Two-year delay in ulcerative colitis diagnosis is associated with anti-tumor necrosis factor alpha use

BACKGROUND Ulcerative colitis(UC)is an uncommon inflammatory bowel disease(IBD).However,its incidence has recently increased in South ***,UC diagnoses are frequently delayed,and the relationship between diagnostic delay and UC prognosis has not been extensively studied in South Korean *** To identify meaningful diagnostic delay affecting UC prognosis and to evaluate risk factors associated with diagnostic delay in South Korean *** Medical records of 718 patients with UC who visited the outpatient clinic of six university hospitals in South Korea were reviewed;167 cases were excluded because the first symptom date was *** evaluated the relationship between the prognosis and a diagnostic delay of 3,6,12,18,and 24 mo by comparing the prognostic factors[anti-tumor necrosis factor(TNF)-αuse,admission history due to acute flare-ups,frequent admission due to flare-ups,surgery associated with UC,and the clinical remission state at the latest followup]at each diagnostic *** The mean diagnostic interval was 223.3±483.2 d(median,69 d;75th percentile,195 d).Among the prognostic factors,anti-TNFαuse was significantly increased after a diagnostic delay of 24 *** risk factors predictive of a 24-mo diagnostic delay were age24 mo was associated with increased anti-TNFα***<60 years at diagnosis,smoking history,and misdiagnosis of hemorrhoids were risk factors for delayed diagnosis.

Diagnostic delay in inflammatory bowel disease increases the risk of intestinal surgery

AIM To investigate the factors affecting diagnostic delay and outcomes of diagnostic delay in inflammatory bowel disease(IBD) METHODS We retrospectively studied 165 patients with Crohn's disease(CD) and 130 patients with ulcerative colitis(UC) who were diagnosed and had follow up durations > 6 mo at Korea University Ansan Hospital from january 2000 to December 2015. A diagnostic delay was defined as the time interval between the first symptom onset and IBD diagnosis in which the 76^(th) to 100^(th) percentiles of patients were *** The median diagnostic time interval was 6.2 and 2.4 mo in the patients with CD and UC, respectively. Among the initial symptoms, perianal discomfort before diagnosis(OR = 10.2, 95%CI: 1.93-54.3, P = 0.006) was associated with diagnostic delays in patients with CD; however, no clinical factor was associated with diagnostic delays in patients with UC. Diagnostic delays, stricturing type, and penetrating type were associated with increased intestinal surgery risks in CD(OR = 2.54, 95%CI: 1.06-6.09; OR = 4.44, 95%CI: 1.67-11.8; OR = 3.79, 95%CI: 1.14-12.6, respectively). In UC, a diagnostic delay was the only factor associated increased intestinal surgery risks(OR = 6.81, 95%CI: 1.12-41.4).CONCLUSION A diagnostic delay was associated with poor outcomes, such as increased intestinal surgery risks in patients with CD and UC.

Development of Preliminary Design Program for Combustor of Regenerative Cooled Liquid Rocket Engine

An integrated program was established to design a combustor for a liquid rocket engine and to analyze regenerative cooling results on a preliminary design *** of burnt gas from a kerosene-LOx mixture in the combustor and rocket performance were calculated from CEA which is the code for the calculation of chemical *** heat transfer of regenerative cooling was analyzed by using SUPERTRAPP code for coolant properties and by one-dimensional correlations of the heat transfer coefficient from the combustor liner to the *** of the combustors of F-1 and RS-27A engines were designed from similar input data and the present results were compared to actual data for ***,the combustors of 30 tonf class,75 tonf class and 150 tonf class were designed from the required thrust,combustion chamber,exit pressure and mixture ratio of *** wall temperature,heat flux and pressure drop were calculated for heat transfer analysis of regenerative cooling using the profiles.

Improved block matching approach to fast disparity estimation

An improved block matching approach to fast disparity estimation in machine vision applications is proposed, where the matching criterion is the sum of the absolute difference（SAD）.By evaluating the lower bounds, which become increasingly tighter for the matching criteria, the method tries to successively terminate unnecessary computations of the matching criteria between the reference block in one image and the ineligible candidate blocks in another *** also eliminates the ineligible blocks as early as possible, while ensuring the optimal disparity of each ***, the proposed method can further speed up the elimination of ineligible candidate blocks by efficiently using the continuous constraint of disparity to predict the initial disparity of each *** performance of the new algorithm is evaluated by carrying out a theoretical analysis, and by comparing its performance with the disparity estimation method based on the standard block *** results demonstrate that the proposed algorithm achieves a computational cost reduction of over 50.5% in comparision with the standard block matching method.

