Distinct risk factors for early and late blood transfusion following pancreaticoduodenectomy

Background: The International Study Group of Pancreatic Surgery(ISGPS) has defined two periods of postpancreatectomy hemorrhage, early(24 h). A previously published Blood Usage Risk Score(BURS) aimed to predict early and late blood transfusion. The primary aim of this study was to define risk factors for early and late blood transfusion after pancreaticoduodenectomy. Secondary aims were to assess the predictive accuracy of the ***: In this retrospective observational study, multivariable analyses were used to identify independent risk factors for both early and late blood transfusion. The predictive ability of the BURS was then assessed using a receiver operating characteristic(ROC) curve ***: Among 628 patients, 99(15.8%) and 144(22.9%) received early and late blood transfusion, respectively. Risk factors for blood transfusion differed between early and late periods. Preoperative anemia and venous resection were associated with early blood transfusion whilst Whipple’s resection(as opposed to pylorus preserving pancreaticoduodenectomy), lack of biliary stent and a narrow pancreatic duct were predictors of late blood transfusion. The BURS was significantly predictive of early blood transfusion,albeit with a modest degree of accuracy(AUROC: 0.700, P < 0.001), but not of late blood transfusion(AUROC: 0.525, P = 0.360). Late blood transfusion was independently associated with increasing severity of postoperative pancreatic fistula(POPF)(OR: 1.85, 3.18 and 9.97 for biochemical, types B and C POPF,respectively, relative to no POPF).Conclusions: Two largely different sets of variables are related to early and late blood transfusion following pancreaticoduodenectomy. The BURS was significantly associated with early, albeit with modest predictive accuracy, but not late blood transfusion. An understanding of POPF risk allows assessment of the need for late blood transfusion.

Lymphoepithelial cysts of the pancreas:a management dilemma

Pancreatic lymphoepithelial cysts（LECs） are rare,benign lesions that are typically unexpected post-operative pathological findings. We aimed to review clinical, radiological and pathological features of LECs that may allow their preoperative diagnosis. Histopathology databases of two large pancreatic units were searched to identify LECs and notes reviewed to determine patient demographic details, mode of presentation, investigations, treatment and outcome. Five male and one female patients were identified. Their median age was 60 years. Lesions were identified on computed tomography performed for abdominal pain in two patients, and were incidentally observed in four patients. Five LECs were located in the tail and one in the body of the pancreas, with a median cyst size of 5 cm. Obtaining cyst fluid was difficult and a largely acellular aspirate was yielded. The pre-operative diagnosis was mucinous cystic neoplasm in all patients. This series of patients were treated distal pancreatectomy and splenectomy. A retrospective review of radiological examinations suggested that LECs have a relatively low signal on T2 imaging and a high signal intensity on T1 weighted images. LECs appear more common in elderly males, and are typically incidental, large, unilocular cysts. Close attention to signal intensity on MRI may allow preoperative diagnosis of these lesions.

AIM: To investigate modulation of antagonist and agonist binding to adenosine A1receptors by MgCl2and 5'-guanylimidodiphosphate (Gpp（NH）p) using rat brain membranes and the A1antagonist [3H]-8-cyclopentyl-1,3-dipropylxanthine （[3H]DPCPX） and the A1agonist [3H]-2-chloro-N6-cyclopentyladenosine （[3H]CCPA）. METHODS: Parallel saturation and inhibition studies were performed using well-characterised radioligand binding assays and a Brandel Cell Harvester. RESULTS: MgCl2produced a concentration-dependent decrease （44%）, whereas Gpp（NH）p increased [3H]DPCPX binding （19%）. In [3H]DPCPX competition studies, agonist affinity was 1.5-14.6-fold higher and 4.6-10-fold lower in the presence of 10 mmol/L MgCl2and 10 μmol/L Gpp（NH）p respectively; antagonist affinity was unaffected. The decrease in agonist affinity with increasing Gpp（NH）p concentrations was due to a reduction in the proportion of binding to the high affinity receptor state. Osmolyte transport in Staphylococcus aureus and the role in pathogenesis

Osmolyte transport is a pivotal part of bacterial life, particularly in high salt environments. Several low and high affinity osmolyte transport systems have been identified in various bacterial species. A lot of research has centered on characterizing the osmolyte transport systems of Gram-negative bacteria, but less has been done to characterize the same transport systems in Gram-positive bacteria. This review will focus on the previous work that has been done to understand the osmolyte transport systems in the species Staphylococcus aureus and how these transporters may serve dual functions in allowing the bacteria to survive and grow in a variety of environments, including on the surface or within humans or other animals.

Further compatibility tests of the entomopathogenic fungus Lecanicillium muscarium with conventional insecticide products for control of sweetpotato whitefly, Bemisia tabaci on poinsettia plants

The effect on spore germination of the entomopathogenic fungus Lecanicillium muscarium following direct exposure for 24 h to the insecticides Majestik, Spray Oil, Agri- 50E, Savona and Oberon for the control of both egg and second instar stages of the sweetpotato whitefly, Bemisia tabaci, was determined. Exposure to both Agri-50E and Oberon was followed by acceptable spore germination. Infectivity rates ofL. muscarium on poinsettia foliage in the presence of dry residues of the insecticides were also investigated. No significant detrimental effects on the levels of control of B. tabaci were recorded compared with fungus applied to residue-free foliage. Sequential application of the chemicals Savona, Spray Oil and Majestik with the fungus all produced mortalities of second instar B. tabaci above 90%. Incorporation of these chemicals with L. muscarium into integrated control programs for B. tabaci is discussed.

Surface-Driven High-Pressure Processing

The application of high pressure favors many chemical processes, providing higher yields or improved rates in chemical reactions and improved solvent power in separation processes, and allowing activation barriers to be overcome through the increase in molecular energy and molecular collision rates. High pressures-up to millions of bars using diamond anvil cells-can be achieved in the laboratory, and lead to many new routes for chemical synthesis and the synthesis of new materials with desirable thermody- namic, transport, and electronic properties. On the industrial scale, however, high-pressure processing is currently limited by the cost of compression and by materials limitations, so that few industrial processes are carried out at pressures above 25 MPa. An alternative approach to high-pressure processing is pro- posed here, in which very high local pressures are generated using the surface-driven interactions from a solid substrate. Recent experiments and molecular simulations show that such interactions can lead to local pressures as high as tens of thousands of bars （1 bar=1×10^5 Pa）, and even millions of bars in some cases. Since the active high-pressure processing zone is inhomogeneous, the pressure is different in dif- ferent directions. In many cases, it is the pressure in the direction parallel to the surface of the substrate （the tangential pressure） that is most greatly enhanced. This pressure is exerted on the molecules to be processed, but not on the solid substrate or the containing vessel. Current knowledge of such pressure enhancement is reviewed, and the possibility of an alternative route to high-pressure processing based on surface-driven forces is discussed. Such surface-driven high-pressure processing would have the advantage of achieving much higher pressures than are possible with traditional bulk-phase processing, since it eliminates the need for mechanical compression. Non-traumatic osteonecrosis is a cause of joint pain and deformity, not uncommonly caused by corticosteroid use. Despite this, corticosteroid induced osteonecrosis is poorly represented in the literature. We performed a detailed review of corticosteroid induced osteonecrosis, including aetiology, prevention, screening, clinical presentation, investigations, staging systems and treatment. We present this in the context of a patient at our institution who developed bilateral hip, shoulder and knee osteonecrosis following high dose corticosteroid therapy for dermatomyositis.
