Impact of cytochrome P450 2D6 polymorphisms on decision-making and clinical outcomes in adjuvant hormonal therapy for breast cancer

BAcKGROUND There are concerns that tamoxifen is less effective in Asian women because of the high prevalence of impaired function cytochrome P4502D6(cYP2D6)*** To evaluate how knowledge of cYP2D6 genotype impacted the choice of hormonal agent and how cYP2D6 genotype and agent were associated with clinical *** Eighty-two women were ***-eight completed cYP2D6 genotyping and were categorized into poor,intermediate(IM)and extensive or ultra metabolizer *** with poor metabolizer and IM phenotypes were recommended aromatase inhibitors as the preferred *** More than 70%of the women had an IM phenotype,32%an extensive or ultra metabolizer phenotype,and 0%had a poor metabolizer *** of genotype,more women opted for aromatase ***,80%of women completed 5 years of hormonal *** women developed recurrence,3 contralateral breast cancer,5 died,and 1 was diagnosed with a second primary ***-year recurrence-free and overall survival were slightly better in women with the extensive or ultra metabolizer phenotype compared to those with the IM phenotype,though not statistically significant[P=0.743,hazard ratio(HR):1.441,95%confidence interval(cI):0.191 to 10.17 and P=0.798,HR:1.327,95%cI:0.172 to 9.915,respectively].Women receiving aromatase inhibitors also appeared to have a better,but also nonsignificant,5-year recurrence-free and overall survival(P=0.253,HR:0.368,95%cI:0.031 to 0.258 and P=0.292,HR:0.252,95%cI:0.005 to 4.951,respectively).cONcLUSION The IM phenotype was highly prevalent but was not associated with clinical outcome.

Objective: Lack of effective therapies remains a problem in the treatment of oral squamous cell carcinoma(OScc), especially in patients with advanced tumors. OScc development is driven by multiple aberrancies within the cell cycle pathway, including amplification of cyclin D1 and loss of p16. Hence, cell cycle inhibitors of the cDK4/6-cyclin D axis are appealing targets for OScc treatment. Here, we determined the potency of palbociclib and identified genetic features that are associated with the response of palbociclib in ***: The effect of palbociclib was evaluated in a panel of well-characterized OScc cell lines by cell proliferation assays and further confirmed by in vivo evaluation in xenograft models. PIK3cA-mutant isogenic cell lines were used to investigate the effect of PIK3cA mutation towards palbociclib ***: We demonstrated that 80% of OScc cell lines are sensitive to palbociclib at sub-micromolar ***, palbociclib was effective in controlling tumor growth in mice. We identified that palbociclib-resistant cells harbored mutations in PIK3cA. Using isogenic cell lines, we showed that PIK3cA mutant cells are less responsive to palbociclib as compared to wild-type cells with concurrent upregulation of cDK2 and cyclin E1 protein levels. We further demonstrated that the combination of a PI3 K/mTOR inhibitor(PF-04691502) and palbociclib completely controlled tumor growth in ***: This study demonstrated the potency of palbociclib in OScc models and provides a rationale for the inclusion of PIK3cA testing in the clinical evaluation of cDK4/6 inhibitors and suggests combination approaches for further clinical studies.

We report the discovery of KMT-2020-BLG-0414 Lb,with a planet-to-host mass ratio q2=0.9-1.2×10-5=3-4 q⊕at 1σ,which is the lowest mass-ratio microlensing planet to *** with two other recent discoveries(4?q/q⊕?6),it fills out the previous empty sector at the bottom of the triangular(log s,log q)diagram,where s is the planet-host separation in units of the angular Einstein radiusθ***,these discoveries call into question the existence,or at least the strength,of the break in the mass-ratio function that was previously suggested to account for the paucity of very low-q *** to the extreme magnification of the event,Amax~1450 for the underlying single-lens event,its light curve revealed a second companion with q3~0.05 and|log s3|~1,i.e.,a factor~10 closer to or farther from the host in *** measurements of the microlens parallaxπE and the angular Einstein radiusθE allow estimates of the host,planet and second companion masses,(M1,M2,M3)~(0.3 M⊙,1.0 M⊕,17 Mj),the planet and second companion projected separations,(a⊥,2,a⊥,3)~(1.5,0.15 or 15)au,and system distance DL~1 *** lens could account for most or all of the blended light(I~19.3)and so can be studied immediately with high-resolution photometric and spectroscopic observations that can further clarify the nature of the *** planet was found as part of a new program of high-cadence follow-up observations of high-magnification *** detection of this planet,despite the considerable difficulties imposed by cOVID-19(two KMT sites and OGLE were shut down),illustrates the potential utility of this program.

