Spontaneous Infection of Ascites Fluid at the National and University Hospital Hubert Koutoukou Maga in Cotonou: Prevalence and Associated Factors

Background: Spontaneous ascites fluid infection (SAFI) is an extremely serious and frequently encountered complication in cirrhotic patients. We aimed to determine the prevalence of SAFI and the factors associated with it in the largest hospital in Cotonou. Methods: This was a retrospective descriptive and analytical study conducted from January 2013 to July 2019, at the National and University Hospital Hubert Koutoukou Maga (CNHU-HKM) in Cotonou, Benin. All patients followed in the University Clinic of Hepato-Gastroenterology and diagnosed with SAFI were included. Results: Eighty-two patients were included, predominantly males (69.5%), with a mean age of 51.5 ± 14.5 years. Among them, 32 had SAFI, i.e., a prevalence of 39%. Of the 32 cases of SAFI, the culture of ascites fluid was positive in 6 cases (18.7%). The most frequent germ found in SAFI was Escherichia coli (5 patients, 83.3%). The factors associated with SAFI in this study were: abdominal pain (p = 0.004), increased bilirubinemia (p = 0.009), decreased prothrombin level 20 (p = 0.001). Conclusion: SAFI was common in cirrhotic patients in the department. Certain clinical and paraclinical factors were associated with it, as was the severity of cirrhosis. Early diagnosis and aetiological management of cirrhosis could reduce its frequency.

Epidemiological, Clinical Progress Aspects of Congenital Heart Disease with Neonatal Revelation at the Mother-Child Hospital of Bingerville (HME) Concerning 98 Cases from January 2021 to December 2022 (Côte d’Ivoire)

Introduction The distribution of congenital heart disease (CHD) in sub-Saharan Africa is highly imprecise and varies from one region to another due to the inequality of diagnostic facilities. The aim of this study was to determine the in-hospital prevalence of congenital heart disease in children at the Mother-Child hospital of Bingerville (HME) by specifying the diagnostic, therapeutic and evolutionary aspects. Materials and methods We conducted a retrospective, descriptive, cross-sectional study at HME of Bingerville from January 2021 to December 2022. All newborns with congenital heart disease confirmed by echocardiography were included in the study. Results Of 656 admissions to the neonatology department over the study period, congenital heart disease accounted for 14.9% (98/656) of cases. In our series, 76.7% were diagnosed before the 1st week of life, with a mean chronological age of 5.18 days and extremes of 0 and 46 days. There were as many male patients (50%) as female (50%), i.e. a sex ratio of 1. These newborns were premature in 60.2% of cases, with a mean and median gestational age of 34 weeks’ amenorrhea. Most were left-right shunts (90.8%). Persistent ductus arteriosus (PDA) (48.9%) predominated, followed by atrial septal defect (38.7%), ventricular septal defect (13.3%), common trunk artery (CTA) (3.1%) and open septal pulmonary atresia (OSPA) (1%) as the primary cyanogenic heart disease. Pulmonary arterial hypertension (PAH) (50%) was primary in 38.8% and secondary (61.2%). The mortality rate was 30.6%, and all CTA patients died (100%), with a significant statistical relationship (p = 0.027). Progression under treatment was marked by clinical stabilization (68/98) in 69.4% of cases. Conclusion: Congenital heart disease is relatively common at the Bingerville HME. Access to echocardiography should be facilitated in neonatology departments for rapid diagnosis and optimal management of congenital heart disease in newborns.

Characterization and Assessment of the Quality of the Water and Sediments of the Islands (Moudou and Birguime) of Lake Fitri in Chad and Confirmation of the Strains Isolated by the PCR Method

The problem of access to quality water is a major challenge, as it has a major impact on the socio-economic conditions of people in developing countries. The water from the islands of Lake Fitri is one of the main sources of drinking water for the population. The aim of this study is to characterise and assess the quality of the water and sediment from the islands (Moudou and Birguime) of Lake Fitri, and to confirm the strains isolated using the PCR method. A total of fifty (50) samples of water and fifty (50) sediments from the islands of Lake Fitri were analyzed. Standard methods of microbiological and biochemical analysis of water and sediments were used. Isolated Enterobacteriaceae strains were characterized by API 20 E and API Staph galleries and Salmonella was confirmed by PCR method. Antibiotic resistance was determined using a technique recommended by the antibiogram committee of the French microbiology society (CA-SFM, 2019). The microbiological results for the water showed an abundance of total aerobic mesophilic flora (TAMF) (4.31 × 106 ± 8.05 × 105 and 5.29 × 106 ± 2.55 × 105) on the Birguime and Moudou islands successively. The microbiological results for the sediment from Birguime and Moudou islands showed an abundance of thermotolerant coliforms (E. coli) (2.05 × 105 ± 5.43 × 104 and 2.27 × 105 ± 3.49 × 104) alternately. The results obtained after incubation of the biochemical tests by the API 20E, API Staph gallery and their numerical profile proposed by the Api software confirmed the contamination. The antibiogram results showed the emergence of certain resistances to the antibiotics Tobramycin, Flucytosine, Amikacin and Teicoplani. The PCR results for Salmonella spp strains were confirmed. As a result, strict monitoring of the water on the various islands of Lake Fitri must be carried out throughout the annual cycle, by specialized personnel, to ensure proper bio-monitoring of these ecosystems.

