First Trimester screening for Preeclampsia:An Asian Perspective

Preeclampsia(PE)is one of the leading causes of maternal and perinatal morbidity and mortality *** disorder has profound short-term and long-term impacts on both the affected woman’s and her child’s ***-onset PE requiring preterm delivery(preterm PE)is of particular importance because it is associated with a higher risk of adverse pregnancy outcomes than term *** trimester screening model developed by the Fetal Medicine Foundation(FMF),which uses Bayes-theorem to combine maternal characteristics and medical history together with measurements of mean arterial pressure,uterine artery pulsatility index,and serum placental growth factor,has been proven to be effective and have superior screening performance to that of traditional risk factor-based approach for the prediction of *** of high risk pregnant women for preterm PE and giving aspirin prophylaxis before 16^(th)week of gestation would reduce the incidence of preterm *** Asia,although the prevalence of PE is slightly lower than the global estimation,early screening and prevention of this life-threatening condition is still *** FMF Bayestheorem based screening method has been validated in a large-scale prospective Asia-wide study and revealed that the first trimester triple test achieves the highest detection rate,compared with the traditional risk factor-based approaches,and that the screening performance is comparable to the published data from the FMF in East Asian ***,in order to achieve optimal screening performance,the key is to establish standardized methods for biomarker measurements and regular biomarker quality assessment,as each biomarker is susceptible to inaccurate measurement,thus affecting performance of ***,it is of great importance to emphasize that the optimal preventive effect of aspirin on preterm PE is clearly associated with good compliance to *** conclusion,global implementation of an effective first trimest

The cellular fatty acids from a total of 62 strains of Torulopsis glabrata (T. glabrata), saccharomyces cerevisiae (s. cerevisiae), Rhodotorula rubra (R. rubra), candida krusei (c. krusei), candida albicans (c. albicans) and candida tropicalis (c. tropicalis) were examined by capillary gas chromatography. On the basis of fatty acid composition, all strains could be differentiated as to species. These results indicate that capillary gas chromatographic analysis of cellular fatty acids is likely to be useful for rapid identification or grouping of newer isolates of yeast species.

Purpose: To study the safety and efficacy of immediate argon laser peripheral iridoplasty (ALPI) as initial treatment of acute phacomorphic angle-closure (phacomorphic glaucoma) before cataract extraction. Methods: In all, 10 consecutive patients with acute phacomorphic angle-closure and intraocular pressure (IOP) ≥ 40 mmHg were recruited into the study. Each patient received topical atropine (1% ) and timolol (0.5% ), and immediate ALPI as initial treatment The IOP at 15, 30, 60, and 120 min, and then 1 day, after ALPI were documented by applanation tonometry. systemic IOP-lowering drugs were only started if IOP remained above 40 mmHg at 2 h after ALPI. cataract extraction was subsequently performed as definitive treatment. Results: In total, 10 patients (five male, five female), with a mean age± sD of 73.1± 10.3 years were recruited. Mean duration of symptomatic attack was 128± 232 h. After ALPI, the mean IOP was reduced from 56.1± 12.5 to 45.3± 14.5 mmHg at 15 min, 37.6± 7.5 mmHg at 30 min, 34.2± 9.7 mmHg at 60 min, 25.5± 8.7 mmHg at 120 min, and 13.6 ± 4.2 mmHg at 1 day. In one patient, systemic acetazolamide was administered, because the IOP remained above 40 mmHg at 2 h after ALPI. All 10 patients had uncomplicated cataract extraction performed within 4 days after ALPI. No complications from the laser procedure were encountered. conclusion: ImmediateALPI, replacing systemic antiglaucomatous medications, appeared to be safe and effective as first-line treatment of acute phacomorphic angle-closure.

International validation of the chinese University Prognostic Index for staging of hepatocellular carcinoma: a joint United Kingdom and Hong Kong study

The outcome of hepatocellular carcinoma(Hcc) patients significantly differs between western and eastern population centers. Our group previously developed and validated the chinese University Prognostic Index(cUPI) for the prognostication of Hcc among the Asian Hcc patient population. In the current study, we aimed to validate the cUPI using an international cohort of patients with Hcc and to compare the cUPI to two widely used staging systems, the Barcelona clinic Liver cancer(BcLc) classification and the cancer of the Liver Italian Program(cLIP). To accomplish this goal, two cohorts of patients were enrolled in the United Kingdom(UK; n = 567; 2006-2011) and Hong Kong(HK; n = 517; 2007-2012). The baseline clinical data were recorded. The performances of the cUPI, BcLc, and cLIP were compared in terms of a concordance index(c-index) and were evaluated in subgroups of patients according to treatment intent. The results revealed that the median follow-up durations of the UK and HK cohorts were 27.9 and 29.8 months, respectively. The median overall survival of the UK and HK cohorts were 22.9 and 8.6 months, respectively. The cUPI stratified the patients in both cohorts into three risk subgroups corresponding to distinct outcomes. The median overall survival of the cUPI low-, intermediate-, and high-risk subgroups were 3.15, 1.24, and 0.29 years, respectively, in the UK cohort and were 2.07, 0.32, and 0.10 years, respectively, in the HK cohort. For the patients who underwent curative treatment, the prognostic performance did not differ between the three staging systems, and all were suboptimal. For those who underwent palliative treatment, the cUPI displayed the highest c-index, indicating that this staging system was the most informative for both cohorts. In conclusion, the cUPI is applicable to both western and eastern Hcc patient populations.
