The systemic processes involved in the manifestation of life・threatening COVId-19 and in disease recovery are still incompletely understood,despite investigations focusing on the dysregulation of immune responses after SARS-CoV-2 *** define hallmarks of severe COVId-19 in acute disease(n=58)and in disease recovery in con valesce nt patie nts(n=28)from Han nover Medical School,we used flow cytometry and proteomics data with unsupervised clustering *** our observational study,we combined analyses of immune cells and cytokine/chemokine networks with endothelial activation and *** patients displayed an altered immune signature with prolonged lymphopenia but the expansion of granulocytes and plasmablasts along with activated and terminally differentiated T and NK cells and high levels of SARS-CoV-2-specific *** core signature of seven plasma proteins revealed a highly inflammatory microenvironment in addition to endothelial injury in severe *** within this sign ature were associated with either disease progression or *** summary,our data suggest that besides a strong inflammatory response,severe COVId-19 is driven by endothelial activation and barrier disruption,whereby recovery depends on the regeneration of the endothelial integrity.

Purpose As a prototype of deep Underground Neutrino Experiment far detector,ProtodUNE single-phase Liquid Argon(LAr)Time Projection Chamber(TPC)sits in H4 beam line at CERN to characterize detector *** consists of 6 full-size Anode Plane Assemblies(APAs)plus 18 Cathode Plane Assembly(CPA)modules to form two 3.6-m drift regions with a total of 15,360 readout *** achieve a good signal-to-noise ratio with this noble liquid detector(770ton LAr),cold electronics developed at BNL for extremely low temperature(77K–89K)operation is an optimal solution,which decouples the electrode and cryostat design from the readout *** With CMOS front end ASICs integrated with the TPC electrodes,the electronic noise is independent of the fiducial volume and much lower than with readout electronics at room *** addition,signal digitization and multiplexing to high-speed links inside the cryostat result in large reduction in the quantity of cables and the number of cryostat penetrations,giving the designers of both the TPC and the cryostat the freedom to choose the optimum *** and conclusions By April 2018,we have successfully instrumented 6 APAs with cold electronics,and the results of the integration test before the final installation in the cryostat look very promising.

Background: Propofol is often the drug of choice for ambulatory orthopedic cases. However, propofol can be associated with apnea or other events requiring airway interventions. dexmedetomidine (dex) has the unique pharmacologic profile of providing sedation without respiratory depression. This is particularly relevant in patients with morbid obesity and/or challenging airways. The hypotheses were: 1) Propofol would cause more apnea or require more airway manipulations than dexmedetomidine;2) Propofol would have shorter post anesthesia unit recovery times;and 3) dexmedetomidine would be associated with more bradycardia and hypotension. Methods: After IRB approval, 50 patients were randomized to receive either propofol or dex for Total Intravenous anesthesia (TIVA) after interscalene brachial plexus block was performed preoperatively under ultrasound guidance. The main end points that we measured where: airway interventions, intra-operative bradycardia, hypotension, and PACU length of stay. Results: There were more airway interventions in the propofol group compared to the dex group. Additionally, the dex group had significantly longer PACU stays. Conclusion: We would recommend that dex should be preferentially considered for patients predisposed to airway obstruction;however, the standard use of dex over Propofol needed to reconsider since the use of dex as the agent for TIVA was associated with longer PACU stays.

The hexanucleotide repeat mutation in the intron-1 of the chromosome 9 open reading frame (C9orf72) is a frequent cause of amyotrophic lateral sclerosis (ALS) and frontotemporal dementia (FTd). Altered RNA folding plays a role in ALS pathogenesis in two ways: non-ATG translation of the repeat can lead to aggregates of the known C9orf72 specific dipeptide polymer, whereas the repeat also can form neurotoxic RNA inclusions that dose-responsively kill motor neurons. We report the presence of a homology in the 5’untranslated region (UTR) of the messenger RNA encoding C9orf72 with the iron responsive elements (IRE) that control expression of iron-associated transcripts and predict that this RNA structure may iron-dependently regulate C9orf72 translation. We previously report altered serum ferritin levels track with severity of ALS in patients. Here, we conduct bioinformatics analyses to determine the secondary structure of the 5’UTR in C9orf72 mRNA and find it aligned with IREs in the human mitochondrial cis-aconitase and L and H-ferritin transcripts. Comparison of the role of RNA repeats in Friedriech’s ataxia and fragile X mental retardation suggests the utility of RNA based therapies for treatment of ALS. Antisense oligonucleotides (ASO) have been reported to therapeutically target these GGGGCC repeats. At the same time, because the function of C9orf72 is unknown, knockdown strategies carry some risk of inducing or compounding haploinsufficiency. We propose, for consideration, an approach that may enhance its therapeutic dynamic range by increasing the 5’UTR driven translation of C9orf72 protein to compensate for any potential ALS-specific or ASO-induced haploinsufficieny.

dosimetric Analysis of Prone Breast Treatment in Tomotherapy and Conventional Linear Accelerator

Purpose: To evaluate planning quality and dosimetric differences of clinically deliverable 3d conformal plans generated from Tomotherapy with TomodirectTM (Td) and conventional field-in-field approach in prone breast treatment. Materials and methods: Total of twelve randomly selected early stage left breast patients who went through lumpectomy and were previously treated on traditional Linear Accelerator (LINAC) have been re-planned and tested on Tomotherapy Tomodirect module. Baseline prescription dose was chosen at 50.4 Gy (1.8 Gy × 28 fractions) to cover ≥95% of PTV for planning criteria with other critical structure dose constraints in the thoracic region. Planning outcomes such as d95 (95% of volume of PTV receiving the prescribed dose), d5 and d1, heart, both lungs as well as the contralateral breast were simultaneously evaluated. Conformity of the prescription isodose/volume to PTV was evaluated as conformity index (CI) and dose uniformity was also evaluated with homogeneity index (HI) in the same study series. All outcome parameters were analyzed and summarized to evaluate dosimetric impact of planning qualities between these two planning platforms. Results: The planning results indicate that CI, HI, d95, d5 and d1 of PTV, critical structures such as heart, ipsilateral and contralateral lungs as well as contralateral breast doses were comparable but with better overall statistical end points from Td plans. The d95, d5 and d1 of PTV for Td plans were superior in dosimetric analysis and more uniform than those plans generated from PinnacleTM field-in-field planning technique. Overall, Td plans have superior planning quality than the conventional method does, with straightforward and automated planning process once the beam delivery parameters were established. 2018年4月《传播学刊》(Journal of Communication)将出版"新酵母专刊(Ferments of the Field)"回顾传播学近年发展,展望未来研究方向。此举继承35年前Jo C首次"酵母专刊"之精神,不但讨论各领域内部已取得的成果,更强调学术发展中的问题,及学科在外部社会发展中扮演的角色。本文作者与福克斯(Christian Fuchs)共同编辑此期刊物。专刊最终收入论文20篇,其中约一半来自广义批判学派,一半来自媒介效果、政治传播及组织研究学派。本文回顾酵母专刊历史,讨论今次新酵母专刊的编辑过程,简介专刊内容,重点讨论批判学派的脉络、它与主流传播研究的关系、新唯物主义转向、全球范围的传播研究、数码媒介环境带来的挑战、传播实践与有机的传播研究知识分子等问题。
