Objective: To determine the feasibility and safety of skeletal myoblast transp lantation in patients with chronic myocardial infarction undergoing coronary art ery bypass grafting. Methods: Twelve patients with a previous myocardial infarct ion and ischemic coronary artery disease underwent treatment with coronary arter y bypass grafting surgery and intramyocardial injection of autologous skeletal m yoblasts cultured with autologous serum. Global and regional cardiac function wa s assessed by echocardiogram. fluorine 18 fluorodeoxyglucose and nitrogen 13-am monia positron emission tomography studies were used to determine cardiac viabil ity and perfusion. A group of historical control patients(n=14) treated with coronary artery bypass grafting surgery wi thout myoblast transplantation was analyzed. Results: The left ventricular eject ion fraction improved from 35.5%±2.3%(mean±SEM) before surgery to 55.1%±8. 2%at 12 months(Pfrom 33.6%±9.3%to 38.6%±1 1%in the control group. Regional contractility also improved in the myoblast gr oup, particularly in cardiac segments treated with skeletal myoblasts(wall motio n score index: 3.02±0.17 at baseline vs 1.36±0.14 at 12 months; Pfluorine 18-fluorodeoxyglucose and nitrogen 13-ammonia positron emis sion tomography showed an increase in viability and perfusion 12 months after su rgery both globally and in segments treated with myoblasts(P=.012 and P=.004). S keletal myoblast implantation was not associated with adverse events or an incre ased incidence of cardiac arrhythmias. Conclusions: In patients with previous my ocardial infarction, treatment with skeletal myoblasts in conjunction with coron ary artery bypass is safe and feasible and is associated with an increased globa l and regional left ventricular function, improvement in viability, and perfusio n of cardiac tissue and no significant incidence of arrhythmias.

Probability to retrieve testicular spermatozoa in azoospermic patients

Aim: The degree of probability to retrieve spermatozoa from testicular tissue for intracytoplasmic sperm injection intooocytes is of interest for counselling of infertility patients. We investigated the relation of sperm retrieval to clinical dataand histological pattern in testicular biopsies from azoospermic patients. Methods: In 264 testicular biopsies from142 azoospermic patients, the testicular tissue was shredded to separate the spermatozoa, histological semi-thin sec-tions of which were then evaluated using johnsen score. Results: The retrieval of spermatozoa correlated signifi-cantly (P fSH concentration ( r = -0.66), the maximum score(r = 0.85) and the mean johnsen score (r = 0.81). In the multivariate regression analysis the successful testicularsperm extraction showed the closest relationship to the maximum score. The testicular volume correlated significantlywith the mean johnsen score ( r = 0.64, P fSH concentration mainly with the maxi-mum score ( r = -0.77; P ficantly lower johnsenscore compared to the patients who did not have any testicular disease in the past (3.7 ± 2.4 vs. 5.9 ± 2. 5; P fSH concentration in serum were related to thejohnsen score which correlated significantly with the sperm retrieval. The successful sperm retrieval can be expected inall azoospermic patients irrespective of the results of clinical examination. However, the probability of retrieval of sper-matozoa decreased significantly in patients with a fSH level > 18 U/L, tesficular volume johnsen scorejohnsen score < 7.

To assess the performance and clinical value of the postwash total motile sper m count (postwash TMC) as a test to predict intrauterine insemination (IUI) outc ome. Meta-analysis of diagnostic tests. Tertiary fertility center. Patients und ergoing IUI. We detected 16 studies that reported on postwash TMC at inseminatio n and IUI outcome. Summary receiver operating characteristics (ROC) curves indic ated a reasonable predictive performance toward IUI outcome, and, at cut-off le vels between 0.8 to 5 million motile spermatozoa, the postwash TMC provided a su bstantial discriminative performance. At these cut-off levels, the specificity of the postwash TMC, defined as the ability to predict failure to become pregnan t, was as high as 100%; the sensitivity of the test, defined as the ability to predict pregnancy, was limited. The postwash TMC at insemination could potential ly be used in counseling patients for either IUI or IVf. However, to enable pati ent counseling before the start of treatment, further studies are needed to esta blish the accuracy of a postwash TMC during the fertility workup rather than at insemination.