Dear Editor,Plant tissue culture involves callus formation and de novo shoot ***,explants from differentiated tissues are used to generate a pluripotent cell mass,called callus,on auxin-rich callus-inducing medium(cIM),followed by shoot regeneration on cytokinin-rich shoot-inducing medium(SiM).callus results from division of pericycle-like cells(Atta et al.,2009;Sugimoto et al.,2010);its cellular identity resembles that of lateral root primordia(Atta et al.,2009;Sugimoto et al.,2010).callus acquires cellular pluripotency by forming root stem cell niches on cIM(Sugimoto et al.,2010)。

Objective: To describe the clinical and demographic characteristics of nonneuronopathic Gaucher disease (GD) in children at the time of diagnosis. Design: Longitudinal observational data base of the International collaborative Gaucher Group Gaucher Registry. Setting: Data reported to the Registry from january 1, 1989, to june 3, 2005, were included in this report. Patients/Participants: All 887 patients were diagnosed as having nonneuronopathic GD from birth to younger than 18 years and did not receive enzyme replacement therapy. Main Outcome Measures: Eight measures of the clinical manifestations and demographics of nonneuronopathic GD. Results: The most common signs and symptoms noted were splenomegaly (95%),hepatomegaly (87%), radiologic bone disease (81%), thrombocytopenia(50%), anemia (40%), growth retardation (34%),bone pain (27%), and bone crisis (9%). Anemia and more severe splenomegaly and hepatomegaly were observed more frequently in younger patients. Skeletal manifestations were found more often in older children. Only 23%were identified as Ashkenazi jews. conclusions: Nonneuronopathic GD commonly manifests in childhood and affects many ethnic *** high prevalence of rare mutations may be associated with earlier onset and/or more severe disease. Increased awareness of the clinical and demographic characteristics of nonneuronopathic GD in children may improve early recognition of this treatable lysosomal storage disorder, decrease morbidity, and prevent irreversible sequelae.

The utility of air-contrast barium enema and colonoscopy for evaluation of the colon has been debated. Air-contrast barium enema is less expensive and invasive than colonoscopy, but it also is less sensitive and specific. Further, although air-contrast barium enema may be less painful than colonoscopy, it often is poorly tolerated by patients. Thus, this study compared the sensitivity and the specificity of air-contrast barium enema and colonoscopy for detection of colonic lesions in patients with fecal occult blood. Over a 30-month period, patients with fecal occult blood were recruited. Patients underwent standard air-contrast barium enema, followed by colonoscopy 7 to 14 days later. colonoscopists were blinded to the results of aircontrast barium enema until the colonoscopy was completed, after which the results were disclosed. If the findings were discrepant, colonoscopy was repeated. A total of 100 patients were evaluated. Nine air-contrast barium enemas were reported to be inadequate, and the cecum was not intubated at colonoscopy in two patients. In the remaining patients, 5 cancers were identified (1 each cecum, transverse colon, descending colon, sigmoid colon, and rectum) by both studies. Sixty-six polypoid lesions were identified in 30 patients. Diverticula were identified in 42 patients by air-contrast barium enema and in 18 patients by colonoscopy. Air-contrast barium enema detected 3 of 36 polypoid lesions 5 mm or less in diameter, 5 of 15 adenomas 6 to 9 mm in size, and 4 of 15 adenomas 10 mm or greater in diameter (sensitivity 8%, 33%, and 27%, respectively). After excluding patients with diverticula, air-contrast barium enema detected 3 of 7 adenomas 10 mm or greater in size. Overall, 12 polypoid lesions or filling defects were identified by air-contrast barium enema that could not be verified by colonoscopy. The specificity of air-contrast barium enema for lesions 1.0 cm or greater in size was 100%; for those 6 mm or greater, it was 97%. Air-contra