Lactic Acid Fermentation from Coffee Ground Waste Hydrolysate by Lactobacillus rhamnosus

Lactic acid is an important organic acid that is widely used in the food,pharmaceutical,and cosmetic *** acid was produced from coffee ground waste which contains fermentable sugars and is increasingly generated from our daily dietary *** 114 strains of Lactobacillus species,Lactobacillus rhamnosus ATCC 10863 was selected for the production of lactic acid from coffee ground *** alkali pretreatment and saccharification,cellulose and hemicellulose in coffee ground waste were converted into fermentable *** experiments were conducted at various alkali solution,concentrations,and *** pretreatment with 35 g/L of KOH at 121oC for 60 min,the highest concentration of fermentable sugars was *** optimum concentration of Viscozyme L was 2%when saccharification was proceeded at 55oC for 7 *** productivity of lactic acid fermentation was the highest(0.59 g/L/h)at 100 g of coffee ground waste(1x concentration),whereas the lactic acid concentration was the highest at 600 g of coffee ground waste(6x concentration).As the concentration of coffee ground increased,the lactic acid concentration was also increased,however,the amount was not proportional to the coffee ground waste *** this study,it was found that coffee ground waste could be used as a culture medium for Lactobacillus rhamnosus ATCC 10863 through pretreatment and saccharification for the production of lactic acid.

Second-line bismuth-containing quadruple therapy for Helicobacter pylori eradication and impact of diabetes

To investigate Helicobacter pylori (H. pylori) eradication rates using second-line bismuth-containing quadruple therapy and to identify predictors of eradication *** study included 636 patients who failed first-line triple therapy and received 7 d of bismuth-containing quadruple therapy between january 2005 and December 2015. We retrospectively demonstrated H. pylori eradication rates with respect to the year of therapy as well as demographic and clinical factors. H. pylori eradication was confirmed by a 13C-urea breath test or a rapid urease test at least 4 wk after the completion of bismuth-based quadruple therapy: proton pump inhibitor, metronidazole, bismuth, and *** overall eradication rates by intention-to-treat analysis and per-protocol analysis were 73.9% (95%CI: 70.1%-77.4%) and 94.5% (95%CI: 92.4%-96.5%), respectively. Annual eradication rates from 2005 to 2015 were 100.0%, 92.9%, 100.0%, 100.0%, 100.0%, 97.4%, 100.0%, 93.8%, 84.4%, 98.9%, and 92.5%, respectively, by per-protocol analysis. A multivariate analysis showed that diabetes mellitus (OR = 3.99, 95%CI: 1.56-10.20, P = 0.004) was associated with H. pylori eradication therapy *** second-line bismuth-containing quadruple therapy for H. pylori infection is still effective in Korea, and diabetes mellitus is suggested to be a risk factor for eradication failure.

Prognostic significance of the neutrophil-to-lymphocyte ratio and platelet-to-lymphocyte ratio in patients with stage Ⅲ and Ⅳcolorectal cancer

AIM To evaluate the prognostic value of the neutrophil-tolymphocyte ratio(NLR) and platelet-to-lymphocyte ratio(PLR) in patients with colorectal cancer(CRC).METHODS Between April 1996 and December 2010, medical records from a total of 1868 patients with CRC were retrospectively reviewed. The values of simple inflammatory markers including NLR and PLR in predicting the long-term outcomes of these patients were evaluated using Kaplan-Meier curves and Cox regression *** The median follow-up duration was 46 mo(interquartile range, 22-73). The estimation of NLR and PLR was based on the time of diagnosis. In multivariate Cox regression analysis, high NLR (≥ 3.0) and high PLR(≥ 160) were independent risk factors predicting poor long-term outcomes in patients with stage Ⅲ and Ⅳ CRC. However, high NLR and high PLR were not prognostic factors in patients with stage Ⅰ and Ⅱ *** In this study, we identified that high NLR (≥ 3.0) and high PLR (≥ 160) are useful prognostic factors to predict long-term outcomes in patients with stage Ⅲ and Ⅳ CRC.