Crop and livestock production is critical to food security in The Gambia. Over the years, the country has experienced a reduced yield due to perceived climate change events with limited studies on how climate change and pollution affect crop production. This study assesses farmers’ knowledge and perceptions of the effects of climate variability and pollution on crop production and their varying adaptation strategies in The Gambia. Both quantitative and qualitative methods were used in this study. The sample size for quantitative data collection was calculated as 432 while the qualitative data involves both the focus group discussions and key informant interviews. The focus group discussions comprised two districts in each of the six agricultural regions and two farming communities engaged in crop production were chosen from each district. Furthermore, eight key informant interviews from relevant institutions were conducted. The study shows that The Gambia is highly vulnerable to extreme climatic events. Although most farmers opined that agricultural land contamination emanates from farm runoff and indiscriminate waste dumping, they had little knowledge of heavy metal pollution and bioremediation. The results showed that farmers experienced constraints such as inadequate access to credit, water, and irrigation facilities, insufficient access to efficient inputs, salt intrusion, etc. which threatened food security. The study concludes that crop farmers acknowledged the existence and impacts of climate change, and therefore recommend the availability and affordability of climate change resilient crops and promote variability awareness campaigns to address climate change impacts in The Gambia.

Epidemiological Aspects of Stillbirth and Neonatal Deaths in the Delivery Room at the Libreville Mother-Child University Hospital from 2019 to 2022

Introduction: Stillbirths are estimated at 2 million each year, of which more than 40% occur during labour. Our objective was to study the epidemiological aspects of stillbirth and neonatal deaths in the delivery room in our health facility. Patients and methods: Prospective, descriptive and analytical study, conducted at the Jeanne Ebori Foundation Mother-Child University Hospital over 4 years (January 2019-December 2022). All neonatal deaths in the delivery room or foetal death in utero, were included. Results: Among the 18,346 deliveries performed, 512 newborns were declared dead in the delivery room (27.9‰ live births), divided into in utero foetal death (19.0‰) and immediate neonatal death (8.9‰). The mean age was 34.3 weeks of amenorrhea. The rate of preterm birth was 60.4%. The sex ratio was 1.1. The average weight was 2186.6. The main causes were vascular (46.1%), foetal (20.2%), adnexal (17.1%) and asphyxia per partum (16.6%). Foetal causes were more likely to result in IUFD than other causes (OR = 6.4 [2.4 - 15.7], p < 0.001). After birth, partum asphyxia was more likely to lead to death before 15 minutes of life than other causes (OR = 11 [6.1 - 18.9], p Conclusion: The causes of stillbirth and early neonatal mortality are dominated by maternal vascular pathologies. However, the proportion of childbirth-related causes remains worrying. Better monitoring of pregnancy and labour will minimize this prevalence in our hospital.

Cancer is one of the deadliest diseases in developing countries. In recent years, natural plant-based compounds have been used in the search for drugs to combat numerous diseases, including cancer. In this study, we evaluate the cytotoxic properties of paanfo tiben 1 and paanfo tiben 2, two traditional herbal formulations from Burkina Faso used in the treatment of cancer in Burkina Faso. To this end, the recipes were infused and freeze-dried. The dry extracts obtained were used to determine total phenolics and flavonoids content, assess antioxidant activity using the DPPH, ABTS and FRAP methods, evaluate anti-inflammatory properties by inhibiting 15-LOX, COX 1 and 2, and assess cytotoxic activity on HeLa cervical cancer and HePG2 liver cancer cell lines using the MTT test. The paanfo tiben 1 recipe showed the highest levels of total phenolics and flavonoids, as well as the best antioxidant activities, with IC50 values of 21.020 ± 0.6 µg/ml and 22.94 ± 0.57 µg/ml for DPPH and ABTS, and 165.15 mM EAA/mg dry extract for FRAP. It also exhibited the best cytotoxic activity with IC50 values of 112.02 ± 0.025 µg/ml on HeLa cells and 80.67 ± 6.08 µg/ml on HepG2 cells. On the other hand, paanfo tiben 2 exhibited the best anti-inflammatory activities through inhibition of 15-LOX and COX 1, with inhibition percentages at 100 µg/ml of 32.523% and 24.717 % respectively. These results could justify the traditional use of these two recipes by traditional health practitioners in the treatment of cancer sufferers in Burkina Faso.

