21%) showedCandida species in consecutive blood culture until the end of the study, but others showed no candidemia. CONCLUSIONS: Isolated biliary candidiasis may be associ-ated with poor prognosis in patients with unresectable chol-angiocarcinoma. Especially, repeated biliary candidiasis may have the possibility of progression to candidemia. We suggest that biliary dilatation treatment or antifungal agents might be helpful for patients with biliary candidiasis.

BACKGROUND Urinary tract infection(UTI)is one of the most common bacterial *** pyelonephritis or upper urinary tract infection is often accompanied by bacteremia;however,bacteremia resolves in most cases without ***,complications due to bacteremia *** of these is *** mainly affects the lumbar vertebral bodies,and rarely affects other *** SUMMARY An 80-year-old woman presented to the hospital with a two-month history of pain in both *** months ago,she was admitted to the hospital for fever,flank pain,and urinary frequency and was diagnosed with bacteremic *** hospitalization,she complained of pain in both legs;however,the pain resolved shortly after,and no abnormalities were observed on physical ***,she was placed on 2-wk antibiotic therapy for UTI without further evaluation for leg ***,pain recurred after discharge and persisted;therefore,an imaging test was *** scan and magnetic resonance imaging suggested osseous infection in both femurs,tibiae and *** treatment was performed,and tissue-and bone cultures revealed Escherichia coli,a previously observed pathogen,which demonstrated same antibiotic sensitivities,as noted in previous *** was diagnosed with disseminated osteomyelitis,as a complication of UTI,and was placed on an 8-wk antibiotic *** Indication for osteomyelitis should be high regardless of bone pain at sites other than lumbar spine after or during UTI.

AIM: To evaluated the value of 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT) scan in diagnosis of hepatocellular carcinoma (HCC) and extrahepatic metastases. METHODS: A total of 138 patients with HCC who had both conventional imaging modalities and 18F-FDG PET/CT scan done between November 2006 and March 2011 were enrolled. Diagnostic value of each imaging modality for detection of extrahepatic metastases was evaluated. Clinical factors and tumor characteristics including PET imaging were analyzed as indicative factors for metastases by univariate and multivariate methods. RESULTS: The accuracy of chest CT was significantly superior compared with the accuracy of PET imaging for detecting lung metastases. The detection rate of metastatic pulmonary nodule ≥ 1 cm was 12/13 (92.3%), when < 1 cm was 2/10 (20%) in PET imaging. The accuracy of PET imaging was significantly superior compared with the accuracy of bone scan for detecting bone metastases. In multivariate analy- sis, increased tumor size (≥ 5 cm) (P = 0.042) and increased average standardized uptake value (SUV) uptake (P = 0.028) were predictive factors for extrahepatic metastases. Isometabolic HCC in PET imaging was inversely correlated in multivariate analysis (P = 0.035). According to the receiver operating characteristic curve, the optimal cutoff of average SUV to predict extrahepatic metastases was 3.4. CONCLUSION: 18F-FDG PET/CT scan is invaluable for detection of lung metastases larger than 1 cm and bone metastases. Primary HCC having larger than 5 cm and increased average SUV uptake more than 3.4 should be considered for extrahepatic metastases.

Development of a valid and reliable seven emotions impairment questionnaire and assessment of its predictability for phlegm and blood stasis

OBJECTIVE:To develop and validate a Seven Emotions Impairment questionnaire(SEIQ),to define an optimum cut-off point for the SEIQ,and to examine whether SEI was predictive of Phlegm and Blood Stasis(BS).METHODS:Two hundred outpatients and 75 college students were asked to complete the SEIQ,the Profile of Mood States(POMS),Phlegm Pattern Questionnaire(PPQ),and BS Questionnaire(BSQ).Twelve clinicians determined whether the outpatients exhibited *** data were used to examine the internal consistency and determine validity for the ***,POMS,PPQ,and BSQ data were used to examine concurrent validity and predictability of SEI for Phlegm and BS in the college *** SEIQ scores and the clinicians' diagnoses of the outpatients were considered to define an optimum cut-off score for the ***:The 18-item SEIQ had satisfactory internal consistency(α = 0.905) and concurrent *** the construct validity test,four factors(chest-anxiety,fatigue-depression,working-family-troubles,and sleep-memory) were *** the receiver operator characteristic curve curve analysis,the sensitivity,specificity,and area under the curve of the SEIQ were 67.2%,72.1%,and 73%,*** optimum cut-off score was defined as nine *** scores were strongly predictive of Phlegm and BS(β = 0.862 and 0.673,respectively).CONCLUSION:Based on our results,we concluded that the SEIQ is a reliable and valid instrument for evaluating SEI,and is strongly predictive of Phlegm and BS.

Rhabdomyolysis following severe hypokalemia caused by familial hypokalemic periodic paralysis

Rhabdomyolysis continues to appear with increasing frequency and represents a medical emergency requiring rapid appropriate treatment. One of the unusual causes of nontraumatic rhabdomyolysis is hypokalemic periodic paralysis without secondary causes. Primary hypokalemic periodic paralysis is a rare genetic disease characterized by episodic attacks of muscle weakness due to decreases in serum potassium. A 30-year-old woman who had 3 episodic attacks of hypokalemic periodic paralysis was admitted in emergency room with sudden onset symmetrical muscle weakness. After several hours, she started to complain myalgia and severe ache in both calves without any changes. Laboratory test showed markedly elevated creatine phosphokinase, lactic dehydrogenase levels with hypokalemia, rhabdomyolysis resulting from hypokalemia was diagnosed. Here, we report an unusual case of rhabdomyolysis caused by severe hypokalemia, which was suggested a result of familial hypokalemic periodic paralysis.

