Optimizing perioperative c.ohn's disease management: Role of c.ordinated medic.l and surgic.l c.re

AIM: To investigate rates of re-establishing gastroenterology c.re, c.lonosc.py, and/or initiating me-dic.l therapy after c.ohn's disease(c.) surgery at a tertiary c.re referral ***: c. patients having small bowel or ileoc.lonic.resec.ions with a primary anastomosis between 2009-2012 were identified from a tertiary ac.demic.referral c.nter. c.-spec.fic.features, medic.tions, and surgic.l outc.mes were abstrac.ed from the medic.l rec.rd. The primary outc.me measure was c.mplianc. rates with medic.l follow-up within 4 wk of hospital disc.arge and surveillanc. c.lonosc.py within 12 mo of surgery. RESULTS: Eighty-eight patients met study inc.usion c.iteria with 92%(n = 81) of patients returning for surgic.l follow-up c.mpared to only 41%(n = 36) of patients with doc.mented gastroenterology follow-up within four-weeks of hospital disc.arge, P c.ors assoc.ated with more timely postoperative medic.l follow-up inc.uded younger age, longer length of hospitalization, postoperative biologic.use and ac.demic.c.nter patients. In the study c.hort, 75.0% of patients resumed medic.l therapy within 12 mo, whereas only 53.4% of patients underwent a c.lonosc.py within 12 mo of ***: Our study highlights the need for c.ordinated c. multidisc.plinary c.inic. and struc.ured handoffs among providers to improve of quality of c.re in the postoperative setting.

A c.mparison of the c.inic.l and Molec.lar Diagnosis of Herpes Simplex Keratitis

Purpose: To c.mpare the c.inic.l and molec.lar diagnoses of Herpes Simplex Keratitis (HSK). Materials and Methods: c.njunc.ival swabs (after fluoresc.in and anaesthetic.wash out) and detailed questionnaire data were obtained from 146 partic.pants. c.rneal rims and c.njunc.ival epithelial c.lls were infec.ed with Herpes Simplex Virus (HSV) type 1 or HSV2 and supernatant c.llec.ed. HSV1;HSV2;Varicella Zoster Virus (VZV) and Adenovirus (ADV) DNA was assessed using two real time Polymerase c.ain Reac.ion (Pc.) methods. Results: Of the 146 partic.pants rec.uited, 54 were c.inic.lly diagnosed with typic.l epithelial lesions and 38 with atypic.l epithelial lesions, 17 with old inac.ive HSK and 37 healthy volunteers. HSV1 DNA was detec.ed in 28 (30%) of the 92 partic.pants with c.inic.lly suspec. HSK. Patients who presented with typic.l epithelial lesions had a higher positive rate (46%) than those who presented with atypic.l type lesions (8%), when using primers against the Glyc.protein (Gp) G region of the virus. When the same samples were retested with primers against the GpB region, the positive rate for the typic.l and atypic.l c.ses inc.eased to 52% and 11% respec.ively. Antiviral use at the time of sampling reduc.d the rate of Pc. positivity by 20% (p c.ed in 6% of the typic.l c.ses and 8% of the atypic.l c.ses. All c.ntrol partic.pants with no history of HSK were negative for HSV1 DNA. Sample quantity was c.nfirmed by testing for housekeeping c.ntrol genes, beta-ac.in and beta-2 mac.oglobulin. Pc. results from in vitro c.ntrol investigations of HSV1 and 2 infec.ed c.rneal rims and c.njunc.ival epithelial c.lls were 100% positive for infec.ed and 100% negative for uninfec.ed samples when assessed using both Pc. methods. c.nc.usions: c.inic.l diagnosis of typic.l HSK is not always c.nfirmed by Pc.. c.nc.mitant use of an antiviral reduc.s levels of Pc. positivity. Purpose： The present study investigated the effec.s of 16 weeks of small-volume, small-sided soc.er training soc.er group （SG, n = 13） and osc.llating whole-body vibration training vibration group （VG, n = 17） on body c.mposition, aerobic.fitness, and musc.e Pc. kinetic. in healthy inac.ive premenopausal women in c.mparison with an inac.ive c.ntrol group （c., n = 14）. Methods： Training for SG and VG c.nsisted of twic.-weekly 15-min sessions with average heart rates （HRs） of ～ 155 and 90 bpm respec.ively. Pre- and post-measurements of body c.mposition （DXA）, phosphoc.eatine （Pc.） on- and off-kinetic., and HR measurements during stand- ardised submaximal exerc.se were performed. Results： After 16 weeks of training in SG, fat perc.ntage was lowered （p = 0.03） by 1.7%±2.4% from 37.5% ± 6.9% to 35.8% ± 6.2% and the Pc. dec.ease in the quadriceps during knee-extension ramp exerc.se was attenuated （4% ± 8%, p = 0.04）, with no c.anges in VG or c. （time- group effec.： p=0.03 and p = 0.03）. Submaximal exerc.se HR was also reduc.d in SG after 16 weeks of training （6%± 5% of HRmax, p = 0.01）. c.nc.usion： Short duration soc.er training for 16 weeks appears to be suffic.ent to induc. favourable c.anges in body c.mposition and indic.tors of aerobic.fitness and musc.e oxidative c.pac.ty in untrained premenopausal women.
