Comparison of cardioprotective effects of mibefradil and ramipril in stroke-prone spontaneously hypertensive rats

AIM: To elucidate the cardioprotective effects of T-type calcium channel blocker mibefradil and compare with that of the angiotensin-converting enzyme inhibitor ramipril in a stroke-prone spontaneously hypertensive rats (SHR- SP) model of congestive heart failure (CHF) after myocardial infarction (MI). METHOdS: SHR-SP rats were subjected to permanent ligation of the left anterior decending coronary artery. Treatment with mibefradil (10 mg·kg-1·d-1), ramipril (10 mg·kg-1·d-1), or placebo was initiated 4 weeks before surgery and continued up to 6 weeks after induction of MI. Sham-operated rats served as controls. RESULTS: In placebo-treated MI rats, six weeks after MI, left ventricular circumference, inner diameter, and left ventricular end-diastolic pressure (LVEdP) were increased, whereas mean arterial blood pressure (MAP) and maximum rate of rise of left ventricular pressure (dp/ dtmax) were decreased compared with sham-operated controls (Pd MI rats, heart weight, heart weight to body weight ratio and interstitial collagen content were reduced (PdP was slightly decreased (P>05), and dp/dtmax was improved (Pd with placebo-treated MI rats. In contrast, in mibefradil-treated MI rats, heart weight, heart weight to body weight ratio were slightly but not signifi- cantly reduced, LVEdP was slightly elevated compared with placebo-treated MI rats, and was elevated (Pd with ramipril-treated MI rats, although interstitial collagen content were reduced (Pd with placebo-treated MI ***: Chronic treatment with ramipril diminishes cardiac remodeling of heart failure after MI to a greater extent than mibefradil. Moreover, 6 weeks after MI, mibefradil treatment results in a slight rise in LVEdP compared with placebo-treated rats. Outcomes of combination MRI-targeted and transperineal template biopsy in restaging low-risk prostate cancer for active surveillance

Objective:Active surveillance(AS)offers a strategy to reduce overtreatment and now is a widely accepted treatment option for low-risk prostate *** ideal tool for risk-stratification would detect aggressive cancers and exclude such men from taking up AS in the first *** evaluate if a combination of transperineal template biopsy with magnetic resonance imaging(MRI)-targeted biopsy identifies significant prostate cancer amongst men initially diagnosed with low-risk prostate ***:This prospective,single-blinded study included men with low-risk prostate cancer(d’Amico’s Criteria)diagnosed on conventional transrectal ultrasound-guided *** first underwent multiparametric MRI of the prostate6 weeks after initial *** suspicious lesion is mapped and assigned a Prostate Imaging Reporting and data System(PIRAdS)*** biopsy is first performed with the surgeon blinded to MRI findings followed by MRI-targeted biopsy using a robotic transperineal biopsy ***:The age of the 19 men included is 65.4±4.9 years(mean±Sd).Prostate specific antigen(PSA)at diagnosis and at the time of transperineal biopsy were comparable(7.3±1.7 ng/mL and 7.0±1.8 ng/mL,p Z 0.67),so were prostate volumes(34.2±8.9 mL and 32.1±13.4 mL,p Z 0.28).MRI-targeted biopsy had a higher percentage of cancer detection per core compared to template biopsy(11.7%vs.6.5%,p Z 0.02),this was more than 3 times superior for Gleason 7 disease(5.9%vs.1.6%,pd significant disease with MRI-targeted biopsy *** of 19 patients(15.8%)had significant disease with template biopsy *** combination,both techniques upclassified five patients(26.3%),all of whom underwent radical *** mount histology confirmed tumour location and *** six patients with PIRAdS 5 lesions had cancer detected(66.6%significant disease).Conclusion:A combination of MRI-targeted and template biopsy may optimally risk-classi

dosimetric Comparisons of Lung SBRT with Multiple Metastases by Two Advanced Planning Systems

