Clinical outcome in patients with hepatocellular carcinoma after living-donor liver transplantation

AIM: To investigate risk factors for hepatocellular carcinoma (HCC) recurrence after living donor liver transplantation (LDLT) and efficacy of various criteria. METHODS: From October 2000 to November 2011, 233 adult patients underwent LDLT for HCC at our institution. After excluding nine postoperative mortality cases, we analyzed retrospectively 224 patients. To identify risk factors for recurrence, we evaluated recurrence, disease-free survival (DFS) rate, survival rate, and various other factors which are based on the characteristics of both the patient and tumor. Additionally, we developed our own criteria based on our data. Next, we compared our selection criteria with various tumor-grading scales, such as the Milan criteria, University of California, San Francisco (UCSF) criteria, TNM stage, Barcelona Clinic Liver Cancer (BCLC) stage and Cancer of the Liver Italian Program (CLIP) scoring system. The median follow up was 68 (6-139) ***: In 224 patients who received LDLT for HCC, 37 (16.5%) experienced tumor recurrence during the follow-up period. The 5-year DFS and overall survival rates after LDLT in all patients with HCC were 80.9% and 76.4%, respectively. On multivariate analysis, the tumor diameter {5 cm; P < 0.001; exponentiation of the B coefficient [Exp(B)], 11.89; 95%CI: 3.784-37.368} and alpha fetoprotein level [AFP, 100 ng/mL; P = 0.021; Exp(B), 2.892; 95%CI: 1.172-7.132] had significant influences on HCC recurrence after LDLT. Therefore, these two factors were included in our criteria. Based on these data, we set our selection criteria as a tumor diameter ≤ 5 cm and AFP ≤ 100 ng/mL. Within our new criteria (140/214, 65.4%), the 5-year DFS and overall survival rates were 88.6% and 81.8%, respectively. Our criteria (P = 0.001), Milan criteria (P = 0.009), and UCSF criteria (P = 0.001) showed a significant difference in DFS rate. The biosynthesis of host lipids and/or lipid droplets(LDs)has been studied extensively as a putative therapeutic target in diverse viral ***,directly targeting the LD lipolytic catabolism in virus-infected cells has not been widely ***,we show the linkage of the LD-associated lipase activation to the breakdown of LDs for the generation of free fatty acids(FFAs)at the late stage of diverse RNA viral infections,which represents a broad-spectrum antiviral *** of membrane transporter systems due to virus-induced cell injury results in intracellular malnutrition at the late stage of infection,thereby making the virus more dependent on the FFAs generated from LD storage for viral morphogenesis and as a source of *** replication of SARS-CoV-2 and influenza A virus(IAV),which is suppressed by the treatment with LD-associated lipases inhibitors,is rescued by supplementation with *** administration of lipase inhibitors,either individually or in a combination with virus-targeting drugs,protects mice from lethal IAV infection and mitigates severe lung lesions in SARS-CoV-2-infected ***,the lipase inhibitors significantly reduce proinflammatory cytokine levels in the lungs of SARS-CoV-2-and IAV-challenged animals,a cause of a cytokine storm important for the critical infection or mortality of COVID-19 and IAV *** conclusion,the results reveal that lipase-mediated intracellular LD lipolysis is commonly exploited to facilitate RNA virus replication and furthermore suggest that pharmacological inhibitors of LD-associated lipases could be used to curb current COVID-19-and future pandemic outbreaks of potentially troublesome RNA virus infection in humans.

Delamination in composite structures plays a major role in lowering structural strength and stiffness, consequently downgrading system integrity and reliability. The aim of this paper is to show that Lamb waves may be effectively generated using piezoelectric transducers embedded inside a composite plate for health monitoring application. A Lamb wave-based quantitative identification technique for delamination in VARTM composite structures was established. Propagation of Lamb waves in composite plates was evaluated using dynamic FEM analysis. Then, an experiment is carried out to verify the validity of the analytical study based on finite element models. The study was carried out to assess damages in composite plate by fusing information from multiple sensing paths of the embedded network. It was based on the Hilbert transform, signal correlation and probabilistic searching. The obtained results show that satisfactory detection of defects could be achieved by proposed method

Prediction of the indication criteria for endoscopic resection of early gastric cancer