Recalling some of the most important events and persons during his education and career, the author sketches his growth from a young engineer, educated in the sanctuary of solid state reactions, to an involved fully devoted scientific career for the study of fluids in the deep Earth. Most important in this respect was the discovery of C02 inclusions in granulites, which triggered years of discussion on fluid- absent or fluid-assisted granulite metamorphism. To some extent, this debate is a continuation of the former granite controversy, but it shows also how the famous battle of ＂soaks against pontiffs＂ could have been easily avoided.

Assessing human sperm morphology： top models, underdogs or biometrics？

The assessment of the percentage of spermatozoa having an ＇ideal＇ morphology using so-called strict method is the method recommended in the latest edition of the World Health Organization （WHO） laboratory manual for semen analysis. This recommendation is a result of the statistical association between ＇ideal＇ sperm morphology and fertility, and of the current general belief that sperm morphology assessment should be used primarily as a fertility tool. The notion of an ＇ideal＇ sperm morphology has persisted despite the very low percentage of such spermatozoa in the semen of fertile men, a subject of intense controversy. The detailed categorization of each abnormal spermatozoon has thus, for a long time, been considered optional and partially redundant, an idea which is reflected in the earlier editions of the WHO manual. However, several recent studies have shown the importance of carefully assessing abnormal sperm morphology for use in the diagnosis ＆infertility, to determine fertility prognosis, and for basic or public health studies. One approach, which combines videomicroscopy and computer vision, and is the only approach able to assess the continuum of sperm biometrics, has been used successfully in several recent clinical, basic and toxicology studies. In summary, the visual assessment of detailed sperm morphology--including the categorization of anomalies allowing arithmetically derived indices of teratozoospermia--and the more modern computer-based approaches, although often considered to be redundant, are in fact complementary. The choice of the most appropriate method depends on the field of investigation （clinical, research, toxicology） and the problem being addressed. Each approach has advantages as well as certain limitations, which will be discussed briefly herein.

End-of-Pregnancy Monitoring at CHUMEFJE through the Functional Investigation Centre (FIC) in 2022

Introduction: The end of pregnancy is a high-risk period for both mother and foetus. Rigorous monitoring can prevent complications before delivery. Materials and Method: We conducted a descriptive cross-sectional observational study. It took place in the delivery room of the Teacher hospital Mother and Child of Jeanne Ebori Fondation from the 01 October 2020 to 01 October 2021. All patients followed at the Functional Investigation Centre (FIC) of the CHUMEFJE and who gave birth in that same hospital were included. Data were collected on the basis of pregnancy diaries, the fic register and delivery room registers. They were analysed using SPSS Statistical Software. Results: During the period of our study, 4086 parturients arrived in the delivery room. Of these, 150 were followed up at the FIC, giving a prevalence of 3.7%. The majority of parturients (48%) had only one prenatal contact. 6 (4%) patients underwent pelvic scans, and 4 (2.6%) presented with a narrowed pelvis. A vaginal delivery was performed in 80% of cases, and of the caesarean sections, 9 (30%) could be scheduled. The maternal prognosis was marred by one post-partum complication of hypertension, and newborns with poor adaptation to life outside the womb accounted for 3.3% of cases. Conclusion: The Functional Investigation Centre makes it possible to detect anomalies at the end of pregnancy with a view to better planning of delivery.

A unified plasticity methodology for rate-and temperature-sensitive alloys exhibiting a non-linear kinematic hardening behavior

In this paper, a novel unified plasticity methodology is proposed to allow the coupling of rate-and temperature-sensitivity of engineering alloys as well as the non-linear kinematic hardening behavior often observed during cyclic loading. The proposed methodology is general in the sense that an arbitrary constitutive model may be chosen for the viscoplastic part, as well as the cyclic part. We adapt our model with a physically-motivated viscoplasticity flow rule and a nonlinear kinematic hardening model. In contrast with other unified plasticity models, the simplified theory involves few material parameters that can be readily calibrated from standard mechanical tests. The capabilities of the proposed theory are demonstrated for a hot rolled annealed 304 L stainless steel supplied by Vimetal Peckover. The model is tested with stress–strain curves obtained from standard tensile and cyclic uniaxial tests at various strain amplitudes and strain-rates, and good accuracy of the response is obtained for strains up to 15%, within a temperature range of 293–673 K. We note that the cyclic plasticity model in our adapted theory can be readily enhanced with ratchetting, mean stress relaxation, strain amplitude history, Masing effects or other complex capabilities.