Purpose: To evaluate planning quality of Stereotactic body Radiotherapy (SBRT) with multiple lungmetastases generated by the Pinnacle and Tomotherapy planning systems, respectively. Methods and Materials: Nine randomly selected patients diagnosed with non-small cell lung carcinoma with multiple lesions were planned with Philips Pinnacle (version 9.2, Fitchburg, WI) and Tomotherapy (version 4.2, Madison, WI), respectively. Both coplanar and non-coplanar IMRT plans were generated on Pinnacle system. A total dose of 60 Gy was prescribed to cover 95% of Planning Target Volume (PTV) in 3 fractions based on the RTOG0236 protocol prescription [1]. All plans with single isocenter setting were used for multiple lesions planning. A set of nine static beams were used for Pinnacle plansusing direct Machine Parameters Optimization (dMPO) algorithm of RTOT0236 dose constraints. Planning outcomes such as minimum and mean doses, V95, d95 (95% of target volume receivesprescription dose), d5, and d1 to PTV, maximum dose to heart, esophagus, cord, trachea, brachial plexus, rib, chest wall, and liver, mean dose toliver, total lung, right and left lung, volume of chest wall receives 30 Gy, volume of lungs receives 5 Gy and 20 Gy (V5 and V20), conformity index (CI) and heterogeneity index (HI) were all reported for evaluation. Results: Mean volume of PTV was 37.77 ± 23.4 cm3. d95 of PTV with Tomotherapy, coplanar, non-coplanar plan was 60.2 ± 0.3 Gy, 58.6 ± 1.2 Gy, and 59.1 ± 0.7 Gy, respectively. Mean dose to PTV was lower for Tomotherapy (p 5 (p 1 (p = 0.001). CI was higher with Tomotherapyplans (p p 5 which needs more attention for toxicity analysis.

Systematic Analysis of the ArcCheck diode Arrays for Tomotherapy delivery Verification

Objective: The aim of this study was to evaluate the functional characteristics of ArcCheck? array detectors and the application for Tomotherapy IMRT plan verification. Method and Materials: The basic character of the ArcCheck applying on Tomotherapy system was tested. The diode testing procedures included the followings: The short term reproducibility, linearity, dose rate dependence, the profile measurement compared to ion chamber. The effect of insert on gamma passing rate was also studied. A total number of 602 Tomotherapy delivery Quality Assurance (dQA) plans were retrospectively analyzed to obtain the action limit results with statistical significance evaluation. Results: The short term reproducibility, linearity, dose rate dependence, profile to ion chamber correlation and clinical application were all compared with satisfactory outcome with this device. If anterior diode points were properly calibrated with machine absolute output, then there was no significant difference between the ArcCheck phantom with and without insert on gamma passing rate. The average passing rate was 97.5% and the recommend action level was established at 92% based on the 602 patients’ clinical data with 95% confidence level criteria. Conclusion: This comprehensive study shows that ArcCheck is an accurate and efficient device for quality assurance of Tomotherapy with remarkable consistency of 3d volume analysis of arc therapy.

Comparing biomarkers and proteomic fingerprints for classification studies

Early disease detection is extremely important in the treatment and prognosis of many diseases, especially cancer. Often, proteomic fingerprints and a pattern recognition algorithm are used to classify the pathological condition of a given individual. It has been argued that accurate classification of the existing data implies an underlying biological significance. Two fingerprint-based classifiers, decision tree and medoid classification algorithm, and a biomarker-based classifier were examined using a published dataset of mass spectral peaks from 81 healthy individuals and 78 individuals with benign prostate hyperplasia (BPH). For all three methods, classifiers were constructed using the original data and the data after permuting the labels of the samples (BPH and healthy). The fingerprint-based classifiers produced accurate results for the original data, though the peaks used in a given classifier depended upon which samples were placed in the training set. Accurate results were also obtained for the dataset with permuted labels. In contrast, only three unique peaks were identified as putative biomarkers, producing a small number of reasonably accurate biomarker-based classifiers. The dataset with permuted labels was poorly classified. Since fingerprint-based classifiers accurately classified the dataset with permuted labels, the argument for biological significance from a fingerprint-based classifier must be questioned.