Fabrication and Mechanical Properties of Sm2O3 Doped CeO2 Reinforced Ti3AlC2 Nano Composite

The fabrication process of Sm2O3 doped CeO2 reinforced Ti3AlC2 nano composites including the nano particle dispersion process by a hetero-coagulation process was developed using in-situ synthesis and densification process of Ti3AlC2. The effects of Sm2O2 doped CeO2 nano particles on mechanical properties of Ti3AlC2 were investigated. It was found that the presence of 20SDC nano particles in Ti3AlC2 was very effective to improve the mechanical properties of Ti3AlC2 without spoiling the unique characteristics of Ti3AlC2temary carbide.

Reliability and validity of tongue color analysis in the prediction of symptom patterns in terms of East Asian Medicine

OBJECTIVE:To examine whether color parameters of tongue inspection(TI)using a digital camera was reliable and valid,and to examine which color parameters serve as predictors of symptom patterns in terms of East Asian medicine(EAM).METHODS:Two hundred female subjects'tongue substances were photographed by a mega-pixel digital *** with the photographs,the subjects were asked to complete Yin deficiency,Phlegm pattern,and Cold-Heat pattern *** three sets of digital imaging software,each digital image was exposure-and white balance-corrected,and finally L*(luminance),a*(red-green balance),and b*(yellow-blue balance)values of the tongues *** examine intra-and inter-rater reliabilities and criterion validity of the color analysis method,three raters were asked to calculate color parameters for 20 digital image ***,four hierarchical regression models were ***:Color parameters showed good or excellent reliability(0.627-0.887 for intra-class correlation coefficients)and significant criterion validity(0.523-0.718 for Spearman's correlation).In the hierarchical regression models,age was a significant predictor of Yin deficiency(β=0.192),and b*value of the tip of the tongue was a determinant predictor of Yin deficiency,Phlegm,and Heat patterns(β=-0.212,-0.172,and-0.163).Luminance(L*)was predictive of Yin deficiency(β=-0.172)and Cold(β=0.173)***:Our results suggest that color analysis of the tongue using the L*a*b*system is reliable and valid,and that color parameters partially serve as symptom pattern predictors in EAM practice.

Cardiff acne disability index： cross-cultural translation in Korean and its relationship with clinical acne severity, pathological patterns, and general quality of life

OBJECTIVE: To culturally translate the cardiff acne disability index(CADI) into Korean, and to examine its relationship with clinical acne severity,pathological patterns, and general quality of life(Qo L).METHODS: The CADI was culturally and lin-guistically translated into Korean via translation,back-translation, and face validity test process. Two hundred and fifty-four Korean adolescents were asked to complete the Korean version of the CADI(K-CADI), the Phlegm Pattern, the Cold-Heat Pattern, and the Korean version of the General Health Questionnaires. A clinician estimated acne severity for the adolescents, using the Korean Acne Grading System. Finally, reliability and validity of the K-CADI was examined, and the relationships between acne severity, Phlegm, Cold, and Heat patterns, and Qo L level were examined via pathway ***: The K-CADI had satisfactory internal consistency(α = 0.827). The examination of construct validity indicated that the K-CADI had one factor(explaining 59.6% of the total variance). Pathway analysis showed satisfactory model fit(normal fit index = 0.960 and comparative fit index = 0.983),and acne-related Qo L was determinant to Phlegm,Heat, and Cold patterns(0.13-0.27 of β), and Phlegm and Heat patterns lowered one's Qo L level(0.17-0.34 of β).CONCLUSION: The K-CADI is a valid and reliable instrument. Phlegm and Heat patterns should be managed when treating acne since they have a moderating effect on general Qo L aggravation.

Remote induction of in situ hydrogelation in a deep tissue,using an alternating magnetic field and superparamagnetic nanoparticles

In situ hydrogelation systems, such as transdermal polymerization, allow for external control over the gelation processes in a minimally invasive way. Recently, a novel system consisting of near-infrared (NIR) light and plasmonic nanomaterials was demonstrated to cause in vivo transdermal gelation. However, NIR light is not sufficient for gelation induction in deep tissues owing to its limited penetration into tissues. To overcome this problem, here we developed an alternating magnetic field (AMF)-inducible hydrogelation system with superparamagnetic iron oxide nanoparticles (SPIONs), by which a deep-tissue–penetrating AMF can induce heat generation in the SPIONs and temperature elevation (≥ 43 °C), leading to initiation of thermal polymerization of poly(ethylene glycol) diacrylate. The feasibility of our AMF-inducible hydrogelation was successfully demonstrated using thick porcine muscle tissues (> 2 cm), whereas the NIR light–based hydrogelation system could induce gelation only in tissues thinner than several millimeters. Cell viability assays indicated cytocompatibility of the AMF-inducible hydrogelation for cell encapsulation and delivery. In vivo hydrogelation in rat muscle tissues further validated in situ hydrogelation in deep muscle tissues of a living animal. This AMF-inducible hydrogelation system may overcome the conventional problems of depth limitation and should extend the applicable area of on-demand injectable hydrogel systems for tissue engineering and drug delivery applications.

Improved thermal performance of a hydronic radiant panel heating system by the optimization of tube shapes