This study estimates the coefficient of friction on the actual curling sheet with a pebble, without having to mount any devices on it. Existing studies have equipped the ice sheet with external devices to estimate the coefficient of friction. Another paper estimated friction coefficients, assuming that the pebble did not exist, because it was very difficult to estimate the coefficient of friction in the space where the pebble was located. In this study, it is possible to estimate the coefficient of friction of the actual curling sheet without fitting an external device onto the curling sheet. In addition, the friction coefficient estimation method attempted using image processing techniques may be most similar to the actual friction coefficient, reducing the outside influence, compared to the existing papers. It will also be an estimation method using image processing techniques, which form the basis of various friction coefficient screening methods.

AIM: To find risk factors of lymph node metastasis(LNM) in early gastric cancer(EGC) and to find proper endoscopic therapy indication in ***: We retrospectively reviewed the 2270 patients who underwent curative operation for EGC from January 2001 to December 2008. EGC was defined as malignant lesions that do not invade beyond the submucosal layer of the stomach wall irrespective of presence of lymph node ***: Among 2270 enrolled patients, LNM was observed in 217(9%) patients. LNM in intramucosal(M) cancer and submucosal(SM) cancer was detectedin 3 8( 2. 8 %, 3 8 / 1 3 4 0) patients and 1 7 9(19%, 179/930) patients, respectively. In univariate analysis, the risk factors for LNM in EGC were size of tumor, Lauren classification, ulcer, lymphatic invasion, vascular invasion, and depth of invasion. However, in multivariate analysis, size of tumor, lymphatic invasion, vascular invasion, and depth of invasion were risk factors for LNM in EGC. Size of tumor, lymphatic invasion, vascular invasion, and depth of invasion were risk factors for LNM in cases of intramucosal cancer and submucosal cancer. In particular, there was no lymph node metastasis in cases of well differentiated early gastric cancer below 1 cm in size without ulcer regardless of lymphovascular ***: Tumor size, perilymphatic-vascular invasion, and depth of invasion were risk factors for LNM in EGC. There was no LNM in EGC below 1 cmregardless risk factors.

In this work, enhancement of the light extraction efficiency of a 590 nm AIGaInP light-emitting diodes （LED） with a reflective top electrode （RTE） was investigated. A distributed Bragg reflector （DBR）, consisting of AIAs/AIGaAs pairs, grown on an AlGaInP structure was used as a reflector for a reflective top electrode. It was found that a higher output power was observed from the AIGalnP LED with a RTE than from a conventional one. In addition, it was noted that the improvement in the output power depends strongly on the reflectivity of the reflector and that it exhibits a more effective performance with low injection currents. The increase in the optical output power was attributed to the enhanced extraction efficiency caused by a reduction of light absorbed from the emission region to top electrode through the RTE.

Objective: De-qi refers to sensations generated by needle insertion into an acupuncture ***-qi sensation is believed to be associated with treatment effects of acupuncture with the increase of local blood flow, although some patients may feel these sensations *** study aimed to assess the association between the strength of de-qi sensation and local skin blood flow in healthy ***: Forty-five healthy subjects were randomized into real acupuncture(RA) group(n=15), Park-sham acupuncture(SA) group(n=15) and non-needling control(CO) group(n=15).A single acupuncture session was conducted on the right LI4 point with manual stimulation and 10-minute needle ***-qi sensation was assessed immediately after the removal of needling by the Southampton needle sensation *** skin blood flow recording within 2 cm in diameter of ipsilateral LI4 area was performed intermittently from just before the intervention to the end of the trial by the laser Doppler blood perfusion *** between the change of skin blood flow and strength of De-qi was ***: Median change of local skin blood flow from baseline to post-treatment was 48.85, 48.20 and 0.59 arbitrary unit(AU) in RA, SA and CO groups, *** total sensation score was 10, 3 and 0 of 51 points, *** significant correl ation between total sensation score and change of skin blood flow in the RA and SA groups was observed(0.105 9 of Spearman’s rho, P=0.57).Conclusion: The data suggest that there is no association between De-qi sensation and local skin blood flow after acupuncture in healthy *** role of De-qi sensation for physiological changes and clinical outcomes should be further investigated in future acupuncture trials.
